TITLE: Mortality among workers receiving compensation awards for silicosis in Ontario 1940-85.

The mortality experience of 1190 miners and 289 surface industry workers receiving workers' compensation awards for silicosis in Ontario since 1940 has been studied up to mid-1985. Both groups were found to have a significantly increased mortality from lung cancer (miners' SMR: 230; surface workers' SMR: 302) and stomach cancer (miners' SMR: 188; surface workers' SMR: 366). Adjustment for smoking and country of origin did not explain the excesses observed. The lung cancer findings are consistent with observations from silicosis registries in Europe. Possible explanatory factors are discussed.

Patients' denial of the seriousness of their illnesses should not be challenged unless it creates significant problems for the patient or relative. Denial, when challenged, must be done gently by pointing out inconsistencies so that defences are not disrupted and yet awareness can be explored.

Occupational exposure to wood dusts has been well established as a cause of nasal cancer, dermatitis, and pneumonites from molds growing in wood chips. With the exception of studies on western red cedar asthma, there is a dearth of information on the respiratory toxicity of wood dust exposure. This paper reviews the clinical and epidemiologic literature and identifies the specific woods (with botanical names) and their respiratory disease correlates, including pulmonary function declines, chronic and acute symptoms, and impaired mucociliary transport.

Among all subjects attending a Center for prevention of female genital tract cancer Gardnerella vaginalis was isolated from 57 out of 171 women affected by non-specific vaginitis (NSV), from 15 of 703 non-NSV vaginitis patients and from 10 of 338 healthy adults. Postmenopausal women showed an increased rate of asymptomatic carriage of G. vaginalis. The data suggest a relatively weak association between clinical diagnosis of NSV and isolation of G. vaginalis.

The monoclonal antibody RAP 5 immunoreactive with the ras gene product p21 was used in an immunohistochemical study of 57 patients with advanced ovarian cancer and in 28 normal ovaries. The pattern of the staining of various tumor specimens was similar to the germinal epithelium of normal ovaries, whereas the intensity of staining was more enhanced in carcinomas than in normal ovaries. However, we found a lack of correlation among staining intensity of RAP 5 and the histologic type, the histologic grade, the ploidy class, and the clinical outcome.

TITLE: Does maintenance therapy with alpha interferon stabilize cancer growth? A pilot study.

A pilot study was undertaken to test whether alpha interferon (alpha-IFN) maintenance could continue to stabilize cancer measurements after induction therapy. Twenty-one patients who achieved stable disease at the end of their induction treatment were randomized to receive weekly maintenance with human lymphoblastoid interferon (HLBI) (3 MU/m2, i.m.) or to be followed and not given interferon. There were 12 patients in the maintenance groups and 9 in the control observation group. There was no time difference in disease progression between the two arms. This article considers the relevance of the stable disease category in trials of interferon.

The current article reviews the role of estrogen and polypeptide growth factors in control of proliferation and basement membrane invasion of breast cancer cells in vitro. The role of antiestrogens to regulate proliferation, invasion, and growth factor secretion is further highlighted. Finally, the use of in vitro cultures of breast cancer cells to model steps in the malignant progression of the disease is emphasized. The availability of hormone dependent and independent breast cancer cell lines should allow screening for better antiestrogens, antimetastatic drugs, and antagonists of local action of growth factors.

The psychological problems associated with cancer and its treatment can cause a great deal of morbidity for both patients and their families. Signs of anxiety and depression should not be dismissed by physicians and other care-givers as overreactions or simply unavoidable mental states in a patient with a disease as serious as cancer. Rather, physicians should be on the alert for isolation, hopelessness, helplessness, anxiety, depression, poor body image, and sexual problems, and then treat them appropriately, through counseling, desensitization, reconstructive surgery, or the use of antidepressants or anxiolytics. The author discusses symptoms of psychological morbidity and suggests ways to combat it.

Dysphagia secondary to partial pharyngeal stenosis after total laryngectomy is most likely attributable to the size of the reconstructed lumen. To reduce the incidence of this postoperative complication, we have utilized a modification of total laryngectomy that conserves the hypopharyngeal mucous membrane. The results of this surgical procedure are now reported in the context of two similar, contemporaneous groups of patients who underwent either standard wide-field laryngectomy or hypopharyngeal mucosa conservation laryngectomy. Statistical analysis showed no difference between groups in cancer control (P = .80) or survival (P = .65); whereas the group treated with hypopharyngeal conservation laryngectomy had significantly less pharyngeal stenosis as measured by dysphagia and need for dilatation (P = .011).

A rare case of epitheloid sarcoma of the vulva is described. This is a soft-tissue malignancy arising from tenosynovial tissue. The patient presented with a painless lump of the vulva of a month's duration. An excision biopsy was performed followed by a wide local excision after the actual diagnosis was confirmed. Post-operatively, her recovery was uneventful and she was seen in the Cancer clinic at regular intervals. Three years following surgery, she was well with no evidence of any recurrence. The suggested mode of treatment ranged from a wide local excision to radical vulvectomy with groin node dissection.

This review has focused on some of the advantages and disadvantages in the administration of chemotherapy at home, the physician's office, and the ambulatory care facility. Effective planning for chemotherapy administration and the selection process for each of these settings depends on the needs of the patient and family. We can conclude that, although it is possible to use a combination of setting to maximize patient care, the ambulatory care concept provides broader choices for patients, families, and clinicians.

Nine patients, including 4 with primary advanced breast cancer (stage IV) and 5 with local recurrent cancer, underwent chest wall reconstruction using an omental flap and mesh skin grafting. In 2 of these patients the defect of bony chest wall was reconstructed with an acryl-resin plate and omental flap. The postoperative course in all patients was uneventful, except for a slight necrosis on the transposed mesh skin. Flail chest or dyspnea did not occur in those with a bony chest wall reconstruction. The immediate postoperative performance status in 6 of 9 patients and also quality of life improved.

The clinicopathological findings in a patient who developed breast carcinoma ten years after male-to-female sexual reassignment are reported. Only two other cases of transsexual men with breast carcinoma have been reported previously. All three patients received oral estrogens for prolonged periods to maintain secondary female characteristics. Weight and height values measured in a population-based case-control study of breast cancer conducted in Hawaii were used to validate body size information recorded on driver's licenses.
