Intensification of chemotherapy is one of the possibilities that we have to improve treatment results in small cell lung cancer. It can be obtained by various ways: increasing the number of active drugs of a combination or the dosage of the agents given, reducing interval between chemotherapy courses or increasing the total duration of therapy. If the addition of a drug to an active combination, the increase of drug dosages or the administration of megadoses of chemotherapy with or without bone marrow transplantation have not improved survival, prolongation of therapy seems more effective, at least in responders. Weekly chemotherapy schedules and hematopoietic growth factors might change today's results.

The analysis of 265 selected cases of right colon cancer treated over a long period of time allows a number of points to be raised. Attention is drawn to the lack of progress made in diagnostic techniques, but this is thought to be due primarily to the vagueness of symptoms and their scarse investigation rather than to limits imposed by instrumental techniques. Moreover, no real benefit would be produced by a more aggressive approach using visceral or lymphatic exeresis. An earlier diagnosis of tumours due to differentiation, slow growth and rare association with adenoma should allow more encouraging results to be achieved.

Data on human lung histamine H1- and H2-receptors in cancer and chronic inflammatory processes are reported. It has been found that the number of histamine H1-receptors significantly increases both in cancer and chronic pneumonia and does not practically change in tuberculosis lung parenchyma. The binding parameters of histamine H2-receptors both in cancer and inflammatory processes were similar to those obtained for the normal tissue. The important role of parenchymal histamine H1-receptors in the neuromodulation of airways in human lung adenocarcinoma is discussed.

Patients (186) with locally advanced or metastatic breast cancer were treated with the aromatase inhibitor 4-hydroxyandrostenedione given parenterally at 3 different doses. 21% of patients responded to treatment, 93% of objective responders whose oestrogen receptor (ER) status was known had ER positive tumours. The drug was well-tolerated particularly at a dose of 250 mg i.m. every fortnight. At this dose, only 4/96 (4%) patients had to discontinue treatment. We conclude that 4-hydroxyandrostenedione is a well-tolerated form of endocrine treatment for postmenopausal patients with breast cancer.

Prostatic cancer is an unusual malignancy because, commonly, it is slowly progressive and occurs at an age where the patient's life expectancy is short as a result of considerable competing mortality rates from other diseases. In addition, the place of radical surgery, i.e., radical prostatectomy is still not defined completely or established uniformly. METHODS. This article attempts to offer comparisons of the initial forms of primary therapy and to determine the way in which adjuvant therapy may be tested and its results assessed. RESULTS AND CONCLUSION. Additional information should come from large-scale randomized clinical trials involving several hundred patients in each treatment arm.

In order to investigate the practice for control of patients with colorectal adenomata and cancer, a questionnaire investigation was carried out in 1990-1991 among 53 Danish surgical departments. The investigation revealed great differences in the control programmes offered to these patients as regards the methods, the intensity and duration. In addition, differences were present in the control pattern for adenomata in specialist departments as compared with the remaining departments where there was a tendency to very frequent but, on the other hand, brief control. On the basis of these results, it is considered necessary that a reference programme should be prepared on this subject.

Localization of epidermal growth factor receptor (EGFR) in hepatocellular carcinoma was examined by means of immunohistochemistry. It's localization was observed at plasma membrane of the cancer cells. EGFR was found to be expressed in 7 out of 17 hepatocellular carcinoma. No significant difference was observed between EGFR-positive and EGFR-negative cases in tumor size. AFP, macroscopic and histopathological classification, capsular invasion, and portal invasion. However, in EGFR-positive cases, the number of DNA polymerase-alpha positive cells and recurrence rate were higher than that in EGFR-negative cases. These results suggest that EGFR may play an important role in the development and progression of hepatocellular carcinoma.

The occurrence of cancer in 46 couples related only by marriage and treated in a regional hospital is reported. Twenty-four pairs had tumors of the same histopathologic type. Three couples had the same tumors with similar histology: brain (astrocytoma), urinary bladder (transitional cell carcinoma), and lung (adenocarcinoma). The possible significance of environmental, nutritional, and contamination factors are emphasized, although this is a probably coincidental phenomenon.

The retinoblastoma gene is mutated in several types of human cancer and is the best characterized of the tumour-suppressor genes. A mouse strain has been constructed in which one allele of Rb is disrupted. These heterozygous animals are not predisposed to retinoblastoma, but some display pituitary tumours arising from cells in which the wild-type Rb allele is absent. Embryos homozygous for the mutation die between days 14 and 15 of gestation, exhibiting neuronal cell death and defective erythropoiesis.

Due to the preference of the present multimodality treatment of breast cancer, we have found the need for a more customized approach to our patient set-up position. Using Smithers Medical Alpha Cradle Kits (AC 325) we have been able to achieve individual casts for our physically challenging patients. The results are a comfortable, easily reproducible position for a technically involved isocentric set-up which requires little additional simulation time and allows the majority of the allotted time for physics measurements and patient interaction. Examples of patient set-ups and consistent treatment field verification are discussed in the following pages.

Distribution of secretory beta-globulin (S beta G) which possesses affinity for steroids was investigated immunohistochemically. Tissue specificity of S beta G, produced in adult secretory epithelial cells of the seminal vesicles, salivary glands, prostate, bronchi and mammary gland was discovered. The protein was not detected in fetal and embryonal tissues. S beta G synthesis is abnormal in neoplasms: its expression partly preserves in breast cancer cells and increases in epithelium of mammary ducts near the focus of malignancy. In lung cancer and bronchial glands cells near the focus of neoplastic transformation S beta G positive reaction was not observed.

TITLE: Hypocholesterolaemic factor from gallbladder cancer cells.

A patient is reported with drug-resistant familial hypercholesterolaemia in whom serum cholesterol fell after the onset of gallbladder cancer with intraperitoneal invasion. Cancer cells were obtained from ascitic fluid and a cultured cell line established. Incubation of the culture medium of these cells with skin fibroblasts from the patient and from normal subjects increased low-density lipoprotein (LDL) receptor activity in a dose-dependent manner. These results show that gallbladder cancer cells from this patient secrete a substance that stimulates LDL receptor activity. This substance may have contributed towards reduction of serum cholesterol.

The cancer's signal intensity was greater than the fibrotic lung tissue's intensity in an ECG-gated image with relatively short repetition and echo times and, also, in images with long repetition and echo times.
