No currently available evidence from breast cancer screening studies provides strong justification for the implementation of screening in elderly females. Because it is an absence of evidence rather than negative evidence which pertains, it is concluded that if screening is introduced it should include both mammography and clinical examination of the breasts because the two complement one another. As well, stringent quality control procedures are required for both modalities. It is possible that clinical examination of the breasts performed by a nurse-examiner will enhance compliance with screening schedules.

Infections caused by MRSA in otorhinolaryngology were evaluated in this study. From October 1990 to the end of September 1991, MRSA was detected in one hospitalized patient and 14 outpatients of the otorhinolaryngology department. MRSA was detected in the tonsils of five patients other patients--including four with cancer and other underlying diseases. In two of the four patients with chronic otitis media, otorrhea was stopped by disinfecting external ear toxin. Osteomyelitis of the upper jaw in neonates and infants must be treated in consideration of MRSA, as it is a hospital infection caused by cephem-resistant Staphylococcus.

In 16 of 191 patients the initial attempt to place a double-pigtail internal ureteric stent by conventional methods was unsuccessful. These patients had cancers of the rectum (nine patients), bladder (two patients), vagina (one patient), cervix (one patient) or spermatic cord (one patient) or radiation strictures (two patients). When a guide wire could be advanced beyond the stricture, placement of a small angiographic catheter for 7 to 10 days allowed delayed placement of the stent in all but one case.

Results of home parenteral nutrition in cancer patients were evaluated by analyzing files from 498 individuals. The predominant malignancies were cancer of the stomach (44% in men, 29% in women), the colon or the ovaries, respectively. Most patients (71%) admitted to home parenteral nutrition were severely malnourished, based on a Broca-Index of 0.8 or less. The majority (78%) of these subjects gained weight while on home parenteral nutrition. The remainder 22% continued to loose weight. Median survival was 64 (one to 720) days.

A rupture of the ascending aorta which occurred in a woman on the 13th postoperative day following a right upper lobectomy with mediastinal lymph node dissection for lung cancer is reported herein. Fortunately, the patient was rescued from a cardiac tamponade and hemothorax by emergency operation. The operative findings suggested a traumatic rupture of the aorta, however, lymph node dissection of the mediastinum could not be excluded as a possible cause. Therefore, careful mediastinal lymph node dissection should be carefully performed in operations for lung cancer.

The mortality from ovarian cancer is responsible for 450-500 cancer deaths annually in Denmark. This high mortality rate has remained unchanged for decades. No effective screening method is available. Oophorectomy per occasionem may reduce the incidence of ovarian cancer. The literature suggests a reduction in the incidence of ovarian cancer of 5.7% which corresponds to approximately 30 women annually in Denmark when OPO is performed at the age of forty or later. OPO also avoids reoperation due to the Residual Ovarian Syndrome, which is detected in approximately 2% of women after pelvic surgery.

Both in ENT tumours and in breast cancer electrons must be associated with high energy photons, in order to improving the clinical results with reduced risks of severe late effects. Two previous studies have reported the presence of human papillomavirus (HPV) genomic sequences in colorectal cancers. We examined DNAs from 50 colorectal tumors for HPV sequences by polymerase chain reaction (PCR), using HPV consensus L1 primers for amplification. The PCR products were screened with a generic HPV probe consisting of amplimers of HPV-16 and HPV-18. All tumor DNAs were negative for HPV sequences. These results call into the question the reported association of colorectal cancers with HPVs.
