Bone is one of the three most common sites of tumor metastasis. Bone metastasis is a catastrophic complication of cancer, since it is not only associated with intractable pain, but also with other morbidity such as hypercalcemia which usually hastens an uncomfortable death. Despite the importance of bone metastasis/hypercalcemia, until recently clinicians have used specific therapies only for the treatment of hypercalcemia but not for bone metastasis. During this presentation, I will review our current understanding of some of the steps involved in the pathophysiology of bone metastasis and its association with hypercalcemia, and how I think these complications of cancer should be treated.

Radical cystectomy followed by a urinary diversion or an orthotopic neobladder remains standard treatment for patients with muscle-invasive bladder cancer. The advances in surgical technique and reconstructive options allow many patients to enjoy a high quality of life after surgical extirpation. For those unable or unwilling to undergo a radical cystectomy, several treatment alternatives exist, using aggressive TUR and combinations of chemotherapy and/or radiation. Continued investigations into the biological behavior of this malignancy, as well as the identification of more effective chemotherapeutic agents, will allow more definitive treatment approaches with improved outcomes.

Visual disturbances in advanced cancer patients are very rarely signaled, evaluated, or adequately treated. The main causes of sight disturbances are primary eye tumors, ocular metastases, and some paraneoplastic syndromes. Sight alteration can also be associated with asthenia, fatigue, anemia, and hypovitaminosis. These symptoms can be monocular or binocular, and their gravity and evolution can vary. Based on a survey of 156 patients, we estimate the prevalence of visual disturbances to be 12% in advanced cancer patients.

In vulvar cancers HPV 16 positive, HPV 16 negative, and vulvar and vaginal precancerous status (VIN, VAIN) immunohistochemical technik onko- and antioncogenic proteins were evaluated. The contrary correlation between HPV 16 presence and overexpression p53 were detected. It suggest the heterogenic etiology of these cancers. There was payed attention to high activity of virus replication and intensive virion production in VAIN. Nontreated vaginal lesion may make difficult to obtain a positive cervical HPV infection treatment.

We treated a patient with lung cancer in whom a hypercoagulopathy was induced acutely by chemotherapy. He received systemic chemotherapy twice and in both instances developed disseminated intravascular coagulopathy (DIC), accompanied by acute decrements of the peripheral platelet count and plasma fibrinogen, an increment of the fibrin degradation products (FDP), and bleeding tendency with the appearance of skin purpura. In each instance, the plasma thrombosis-inducing activity (TIA) appeared one to three days after chemotherapy and subsided subsequently.

One autopsy case with small cell lung cancer and the syndrome of inappropriate antidiuretic hormone secretion (SIADH) is reported. Both plasma atrial natriuretic peptide (ANP) and arginine vasopressin (AVP) levels were high, and the presence of significantly high levels of ANP and AVP in tumor tissue was confirmed by gel chromatography and radioimmunoassay techniques. To the best of the authors' knowledge, this is the first case in which ectopic ANP production and its secretion into blood (leading to SIADH) were proved.

At the moment, proton-therapy is the most advanced radiotherapeutic technique in cancer treatment. The use of the high energy proton beam (from 70 MeV to 200 MeV) lets a Bragg's peak be moved to different depths, so allowing personal radiotherapeutic treatment. In recent years, many proton-therapy centers have grown up throughout the world with very satisfactory clinical results, first of all in eye melanoma treatment. The future expectations are very promising, even if the very high installation and maintenance expenses of a synchrotron (for proton production) hinder the development of such a method.
