TITLE: Fibrous mass complicating epidural morphine infusion.

The benefits of continuous epidural morphine infusion using an implanted pump delivery system to control intractable cancer pain have recently been described. Most articles on this subject relate to dosage, technique, degree of pain relief, and tolerance. There are some anticipated complications of the treatment related to the surgical implantation of the system and drug toxicity. We present a complication presumably related to the epidural catheter. A fibrous reaction that developed around the catheter tip progressed into a mass. This caused a significant displacement of the spinal cord with the development of long tract symptoms. Identification of this abnormality using myelography and computed tomography led to prompt surgical decompression resulting in improvement of the patient's condition.

Most Pasteurella multocida infections in humans have been associated with animal bites or scratches. While a variety of infections involving P multocida, unassociated with animal trauma, have been described, infections of the urinary tract are rare. We studied a case in which P multocida was isolated from the urine of a woman with advanced uterine cervical cancer. In most of the cases in which P multocida has been isolated from urine, the patients have had anatomic defects of the urinary tract, as well as an underlying chronic illness. These host-related characteristics appear to be important in the pathogenesis of P multocida urinary tract infection.

Macro creatine kinase type 2 (MCK-2) is a promising new tumour marker for carcinoma of the colon. We have purified it from tumour tissue, from serum and, for the first time, from normal colonic epithelium and identified it in a tissue culture line of colon cancer cells. The molecular weight of each preparation, regardless of the source, was similar. Its consistent presence in tumours and in normal colon, contrasted to its variable appearance in serum, implies that there are other factors affecting its release into, or removal from, the circulation.

In vitro chemosensitivity test of human solid cancers, serially transplanted into nude mice was performed by detecting the inhibitory rate of 3H-thymidine incorporation to cancer cells. And the results of in vitro chemosensitivity test were compared with those in nude mice. This in vitro chemosensitivity test could present the quantitative results from a relatively small number of cancer cells within a short term (48 hours). Additionally, many samples could be handled at the same time by using microtestplates and a semiautomatic multiple cell harvester. The predictability of this in vitro chemosensitivity test to the in vivo effects was 80.6% (29/36) with 3 false positive and 4 false negative cases.

The ambient air quality standard established in 1973 is 0.03 ppm annual average for sulfur dioxide and 0.075 mg/m3 for particulates. It is now generally believed that the toxicity of sulfur oxides in ambient air is significantly influenced by the coincident presence of particulates. Inhalation of 1 ppm of sulfur dioxide for 2 hr may produce alterations in pulmonary ventilatory function, both in normal and asthmatic subjects. Effects obtained at 0.5 ppm of sulfur dioxide are controversial and of questionable biological significance. No clear evidence exists that sulfur dioxide or bisulfite causes mutagenicity in mammals, and it is concluded from a variety of animal experiments that long-term exposure to sulfur dioxide alone does not cause cancer.

Two cases of gastric cancer were treated with immunochemotherapy using Tegafur and Schizophyllan. In one case, a marked reduction in the size of metastatic liver tumor and disappearance of most subjective symptoms were observed. The response continued even 8 months after the initial treatment. In the other case, no marked symptoms except occasional dysphagis have been noted for 2 years and 2 months since diagnosis, although the primary tumor size has remained unchanged. Thus, it is presumed that the combined immunochemotherapy regimen may be useful in the treatment of advanced gastric cancer.

It has been suggested that reduction of diagnostic delay in symptomatic colorectal cancer will not improve survival. Published reports show varying effects of duration of symptoms both on stage of cancer and on survival. However, tumour stage can be measured only at the end of the symptomatic period and, therefore, conclusions cannot be drawn about early diagnosis of symptomatic colorectal cancer and its effect on survival. On current evidence, early diagnosis and treatment may improve survival and should be encouraged.

