TITLE: Ultrasonic evaluation of cervical lymph node metastasis of squamous cell carcinoma in oral cavity.

Forty-four patients with oral cancer were examined by ultrasonography to detect cervical lymph nodal metastases. The lymph nodes, which were histopathologically or clinically confirmed to have cancer metastases, showed the following characteristic echogram findings: distinctive nodal boundary, posterior echo enhancement and an increased ratio of the shortest and the longest diameters of the node. It is suggested that these three peculiar ultrasonographic findings are considered to be important items for the differential diagnosis of benign nodes from the cervical metastasis of oral cancer.

Serum alpha-fetoprotein (AFP) was positive in as few as 5.7-13.5% of gastric cancer patients prior to surgery, and most of the positive cases had cancer of stage IV or higher. Thus, the parameter did not seem to be very useful in diagnosis. It was positive in as many as 45% of terminal cancer patients, while many cases with AFP of more than 1000 ng/ml had metastases to the liver. Thus, AFP was proved to be useful for follow-up of the postoperative course. AFP staining was conducted in 29 dissected stomachs and five of them were positive for AFP staining.

High-dose intravenous estrogen therapy was shown to be effective in relieving bone pain due to metastatic disease in 22 of 29 (75.9%) men with advanced hormone-resistant prostate cancer. This clinical response was accompanied by significant falls in serum prostate-specific antigen (PSA) levels in 13 (44.8%) patients. It is suggested that this clinical benefit is due to a direct inhibitory effect of estrogen on prostate cancer cells.

From Dec. 1979 to Dec. 1984, 171 patients suspicious of primary liver cancer (PLC) were examined by fine needle aspiration cytology (FNAC). In this series, 148 patients were finally diagnosed as PLC. By FNAC, 133 of 148 patients showed positive cytology and 4 suspiciously positive cytology. The overall diagnostic accuracy was 92.6% There was no false positive. Neither spreading or bleeding were observed. The results show that FNAC examination, being simple, rapid and safe, could serve as a reliable way of diagnosing the suspected PLC.

Much has been accomplished in the cure of cancer with chemotherapy, but much remains to be done. The primary care physician's responsibility is to know which cancers are curable and in which a significant portion of patients may benefit from chemotherapy. Patients with a malignancy in a relatively resistant organ site represent the greatest challenge to the "art" of medicine. Even if more than half of patients with a given malignancy are cured by chemotherapy, the remainder deserve the right to participate in research protocols, an undertaking that might further improve these otherwise substantial gains. As research studies show improved cure rates with antineoplastic agents, the need to reduce both short- and long-term side effects will remain a challenge.

In May 1987 and 1988, the American Cancer Society (ACS) sponsored Breast Cancer Detection Awareness (BCDA) programs throughout Wisconsin. These programs were made possible by the cooperative efforts of many ACS volunteers, hospitals, clinics, and physicians. The Wisconsin Radiological Society and the Division of Health were also involved in the planning efforts. In 1987, a total of 10,207 women were screened at 100 facilities, and 75 carcinomas were detected. In 1988, of 8,802 women screened at 102 facilities, 52 carcinomas were detected.

Inherent cellular radioresistance and repair of x-ray damage was studied in 19 early-passage squamous cell carcinoma lines derived from head- and neck-cancer patients with known clinical results following radiotherapy. Human tumor cells that were radioresistant and/or proficient in accumulation/repair of x-ray damage were cultured from patients unsuccessfully treated with radiotherapy. Thus, the presence of radiation-resistant and repair-proficient tumor cells was associated with clinical radiation failure, suggesting the possibility of a predictive assay based on in vitro radiobiological parameters.

Twenty-one evaluable patients with advanced squamous-cell cervical cancer were treated with aminothiadiazole at a dosage of 125 mg/m2 weekly. Nineteen had prior chemotherapy. One patient had a partial response; six had stable disease; 14 had increasing disease; and two were unevaluable for response. There was a single life-threatening toxic episode. Aminothiadiazole used in this dosage and schedule has minimal activity in previously treated cervical carcinoma patients.

In this paper, correlations between ischaemic heart disease and several cancers associated with cigarette smoking are examined. Also, tobacco consumption data are correlated against ischaemic heart disease mortality incidence rates. Furthermore, correlations between ischaemic heart disease incidences and per capita consumption of fattening foods (meat, milk and eggs) are analyzed. In this work, results referring to Spain, and at an ecologic level, confirm conclusions of observational and experimental studies carried out in other countries, which points out a strong association between ischaemic heart disease, cigarette consumption and fattening diet.

The utilization of radical operation in gastric cancer cases has led to the development of various operation techniques. With the improvement in perioperative management it can be said that age no longer is a restriction. A study of gastric cancer prognosis indicates an upward trend in survival since 1960 along with the increased detection of early cancer. This suggests the most importance of early detection in increasing the therapeutic results obtained in cases of gastric cancer. In cases with advanced cancer, 5-year survival rate of 44.7 per cent was attained through such aggressive actions as extended dissection of lymph nodes and combined resection of surrounding organs with evaluation of the macroscopic classification of the cancer.

Eleven patients ranging in age from 52 to 80 years, undergoing treatment for cancer with various preparations of interferon received neurobehavioral evaluations after experiencing unexpectedly severe organic mental disorders. The reactions ranged from delirium to extrapyramidal symptoms, mania, and neurasthenia with catatonic episodes. Computed tomographic (CT) scans of the brain disclosed unsuspected pre-existing neurologic abnormalities in all patients, including cerebral atrophy (6/11), brain metastases (4/11), and evidence of head injury incurred 40 years earlier (1/11). These findings suggest that cancer patients with pre-existing neurologic dysfunctions are at increased risk for severe interferon neurotoxicity.

In a cohort study of 5464 union organised Swedish chimney sweeps employed at any time between 1918 and 1980 mortality was studied from 1951 to 1982 with national statistics used as a reference. Follow up was possible for 98.6% of the individuals: 717 deaths were observed against 540 expected. There was an increased mortality from coronary heart disease, respiratory diseases, and several types of malignant tumours. Lung cancer mortality was significantly increased and positively correlated to the number of years employed. A fivefold risk increase for oesophageal cancer and liver cancer was found. The increased mortality could be attributed to exposure to combustion products in the work environment but not to smoking habits.
