As a variety of procedures become technically feasible with laparoscopic techniques, it becomes increasingly important to appropriately select the patients who will benefit from the laparoscopic approach. We report the case of a patient with Dukes C2 colon cancer treated by laparoscopic-assisted sigmoid colectomy who subsequently developed an abdominal wall recurrence at a trocar site scar. The case raises some concerns about the use of the laparoscopic technique in the surgical management of colon cancer.

TITLE: Late relapse of clinical stage I testicular cancer.

PURPOSE: The clinical presentation and treatment of late recurrence (2 or more years after initial management) of testis cancer are defined. MATERIALS AND METHODS: A retrospective search was performed to identify patients treated for late relapse at our university from 1979 to 1992 who had initially presented with clinical stage 1 disease. RESULTS: We treated 35 patients with chemotherapy for late relapse. No patient has remained disease-free with chemotherapy alone. Subsequent surgery resulted in 43% of the patients being currently disease-free. CONCLUSIONS: The treatment of late relapse is surgical. Chemotherapy before late relapse for low volume disease does not uniformly prevent late recurrence.

The differential diagnosis between pancreatic cancer and chronic pancreatitis is very important as the management and prognosis of these two diseases is different. In most patients with pancreatic disease, the diagnosis can be established but there is a subgroup of patients in whom it is difficult to differentiate between these conditions because the clinical presentation is often similar and currently available diagnostic tests may be unable to distinguish between an inflammatory or neoplastic pancreatic mass. This paper reviews the aetiology, pathology and clinical features of these diseases and discusses the limitations of conventional diagnostic methods and how newer techniques may be of value in the differential diagnosis.

No currently available evidence from breast cancer screening studies provides strong justification for the implementation of screening in elderly females. Because it is an absence of evidence rather than negative evidence which pertains, it is concluded that if screening is introduced it should include both mammography and clinical examination of the breasts because the two complement one another. As well, stringent quality control procedures are required for both modalities. It is possible that clinical examination of the breasts performed by a nurse-examiner will enhance compliance with screening schedules.

The Turcot syndrome has been defined as the simultaneous presence of multiple polyposis of the colon and a malignant brain tumor. Infections caused by MRSA in otorhinolaryngology were evaluated in this study. From October 1990 to the end of September 1991, MRSA was detected in one hospitalized patient and 14 outpatients of the otorhinolaryngology department. MRSA was detected in the tonsils of five patients other patients--including four with cancer and other underlying diseases. In two of the four patients with chronic otitis media, otorrhea was stopped by disinfecting external ear toxin. Osteomyelitis of the upper jaw in neonates and infants must be treated in consideration of MRSA, as it is a hospital infection caused by cephem-resistant Staphylococcus.

Better efficacy of chemotherapy for small-cell lung cancer (SCLC) has not been convincingly demonstrated despite many investigations examining increased drug delivery and enhancement of drug diversity. Improvements in the median- and long-term survival of patients with limited SCLC have been shown, but this change is more closely associated with better integration of chemoradiation than with more effective chemotherapy or radiotherapy components. Recently, a number of new active drugs have been identified for treating patients with SCLC. These drugs include the taxanes, the camptothecins, and gemcitabine. Determining the role of each new agent will require carefully designed and conducted trials like those now in progress.

These patients had cancers of the rectum (nine patients), bladder (two patients), vagina (one patient), cervix (one patient) or spermatic cord (one patient) or radiation strictures (two patients). When a guide wire could be advanced beyond the stricture, placement of a small angiographic catheter for 7 to 10 days allowed delayed placement of the stent in all but one case.

Toxins are potent cytotoxic proteins which gain access to the interior of mammalian cells by receptor-mediated endocytosis. However, the trafficking pathways within mammalian cells are complex and toxins must be processed to active forms while avoiding degradation by the lysosomal system. Once delivered to an appropriate intracellular location, the active toxin fragment translocates to the cell cytosol and inhibits protein synthesis. Chimeric toxins are constructed by removing the toxins natural binding domain and replacing it with an antibody or cell-binding ligand that redirects cell killing activity to cancer cells. Gaining an understanding of how toxins manoeuvre within cells is vital for improving the effectiveness of chimeric toxins.

An analysis of the clinical symptoms has been carried out in the group of 110 patients who underwent nephrectomy for a clear-cell carcinoma. Total hematuria (47.3% of patients) has been a predominant symptom of the tumour. Pain felt in the back has been noted in 39.1% of patients. Palpable tumour has been noted in 34.5% of patients. Such symptoms have been present simultaneously (Israel's triad) in 12.3% of patients. Tumour has been diagnosed accidentally, mainly with USG, in 37.3% of patients. It has been found that classic clinical symptoms of this cancer are nowadays less frequent than before introducing ultrasound examination.

Changes in the epidemiology of Streptococcus pneumoniae and other recognized respiratory pathogens, the increasing occurrence of drug-resistant microorganisms, and the detection of new respiratory pathogens have heightened awareness of the public health importance of severe respiratory infections. To characterize the epidemiology of P&I deaths in the United States, CDC further analyzed underlying and multiple cause-of-death mortality files for 1979-1994. This report summarizes the results of this analysis.

Results of home parenteral nutrition in cancer patients were evaluated by analyzing files from 498 individuals. The predominant malignancies were cancer of the stomach (44% in men, 29% in women), the colon or the ovaries, respectively. Most patients (71%) admitted to home parenteral nutrition were severely malnourished, based on a Broca-Index of 0.8 or less. The majority (78%) of these subjects gained weight while on home parenteral nutrition. The remainder 22% continued to loose weight. Median survival was 64 (one to 720) days.

TITLE: [The relation between myelotoxicity and performance status or aging in chemotherapy of non-small cell lung cancer]

One hundred eighty six patients of non-small cell lung cancer treated with cisplatin-based chemotherapy were analyzed. Significant incidence of anemia was observed in PS 2-3 patients as compared to PS 0-1 cases. Incidence of thrombocytopenia was significant in cases 70 or more years old as compared those less than 50 years old. No correlation was detected between myelotoxicity and aging.
