The surname analysis of over 40,000 Wisconsin cancer mortalities for the period 1979 through 1985 partially reveals the relative genetic component of various major cancers. The surname frequencies and frequency distributions indicate that both tumor-promoting and tumor-suppressing genes may be involved. A number of cancers exhibit some probable genetic component, but genetic involvement seems marked in male and female leukemia and male lung cancer. Evidence is found for autosomal and sex-linked, dominant and recessive patterns. Most surprising is the strong evidence for tumor-suppressing genes for most cancers. Clustering of surnames across cancers evinces a number of Wisconsin surnames that may be involved in a cancer family syndrome.

TITLE: [Emission computed tomography in the diagnosis of lung diseases]

Investigations of pulmonary perfusion with 99m Tc-MMA by the method of emission computed tomography (ECT) were conducted in 172 patients with different pulmonary diseases. The authors described ECT procedure of the lungs and scinti-tomographic symptomatology typical of the normal lungs and different types of pulmonary pathology (lung cancer, benign tumors, metastases and chronic inflammatory pulmonary processes). A high sensitivity of ECT as compared to poly-positional scintigraphy was shown. The obtained results suggested that ECT permitted more specified and earlier diagnosis of pulmonary perfusion disorders which was of great importance in the diagnosis of different pulmonary diseases as well as in the assessment of therapeutic efficacy.

A retrospective study of 1000 cases of microscopically diagnosed head and neck cancers in the University Hospital, Kuala Lumpur was done. Head and neck neoplasms comprise 7.1% of all tumours diagnosed in this Hospital and the commonest sites of involvement are the nasopharynx (29.1%), cervical lymph nodes (22.6%), oral cavity (10.2%), thyroid (8.2%) and skin (6.5%). Histologically, 87% of tumours are epithelial in nature. The results of this study show that nasopharyngeal carcinoma is commonest in Chinese males, while oral and laryngeal malignancies occur more frequently in Indians.

The incidence and extent of postactinic pulmonary fibrosis was investigated in relation to the different types of radiation therapy. For this purpose, 131 patients treated with fast electron (for breast cancer) and 148 patients treated with TCT (for breast cancer and cancer of other nature) observed in the period 1976-1987 were followed up (from 13 months to 7 years). The frequency of postactinic fibrosis was much less in the first group of patients than the second (6.61% vs 18.99%); further, the functional damage in these patients proved less and developed in a less unfavourable way.

TITLE: [Two cases of amylase-producing lung cancer]

In 1951, Weiss et al. first reported a case of an amylase-producing lung cancer. Since then, reports on immunohistochemical, ultrastructural, and biochemical studies have been published. We recently have experienced two autopsy cases of lung cancer that manifested high amylase activity in the serum, urine, and pleural effusions. Histologically, both lung cancers were classified as being well differentiated adenocarcinomas, and immunohistochemically, the tumor cells showed positive immunoreactivity for amylase. This paper discusses the histogenesis of amylase-producing lung cancers and cases found in the literature.

TITLE: Pancreatic cancer risk following gastric surgery.

To estimate the risk of pancreatic cancer after gastric surgery for benign disease, we followed all 336 residents of Olmsted County, Minnesota, who had surgical treatment for benign peptic ulcer disease during the years 1935 to 1959. These patients were followed for 6549 person-years of observation. Pancreatic cancer developed in only one patient although 1.6 primary pancreatic cancers were expected (relative risk 0.64; 95% confidence interval, 0.02 to 3.53) based on pancreatic cancer incidence rates for the general population of Olmsted County. We conclude that in our study the incidence of pancreatic cancer is not significantly increased after gastric surgery.

TITLE: Four-day continuous infusion of cisplatin and 5-fluorouracil in head and neck cancer.

A combination of cisplatin and 5-fluorouracil, both administered 4 days continuously as infusion, was assessed in advanced head and neck cancer. Of the 37 patients studied, there were 15 complete and 17 partial responses (40.5% and 45.9%, respectively). Survival is 79.1% at 22 months. None of the patients in complete response has relapsed. In general toxic effects were moderate. Given as initial treatment, the regimen is effective and of considerable use in this type of patient.

The process of cancer metastasis is sequential and selective and contains stochastic elements. The growth of melanoma metastases represents the endpoint of many lethal events that few tumor cells can survive. Primary tumors consist of multiple subpopulations of cells with heterogeneous metastatic properties, and the outcome of metastasis depends on the interplay of metastatic tumor cells with various host factors. This viewpoint is more optimistic than that of metastasis as a random process. A selective biological process is regulated by the interaction of tumor cells with their host, and these complex interactions can now be studied and manipulated.

Six patients with mucinous cancer (2 colonic, 2 pancreatic, and 2 pulmonary origin) had necropsy evidence of thrombosis of large and small systemic, extracranial, and intracranial arteries and veins, and multiple cerebral infarcts and small hemorrhages. On microscopic examination, we found small infarcts and hemorrhages within the brain and mucin within vessels, macrophages, and in areas of infarction. The clinical picture included strokes and encephalopathy. Mucin-producing cancers can be associated with a coagulopathy that causes extensive occlusive vascular disease.

During a 9 year period we excised 70 suspicious non-palpable breast lesions detected by mammography. We found the most effective method of preoperative localisation to be insertion of a self retaining wire, and this procedure was facilitated by a Perspex grid incorporated in the mammography equipment. Thirty biopsies (43%) were found to be malignant, of which 14 contained invasive and 16 in situ carcinoma. The presence of malignancy could not be predicted from the mammographic appearance, although in situ cancer was always associated with microcalcification.

An analysis of 136 cases of stomach cancer led to the determination of the role and place of gamma-topography, echography, computerized x-ray tomography, angiography and laparoscopy in specified preoperative diagnosis of extra-organ spreading of primary tumors. The informative value and shortcomings of certain methods were shown. A high accuracy of the entire set of diagnostic procedures (94.8%) in the preoperative definition of stomach cancer involvement of the abdominal and retroperitoneal organs and tissues was emphasized.

A case of emergency revascularisation of the hepatic artery for liver necrosis following its forced ligature during corpo-caudal pancreatectomy for cancer of the pancreas in referred. Necrosis protection mechanisms, namely higher portal flow, increase in O2 extraction and collateral circulation are analysed. The therapeutic aids for preventing this serious event are presented. It is considered that peripheral expansion of the hepatic artery thrombosis prevented the installation of a valid collateral circulation, although the hypothesis of a reduction in portal flow should not be ignored.

An epidemiological survey has been carried out to establish the incidence of second malignant neoplasms in hereditary retinoblastoma survivors in The Netherlands and the relative risk of cancer in non-affected relatives. The cumulative incidence of second neoplasms was 19% at the age of 35 years. Fathers, unaffected by retinoblastoma, were at risk for pancreatic cancer, the relative risk being 8.3.

The barium enemas of 119 patients with colorectal cancer and 119 age-matched controls were examined for evidence of diverticula. Amongst men, 39.6% of patients with colorectal cancer had diverticulosis, compared with 50% in the control group. In the women, 39.3% of patients with colorectal cancer had diverticular disease, compared with 36% of the controls. There appears to be no significant aetiologic association between the two conditions, since they do not occur together more frequently than in controls, although each is reputedly the result of fibre lack.
