TITLE: The psychological impact of immediate breast reconstruction for women with early breast cancer.

Twenty-five patients were evaluated, 13 who had immediate breast reconstruction and 12 who had delayed breast reconstruction for early breast cancer. Data were elicited about the psychological impact of the cancer, the mastectomy, and the reconstruction. Our results support the conclusion that immediate breast reconstruction is accompanied by a lower incidence of psychological morbidity postoperatively, and we recommend that immediate breast reconstruction be offered as an alternative to women with early breast cancer.

We feel that the guidelines described here reflect the state of the art at the time of writing (April 1982). The role of CT scanning is likely to evolve further and nuclear magnetic resonance (NMR) will probably play a significant role in the future. Experience, expertise, and equipment will vary from one hospital to another and thus rigid rules cannot reasonably be applied for the workup of suspected lung cancer. Ideally the diagnostic approach to an individual patient will be highly tailored, not only in regard to the nature of the pulmonary lesion, but also to his or her overall medical and social situation. We have found that a policy of early consultation between clinician and imaging specialist most readily facilitates effective use of available diagnostic resources.

TITLE: Mitomycin C as an adjuvant in resected gastric cancer.

As a result of their previous experience with mitomycin C at high discontinuous doses in advanced gastric cancer, the authors studied its role as an adjuvant for locally advanced cases after surgical complete resection. Results from 70 evaluable patients are presented. Patients were allocated randomly to receive mitomycin C, 20 mg/m2 I.V. direct once every 6 weeks, four courses, or a placebo. After a follow-up period of 250 weeks, seven patients of treatment arm and 23 controls have already relapsed (p less than 0.001). Toxicity was moderate and controllable by symptomatic measures. The authors consider this investigation a positive contribution in the field of adjuvant therapy of gastric cancer.

Gallbladder carcinoma was induced in hamsters by inserting methylcholanthrene beeswax pellets into the gallbladder lumen. The histogenesis and mode of development of the cancer was studied. The mode of development was classified into four; the infiltrative, the polypoid, the para-adenoma, and the mixed type. Twenty-one gallbladder carcinoma included six of the infiltrative type, seven of the polypoid type, three of the para-adenoma type (each case was accompanied by the infiltrative type), and eight of the mixed type (containing three of the infiltrative plus the para-adenoma type, and five of the infiltrative plus the polypoid type). Concerning histogenesis, the infiltrative type developed as de novo cancer. The polypoid and the para-adenoma type developed from adenomatous hyperplasia or adenoma.

To examine the accuracy of normal pelvic lymph node status in identifying patients with clinical stage B prostatic cancer not having extracapsular tumor extension we reviewed 63 consecutive patients (39 stage B1 and 24 stage B2) treated with radical prostatectomy. Ten per cent of the patients with stage B1 lesions (tumor involving 1 lobe) and 38 per cent with stage B2 lesions (tumor involving both lobes) had extracapsular extension. Extracapsular tumor extension was more common in patients with larger, less differentiated lesions. The results suggest that a negative pelvic lymphadenectomy does not exclude with a high degree of confidence extracapsular tumor extension in patients with clinical stage B2 prostatic cancer.

TITLE: Serum immunoglobulins in squamous cell carcinoma of the oral cavity.

Serum immunoglobulins (IgG, IgA, and IgM) have been studied in 70 patients with squamous cell carcinoma of oral cavity and in 40 age-matched normal controls. The results were correlated with clinical stage, tumor size, lymph node status, tumor differentiation, and treatment modalities. There was a significant rise in serum IgM and IgA in oral cancer patients as compared to controls. It was further raised in advanced clinical stage, but tumor size and histological differentiation and treatment modalities (surgery and chemotherapy) do not seem to affect serum immunoglobulin levels. The observations implies that the extension and dissemination of tumor mass is the important factor for the change in serum immunoglobulin level in squamous cell carcinoma of the oral cavity.

