The frequency of lung cancer was investigated among 2346 workers in the radiochemical plant 'Mayak' who were exposed to radiation, both externally and internally from incorporation of plutonium. The results from the follow-up were analysed in terms of a linear relative risk model, accounting for the protracted exposures. On the basis of the observed data, it is deduced that the life-span incidence of radiation-induced lung cancer is several times larger than the value 0.0085 Sv-1 that is recommended in ICRP Publication 60 for the risk to this organ.

It is possible to screen for medullary thyroid cancer (MTC) with biochemical tests, imaging and genetic testing in families with multiple endocrine neoplasia type 2. The recommendation is based on a consensus statement made by the Members of the European Community concerted action for MTC. The prognostic characteristics are based on 117 patients with MTC diagnosed in Denmark between 1970-1991. Twenty percent of all MTC patients were members of families with two or more thyroid cancer patients. More than half of all MTC patients had neck lymph node involvement at the time of diagnosis. The frequency of neck lymph node involvement was however only 20% amongst those with familiar MTC.

The oral signs of syphilis have been comprehensively described and can be identified at each stage of the infection. Despite the historical recognition that inadequately treated or untreated syphilitic leukoplakia may undergo malignant transformation, many surgical units no longer routinely screen patients presenting with malignant or premalignant lesions. In this study, five of the 63 patients (8%) who presented with neoplasms of the tongue reacted to syphilis antibodies. These results indicate that syphilis is still prevalent in this group of patients and suggest that serological investigations may be warranted in the initial assessment of all patients with oral cancer.

They are out-numbered by the far more common peripheral neuropathies that occur either as a direct result of the cancer (metastases) or its treatment (usually chemotherapy). Nevertheless, paraneoplastic peripheral neuropathies are important because they may be the first sign of an otherwise occult cancer and/or because they may substantially disable the patient even when the cancer itself is asymptomatic. Paraneoplastic disorders are sometimes marked by the presence of autoantibodies that react with proteins both in the underlying cancer and in the nervous system. Their discovery may lead to an early diagnosis and potential cure of the underlying cancer.

The role of radioimmunodetection in the detection of cancer has been established through the development of high affinity immunoconjugates and recent improvements in radiochemistry. Advances in imaging techniques, particularly single-photon emission computed tomography and image registration, also have dramatically improved the sensitivity and accuracy of tumor detection. Molecular engineering of immune constructs provides promise of enhanced tumor localization properties and reduced immunogenicity, allowing repeated studies to be performed. The role of radioimmunodetection in the evaluation of cancer patients will increase in importance in the future.

Papillary carcinoma, the commonest thyroid malignancy, has a good prognosis and low incidence of distant metastases. Brain metastasis is extremely rare with a frequency of 0.1-5% in reported series. The clinical details of two cases of histologically proven brain metastases from papillary thyroid cancer treated in our centre are presented. In one patient, the lesion was in the cerebellar hemisphere, and in the other, the frontal lobe. Also presented is a review of the literature of this unusual clinical presentation.

Results of several randomized studies have shown a significant pathologic downstaging and a decrease in the rate of positive surgical margins using neoadjuvant therapy in stage B cancers. Despite theoretic advantages, neoadjuvant therapy may have side effects and is associated with costs. Until long-term results of biologic and clinical failure rates are available, this approach should be considered investigational.

What is considered highly erotic has differed widely through time and in various cultures. In the U.S. the female breast has come to mean far more than nurturing an infant. Sexuality symbolizes youth, attractiveness, desirability and as such is used for effective commercial marketing. The reality of cancer remains to be dealt with in health care at a physical level but more is needed to assist the emotional adjustment of affected women and their partners. Brief sex therapy for sex symptoms is outlined.

TITLE: [Results of tamoxifen treatment in patients with advanced breast cancer]

The results of advanced breast cancer therapy with tamoxifen have been evaluated. The objective response to the drug has been achieved in 32 (31.7%) patients, and a 2-year survival in 52% of cases. A correlation between the localization of lesions and response rate has been found. The best results have been achieved in localized lesions and lymph nodes metastases. Low toxicity of the drug enables achievement of results comparable to those seen in all types of the hormonal therapy, except androgens.
