TITLE: Ultrasonic evaluation of cervical lymph node metastasis of squamous cell carcinoma in oral cavity.

Forty-four patients with oral cancer were examined by ultrasonography to detect cervical lymph nodal metastases. The lymph nodes, which were histopathologically or clinically confirmed to have cancer metastases, showed the following characteristic echogram findings: distinctive nodal boundary, posterior echo enhancement and an increased ratio of the shortest and the longest diameters of the node. It is suggested that these three peculiar ultrasonographic findings are considered to be important items for the differential diagnosis of benign nodes from the cervical metastasis of oral cancer.

Serum alpha-fetoprotein (AFP) was positive in as few as 5.7-13.5% of gastric cancer patients prior to surgery, and most of the positive cases had cancer of stage IV or higher. Thus, the parameter did not seem to be very useful in diagnosis. It was positive in as many as 45% of terminal cancer patients, while many cases with AFP of more than 1000 ng/ml had metastases to the liver. Thus, AFP was proved to be useful for follow-up of the postoperative course. AFP staining was conducted in 29 dissected stomachs and five of them were positive for AFP staining.

A new system--the Australian Clinico-pathological Staging (ACPS) System--is proposed for the reporting of cases of colorectal adenocarcinoma. It utilizes all information available--clinical, operative and pathological--before a stage is allotted. This contrasts with Dukes' classification which is based solely on the pathological examination of the resected specimen. The ACPS has a special category 'D' for incurable cancer--those with a locally advanced irremovable tumour or with distant metastases. This method requires co-operation between clinician and pathologist and the use of precise terminology with strict adherence to definitions. The validity of the system is tested by analysing the survival pattern of 709 patients from Concord Hospital according to the ACPS and Dukes' system.

TITLE: The subrenal capsule assay in mice. Prediction of rat tumor sensitivity to chemotherapy.

The predictive value of the subrenal capsule (SRC) assay for cancer chemotherapy was tested in five rat tumors. Tumor-bearing rats were used as controls. In order to simulate clinical chemotherapy, different dose regimens and drug combinations were used. All five tumors were evaluable in in the SRC assay. Taking into account cytotoxic drugs, 86% compatibility was obtained between the SRC assay and the rat model. Antitumor effect of three antiestrogenic compounds could not be predicted by the SRC assay. It is concluded that the SRC assay can be expected to have predictive value in the assessment of tumor sensitivity to single and combination chemotherapy based on experimental animal models.

High-dose intravenous estrogen therapy was shown to be effective in relieving bone pain due to metastatic disease in 22 of 29 (75.9%) men with advanced hormone-resistant prostate cancer. This clinical response was accompanied by significant falls in serum prostate-specific antigen (PSA) levels in 13 (44.8%) patients. It is suggested that this clinical benefit is due to a direct inhibitory effect of estrogen on prostate cancer cells.

Tamoxifen, a non-steroidal antiestrogen, is currently used to treat post-menopausal breast cancer. In estrogen-sensitive breast cancer cell lines, tamoxifen and some of its metabolites display, for the induction of estrogen-regulated proteins, a partial agonist/antagonist activity. They also inhibit totally the proliferation of these cells. Evidence is given, which suggests that the pharmacological properties of these antiestrogens are mediated by the estrogen receptor. These antiestrogens interact with the estrogen receptor but they appear to induce only a partial or altered activation of the receptor. This partial or altered activation and the competition with estrogens for binding to the estrogen receptor could explain the estrogen agonist/antagonist properties of antiestrogens.

Abscess of the psoas, which is a well recognized complication in tuberculous osteoarticular infections, can also occur in certain regional diseases of the bones or viscera: common germ osteitis, diverticulosis, colic cancer, appendicitis and, above all, Crohn disease. Abscess of the psoas can also present as an apparently primary pyogenic muscular infection. A case is reported of an abscess of the psoas due to Staphylococcus aureus, which occurred spontaneously in an apparently healthy 50-year-old woman, was cured by surgical drainage, and whose course confirmed its primary nature.