Current opinions concerning the surgical treatment of peptic ulcer disease are reviewed. Because of the differences between duodenal and gastric ulcers the approaches to the surgical treatments of these lesions differ. For duodenal ulcers surgery is seldom required when the ulcer craters do not heal or, more often, when the disease is not adequately controlled. In these cases the preferred treatment is hyperselective vagotomy. However, the relatively high level of recurrence some years after hyperselective vagotomy may lead to more radical surgery in the future. In contrast to duodenal ulcers, the risk of cancer with gastric ulcers, although low and difficult to evaluate, often leads to quicker surgical intervention. For gastric ulcers the surgeon faces the difficult choice between partial gastrectomy and vagotomy with ulcer excision.

Vascular access by means of a permanently implanted cuffed silicone rubber catheter has become increasingly popular in the management of cancer patients. A method is described for the percutaneous placement of this catheter using a modified Seldinger technique. The catheter was successfully placed in 14 of 15 consecutive cancer patients using this method. The morbidity rates were low, and no deaths occurred because of the procedure. This technique allows rapid, accurate placement of the Hickman catheter in patients whose peripheral venous access sites have been exhausted.

Combination therapy of mitomycin C 40 mg/m2 and medroxyprogesterone acetate 1.500 mg/day in patients with refractory metastatic breast cancer was tested within a phase II trial of the AIO. The overall response was 50% with 2 complete and 12 partial remissions. This combination therapy was tolerated well and hematologic toxicity was less than expected. More often pulmonary and renal toxicity appeared. This combination schedule seems to be useful even in patients with pretreated metastatic breast cancer. Hematologic, pulmonary and renal functions have to be observed.

Most of the serious organic diseases of the colon are etiologically linked to the high-saturated-fat and low-fiber Western diet. Benign but common conditions, such as appendicitis and diverticular disease of the colon, appear to be due to deficiency of fiber and attendant low-bulk stools. Colon cancer appears to be due to carcinogens created in the colon itself. Contributing to carcinogen production are cocarcinogens in bile and an increase in anaerobic bacteria, both directly related to high levels of saturated fat in the diet. If these common disorders of the colon are to be controlled, our diet will require major modification. Changes will have to include reduction of saturated fats of animal origin and increase in cereal grains.

Bestatin, which is a chemically well-defined immunomodulator, was examined for its possible influence on various monocyte functions. It was observed that pretreatment of separated human blood monocytes with Bestatin did not change their chemotactic or random migration. In addition, the capacity of monocytes to reduce nitroblue tetrazolium or to inhibit growth of Chang cells was unchanged by Bestatin. Phagocytosis of yeast particles, however, was increased by pretreatment of monocytes with Bestatin in vitro and it was observed that oral administration of Bestatin to cancer patients augmented the phagocytic activity of blood monocytes in vitro.

We have purified the cell-wall skeletons (CWS) of Mycobacterium bovis BCG, Nocardia rubra, Propionibacterium acnes and Listeria monocytogenes as the adjuvant-active principles of these bacterial cells. These cell-wall skeleton preparations were shown to have antitumor activities in experimental tumor systems. Especially BCG-CWS and N. rubra-CWS were applied for the treatment of human cancer. The synthetic immunoadjuvants such as cord factor, mycoloyl and quinonyl derivatives of N-acetylmuramyldipeptides were prepared and their immunological properties were investigated. The effectiveness of synthetic adjuvants on the stimulation of nonspecific host resistance against bacterial and viral infections was also discussed.

Oral cancer is the most common cancer seen in Papua New Guinea. Although there is a broad geographical correlation with betel chewing, there have been no analytical epidemiological studies and the aetiology remains poorly understood. A greater understanding of several putative risk factors and how they may interact is needed prior to any attempt to modify traditional customs. Although current strategies for primary prevention in India are based on the assumption that the risk of betel chewing is related to the tobacco in the quid, this cannot explain oral cancer in Papua New Guinea. In the latter, special epidemiological and laboratory investigations are needed to provide a scientific basis for future prevention.

Determinations of pregnancy-associated alpha-glycoprotein and immunosuppressive acidic protein in sera of patients have improved a detection rate of ovarian cancer in the early stage. In the clinical treatment of ovarian cancer, we employed the regimen based on results of experimental chemotherapy using heterotransplanted human tumors of the ovary. A regimen of cis-platinum, adriamycin and 5-fluorouracil combination in epithelial tumors and a regimen of cis-platinum, vinblastine and bleomycin combination in germ cell tumors may be recommended for treatment of ovarian cancer.
