Two isoforms of the regulatory subunits of cyclic AMP (cAMP)-dependent protein kinase that bind cAMP are inversely expressed during ontogeny and cell differentiation. These cAMP-binding receptor proteins in harmony may regulate the growth of normal cells and their differentiation into nondividing states. Cancer cells can also be made to differentiate and stop growing when the functional balance of these cAMP receptor proteins is restored by treatment with site-selective cAMP analogues or by the use of an antisense oligodeoxynucleotide, suggesting new approaches to cancer treatment.

The current status of cervical cancer treatment in Denmark is discussed. Diagnostic aspects and problems of classification are presented briefly supplemented with a comment on new prognostic parameters based on a semiquantitative score system and flow cytometry. Surgery is the treatment of election for the early stages whereas radiotherapy is the treatment of choice in advanced stages. Chemotherapy should only be employed in the framework of clinical trials. It is concluded that centralised treatment should be maintained.

DNA-repair is a complex enzymatic process which enables all living cells to withstand the deleterious and mutagenic effects of most genotoxic agents. Defective DNA-repair is caused by mutations involving genes which encode the enzymes responsible for recognition and excision of DNA lesions. Some of these genes have been identified in humans. Several severe human diseases are caused by defective DNA repair affecting the entire genome (e.g. xeroderma pigmentosum and trichotiodystrophy) or only actively transcribed genes (Cockayne's syndrome). Using self-made supporting tube, we performed colon-anal anastomosis in 48 radical resections of low rectal carcinoma, to save the anal canal and sphincter. The anastomoses were completed without suturing and the results were satisfactory. Postoperative follow up revealed that the bowel movement and sphincteric function were all satisfactory. The rate of anastomotic leak was about 4% and there was no operative mortality. This new technic is simple, safe and easy to perform, and is considered to be a promising procedure for colon-anal anastomosis after resection of low rectal cancer.

Neuropsychological analysis of 47 cancer patients with widely metastatic disease and significant previous treatment indicated that 34% had cognitive deficits. Previous treatment with biologic response modifiers was associated with a 53% frequency of cognitive abnormalities, whereas only 18% of patients who were never treated with biologics had such impairments. The possibility that neurobehavioral abnormalities will result from metastatic disease and treatment regimens is an important factor in determining the risks and benefits of therapy.

Treatment of cancer pain with opioids through an epidural catheter, with a portable infuser has been recorded in 20 patients retrospective. In general there was a good acceptance of the treatment, which often could liberate the patients from close contacts to the pain clinic. There were not recorded adverse effects in relation to the infuser treatment. One patient had systemic adverse effects because of a very high daily opioid dose. The Pharmacia Deltec infuser employed has proven very secure and easy to handle.

A collaborative multidisciplinary study was conducted to assess both physical and psychosocial needs of 117 adults with various types of cancers. Although these patients functioned adequately in many areas of their lives, they reported that cancer and its treatment had an impact on energy, physical and recreational activities, communication with spouse, sexual and emotional functioning, occupational matters, and treatment-related situations. Younger patients reported more psychosocial, sexual, and treatment-related problems than did older patients. Female patients reported more chemotherapy-related problems than did male patients. Implications for clinicians treating cancer patients were discussed.

Therapy for Hodgkin's disease has been associated with a significant increase in risk for second cancers. To begin an investigation of the association of therapy-induced genetic damage with this risk, somatic mutations at the hypoxanthine phosphoribosyltransferase locus were measured in lymphocytes from patients previously treated for Hodgkin's disease. The results demonstrate that a subset of patients have persistently elevated mutation frequencies, perhaps suggesting that these individuals are among those at significant risk of second cancer development.

The patient was 47 year-old man with a simple intermuscular fistula-in-ano. Fistulectomy revealed that there was a small mucinous adenocarcinoma. The tumor was small and localized in the wall of the fistula and the muscular invasion was not recognized. The mucin contained a little O-acylated sainomucin, and the origin of the tumor was considered as anal gland cell. This case appears as one evidence that the cancer in fistula-in-ano originates from the anal gland cell in the fistula.