In the early 1950s the increase of antibiotic-resistant strains of Staphylococcus aureus in hospitalized patients with infection stimulated a screening program to develop an effective new agent. A soil sample found in the jungles of Borneo contained a microorganism, Streptomyces orientalis, that produced a substance later called vancomycin. Vancomycin was shown to have antistaphylococcal activity and to be relatively safe when administered to patients. With the resurgence of staphylococcal infections resistant to the newer antistaphylococcal antibiotics, there has been a corresponding increase in the use of vancomycin. Vancomycin is also being used for prophylaxis in patients with reduced renal function undergoing dialysis, for the treatment of patients with antibiotic-induced enterocolitis, and in combination with other antibiotics for 'sterilization' of the intestinal tract in patients with cancer.

From Dec. 1979 to Dec. 1984, 171 patients suspicious of primary liver cancer (PLC) were examined by fine needle aspiration cytology (FNAC). In this series, 148 patients were finally diagnosed as PLC. By FNAC, 133 of 148 patients showed positive cytology and 4 suspiciously positive cytology. The overall diagnostic accuracy was 92.6% There was no false positive. Neither spreading or bleeding were observed. The results show that FNAC examination, being simple, rapid and safe, could serve as a reliable way of diagnosing the suspected PLC.

Much has been accomplished in the cure of cancer with chemotherapy, but much remains to be done. The primary care physician's responsibility is to know which cancers are curable and in which a significant portion of patients may benefit from chemotherapy. Patients with a malignancy in a relatively resistant organ site represent the greatest challenge to the "art" of medicine. Even if more than half of patients with a given malignancy are cured by chemotherapy, the remainder deserve the right to participate in research protocols, an undertaking that might further improve these otherwise substantial gains. As research studies show improved cure rates with antineoplastic agents, the need to reduce both short- and long-term side effects will remain a challenge.

Each year, it is estimated, more than 500,000 new cases of nonmelanoma skin cancer develop. The majority of these cutaneous neoplasms are treated by various modalities that include excision, electrodesiccation and curettage, cryosurgery, and irradiation, with greater than 90% success. Certain of the remaining primary tumors, as well as recurrent carcinomas, present a demanding therapeutic challenge. For these lesions, Mohs surgery has evolved as the most reliable and cost-effective treatment modality, offering maximal preservation of normal tissue and therefore the lowest functional and cosmetic morbidity. In this review, the history and evolution of Mohs surgery and the technique itself will be briefly outlined. The indications for Mohs surgery, together with methods of handling the postoperative wound and future applications of the technique, will be discussed in detail.

TITLE: Sex-dependent variations in natural history of transitional cell carcinoma of bladder.

The records of 196 patients with transitional cell carcinoma of the bladder diagnosed up to 1975 were analyzed for age, sex, tumor histology, and survival parameters. Analysis of the series as a whole showed it to be comparable to literature controls in the male to female ratio, average age at presentation, and in survival data. Sex stratification combined with parametric analysis showed that females had longer recurrence-free survivals and died less frequently of bladder cancer than their male counterparts. In addition, the incidence of high-grade tumors was lower in females. These facts and the comments that follow point to a probable less aggressive natural history of transitional cell carcinoma of the bladder in females.

Tartrate-resistant acid phosphatase activity determined by enzyme immunoassay was higher in the serum of cancer patients than that in normal blood donors. The highest activity was found among patients having malignancy metastatic to bone. In recent years there has been an increasing awareness that radical hysterectomy and pelvic lymphadenectomy can play an important role in the management of early invasive cervical cancer. We present a series of 255 patients who had radical hysterectomy for gynecologic malignancy over a nine-year period. The incidence of ureteral fistulas was 0.8% and of vesical fistulas 1.6%. The operative mortality was 0.78%, which compares favorably with the rates in most larger centers in which radical pelvic operations are common. With proper patient selection and excellent operative technique, survival rates of well over 90%, with minimal complications, can be obtained.
