By the early 1980s, epidemiologists had identified many important causes of cancer. They had also proposed the 'multi-stage' model of cancer, although none of the hypothesized events in human carcinogenesis had then been identified. The remarkable advances in cell and molecular biology over the past two decades have transformed the scope and methods of cancer epidemiology. There have been a few new discoveries based purely on traditional methods, and many long-suspected minor risks have been estimated more precisely. But modern epidemiological studies often depend on genetic, biochemical or viral assays that had not been developed 20 years ago.

A simple method for testing the assumption of independent censoring is developed using a Cox proportional hazards regression model with a time-dependent covariate. This method involves further follow-up of a subset of lost-to-follow-up censored subjects. An adjusted estimator of the survivor function is obtained for the dependent censoring model under a proportional hazards alternative. The proposed procedure is applied to an example of a clinical trial for lung cancer and a simulation study is given for investigating the power of the proposed test.

Patients with lung cancer often develop airway obstruction that necessitates palliative bronchoscopic interventions. Expandable metal stents are increasingly used for this indication. We report a case of fatal massive hemoptysis that occurred 1 week after uneventful bronchial stent placement. Previous treatment with external-beam radiotherapy, brachytherapy, and photodynamic therapy probably contributed to pulmonary artery-bronchial fistulization in this patient. This complication should be kept in mind when considering expandable airway stent insertion in heavily pretreated lung cancer patients.

Here, a novel approach to genome expression pattern interpretation is described and applied to the recognition of B-cell malignancies as a test set. Using cDNA microarrays data generated by a previous study, a neural network model known as simplified fuzzy ARTMAP is able to identify normal and diffuse large B-cell lymphoma (DLBCL) patients. Furthermore, it discovers the distinction between patients with molecularly distinct forms of DLBCL without previous knowledge of those subtypes.

In developed societies, chronic diseases such as diabetes, obesity, atherosclerosis and cancer are responsible for most deaths. These ailments have complex causes involving genetic, environmental and nutritional factors. There is evidence that a group of closely related nuclear receptors, called peroxisome proliferator-activated receptors (PPARs), may be involved in these diseases. This, together with the fact that PPAR activity can be modulated by drugs such as thiazolidinediones and fibrates, has instigated a huge research effort into PPARs. Here we present the latest developments in the PPAR field, with particular emphasis on the physiological function of PPARs during various nutritional states, and the possible role of PPARs in several chronic diseases.

OBJECTIVE: To analysis the mutation of BRCA1 gene in 186 breast cancer patients. METHODS: Polymerase chain reaction-single stranded conformational polymorphism (PCR-SSCP) and sequential were used in this analysis. RESULTS: 13/186 of patients showed mutation which comprised 7% of the total number of patients. In 8 patients, the mutation was observed in the intron splicing region and in 5 patients, the mutation was on the exon. CONCLUSION: Mutation analysis of BRCA1 gene is helpful in the determination of developmental potential, early diagnosis and gene therapy for breast cancer.

We present a case of squamous cell carcinoma of the dorsal surface of the tongue that arose in a previously biopsy-confirmed area of lichen planus. The syndrome has a haematological phenotype of early-onset red-cell aplasia but is coupled with a baffling array of pleiotropy. There is discordance with modes of inheritance, physical anomalies, erythropoietic response to corticosteroid therapy, spontaneous 'remissions', and evolution to malignant myeloid transformation and to cancer. The recent discovery of two genes associated with DBA is the entry point for explaining the diversity of the phenotype and for understanding the molecular basis of the syndrome.

We reviewed the trends and death rates of pancreatic cancer and pancreatitis in Japan over the past four decades. It is a disturbing fact that the death rate due to pancreatic cancer is rising, parallel to that of lung cancer and colon cancer and that it is affecting primarily the elderly 65 years and older in both sexes. The cause of this increase is partially attributable to improvements in the diagnosis, changes in life-style, and smoking. The death rate due to chronic and acute pancreatitis has remained constant during the past four decades.