Subcutaneous injection of 4 mg DMBA to noninbred rats leads to a considerable decrease in host resistance to cancer, which progresses with tumor growth. The rats with inhibited host resistance to cancer accepted well, at the stage of tumor germ, the DMBA-induced tumors from other animals, permitting them to grow--the phenomenon that was not observed in normal noninbred young rats. At the stage of clinically demonstrable nodules at the sites of DMBA injections, the tumors with different transplantability did not only get adapted but developed distant metastases in part of the animals.

The authors have encountered four cases of oil aspiration pneumonia complicated by carcinoma. Each had a clear-cut history of chronic intake of an oily substance, radiographic changes, and histologically documented oil aspiration pneumonia. Lung cancer later appeared in the involved area. A small number of similar cases also have been reported. The implication is that oil aspiration pneumonitis may induce bronchogenic carcinoma, particularly either the alveolar cell or the squamous cell variety. The radiographic diagnosis of the malignant transformation is difficult, and consequently the prognosis is poor.

Multiple agents have been suggested as urinary bladder carcinogens. Of these, certain aromatic amines and some tryptophan metabolites (Anthranilic acid, 3 hydroxy Anthranilic acid) have been proved to be carcinogenic. Others are suggested but are not conclusively proved as carcinogenic agents. Various animal species have been used as experimental models for the study of induction of cancer of the bladder and substantial progress has been made in the search for etiologic factors and pathogenesis of bladder tumors. A carcinogenic model of cancer bladder induction is proposed. However, knowledge of the tumorigenesis of cancer of the bladder is still limited and fragmentary. It appears, however, that Tryptophan dysmetabolism play a major role in this field and future experimental and epidemiologic studies should take this fact into consideration.

We have retrospectively studied the problems concerning duration of maintenance therapy and chemotherapy regimens for non-Hodgkin's lymphoma treated at Shikoku Cancer Center Hospital. Four out of 15 complete remission cases had been in complete remission more than 3 years. Maintenance chemotherapy consists of CHOP protocol administering every 4 weeks and the median maintenance duration was 14 months with a median cycle of 14. Thereafter, patients have been followed up without receiving any therapy. The prospective study on maintenance therapy should be designed considering conditions such as 1) sufficient drug doses for cytocidal effects, 2) shorter maintenance period as possible, and 3) minimal side effects and complications to be expected.

In order to reduce the lipoprotein-related risk of coronary heart disease, nutritional recommendations have been formulated for use by communities prone to atherosclerosis and its complications. As such recommendations are potentially of widespread application they require careful scrutiny to assess possible risks as well as benefits. Epidemiological, clinical and experimental data concerning relationships between these nutrients and non-cardiovascular diseases are reviewed with emphasis on cancer mortality. Changes in intake of fats, including polyunsaturated fat, of cholesterol, carbohydrate, fibre, sodium and beta-carotene are discussed, and evidence of a relationship between serum cholesterol concentration and cancer is examined. These considerations offer reasonable reassurance as to the safety of recent dietary recommendations for the reduction of coronary heart disease.

Out of 808 (100%) cases of endometrial carcinomas, registered at the Klinik Bad Trissl during a period of 14 years (1968-1981), over 85 cases (10,5%) of double and triple malignomas were found. 59 cases (7,3%) are correlated with a second primary breast cancer, 26 (3,2%) with other primary malignomas. 9 cases (1,1%) show triple malignomas. 5 precancerous cases (0,6%) were included in the survey. Contrary to medical literature the survey reveals a different distribution of neoplasms among the affected organs.

In a case-control study of 36 men with cancer of the ureter and 307 male controls with the same age distribution tobacco smoking conferred a relative risk for this cancer of 2.9, whereas consumption of analgesics of any kind did not increase the risk. The relative risk for cancer of the renal pelvis, determined for 29 men, was 2.4 for tobacco and 6.0 for phenacetin-containing analgesics. Analgesics containing no phenacetin conferred no increased risk for cancer at this site. Data for 13 women with ureteral cancer, 5 of whom had taken phenacetin, are presented but not analyzed.
