This case emphasizes that radiographic changes, although not diagnostic, can be characteristic and highly suggestive of a disease process especially a benign or malignant tumor. Radiographic changes that suggested the possibility of a malignant process in this case included ill-defined margins of the lesion; bone production within the lesion, especially interdentally at the alveolar crest; widening of the periodontal ligament space around tooth roots within the lesions; and perpendicular bony striae extending outward from the surface of the bone (sun-ray effect). THese changes are not specific for a malignant process: however, when coupled with the clinical history, the probability of malignancy is high. Failure of recognition is considered the greatest problem in malignancy of the oral cavity. Clinicians should consider every abnormal condition as suspicious unless there are valid reasons for ruling out cancer.

TITLE: Cancer and other causes of death among leprosy patients.

A follow-up study was done on the mortality from 1956 to 1975 among 2,383 Japanese patients with leprosy who were admitted to a leprosarium in Japan. The leprosy was classified into two types: lepromatous and tuberculoid. Irrespective of the type of leprosy or the sex of leprosy patient, mortalities were increased from tuberculosis, pneumonia and bronchitis, nephritis and nephrosis, and from total causes. The suicide rate was high among female patients. Deaths from total malignant neoplasms were higher than expected among patients with lepromatous leprosy for both sexes (49 observed vs. 44.02 expected), whereas they were lower than expected among patients with tuberculoid leprosy (35 observed vs. 36.83 expected); however, the differences were not statistically significant. Mortalities from cancers of the cervix and the esophagus among females with lepromatous leprosy were significantly higher. The risk of lymphoreticular cancers was not increased.

TITLE: Pulmonary toxicity of mitomycin.

Six patients with breast cancer developed pulmonary toxicity following mitomycin therapy. The presenting symptoms were shortness of breath and a dry cough. The radiological pictures varied from a normal chest x-ray to extensive bilateral pneumonitis. The histological findings consisted of diffuse alveolar damage progressing to interstitial pulmonary fibrosis. Corticosteroid therapy resulted in complete resolution of pneumonia in one patient. Respiratory symptoms improved in 3 patients following discontinuation of the drug. Two patients with extensive bilateral pneumonitis, who were not treated with steroids, died of respiratory failure. Mitomycin-induced lung toxicity appears to be reversible with the discontinuation of drug and the administration of corticosteroids. Lung biopsy is necessary in order to rule out other diagnoses.

The most important consequence of using chemotherapy for Hodgkin's disease is its cure. Results of the MOPP program, developed at the National Cancer Institute in 1964, indicate that the majority of the 80% of patients who attain complete remission, who are at risk for ten years, have remained disease-free. The presence of symptoms and the histologic subtype, nodular sclerosis, has had important negative influences on prognosis. Review of three long-term adverse consequences of chemotherapy revealed sterility to be the most significant effect of drug treatment in this young population. Adverse effects of the immunosuppressive drugs in MOPP proved to be minimal. The carcinogenic effect of MOPP when used alone was surprisingly small, but increased significantly when combined with irradiation. National mortality from Hodgkin's disease has decreased abruptly since the introduction of effective chemotherapy for Hodgkin's disease in 1970.

TITLE: [Alkaline phosphatase isoenzymes and alpha-1-fetoprotein in hepatic neoplasms]

A study was made of possible relations between the placental alkaline phosphatase-isoenzyme (PAP), alpha 1-fetoprotein and liver neoplasia. Ninety-six with various liver diseases were examined, including 10 with primary and 16 with metastatic carcinoma. It was found that both PAP and AFP may be present in a certain percentage of subjects in this pathology. PAP (observed in 30% of cases of primary carcinoma and 43.6% of metastatic forms, as opposed to only 5.6% in cirrhotics) proved much more specific than AFP, which was present in both types of cancer (albeit at much lower concentrations) and also in acute, active chronic and persistent hepatitis, and in cirrhosis of the liver. The theories presented in the relevant literature are discussed in an attempt to explain the "reawakening" of both PAP and AFP, especially in liver neoplasias.

TITLE: Increased risk of colorectal cancer after cholecystectomy.

The relative risk of developing colorectal cancer after cholecystectomy has been assessed in a retrospective autopsy study using controls individually matched for age and sex. Of the 304 patients dying with colorectal cancer and undergoing autopsy examination at the Helsinki University Central Hospital, 45 had undergone previous cholecystectomy. The corresponding number for the matched-pair controls without colorectal cancer was 32, indicating that the relative risk of developing colorectal cancer is slightly (1.59), but significantly (p less than 0.05), increased after cholecystectomy. The relative risk was highest for cancer of the right colon (3.00). In contrast, no difference in the autopsy incidence of unoperated gallstones was observed between the two groups, suggesting that, rather than lithogenic bile alone, the predisposing factor is related to the cholecystectomy procedure itself.

Progress in the immunological detection of malignant tumors has been delayed by two major obstacles: (a) the lack of true cancer specificity of the known tumor-marker substances; and (b) the lack of a close relationship between the concentration and/or first appearance of the marker antigens and the malignant disease, especially at its early stage. An additional, important, and complicating factor implicating the specificity of the reactions of the test substances is the existence of immunological cross-reactions. Until recently, only two approaches have been pursued to resolve this difficulty of cross-reactivity, both concerned with the antigen. Now there is a third way, namely, the production of monoclonal antibodies. The poor correlation between the stage of the neoplastic disease and the level of circulating tumor markers, e.g., carcinoembryonic antigen, may be explained on a histological basis, or by inherent differences in the carcinoembryonic antigen molecule of different tumor sources. Few studies are available dealing with these problems.

Isoelectric focusing in polyacrylamide gel combined with limited proteolysis is a simple and specific method for quantitation of estradiol receptors in breast cancer tissue. At least eight different samples can be analyzed simultaneously on one gel, and the whole procedure, including sample preparation, takes less than 7 hr. In comparison with sucrose gradient centrifugation, isoelectric focusing is more sensitive, possibly due to the short time (1.5 to 2 hr) needed for the analysis. Furthermore, only one incubation with tritium-labeled estradiol is needed for an analysis, which means that a smaller amount of tumor tissue is needed than for most other methods. This fact allows analysis of the estrogen receptor content in tumor material obtained from fine-needle biopsy.

TITLE: Four-year follow-up on the Albany experience with intrapleural BCG in lung cancer.

One hundred sixty-nine patients were entered into randomized controlled studies of intrapleural bacille Calmette Guérin (BCG) immunotherapy after surgical resection of lung cancer. Long-term follow-up of our initial series of BCG-treated patients with Stage I disease continues to indicate that this treatment was superior to that given to control patients. The recurrence rate in the control population was high, 62% at 3 years. The recurrence rate was 33% at 3 years in the BCG-treated group. A negative preoperative tuberculin test and squamous cell histologic type were favorable prognostic factors for BCG-treated patients. The survival of patients with more advanced disease was not improved by BCG immunotherapy.

In the late 1960's when a series of adolescent girls with adenocarcinoma of the vagina presented themselves to our hospital, we did not immediately suspect the cause. Previous experience with radical hysterectomy and with vaginal reconstruction had prepared us technically to treat them sucessfully. Then a clue to etiology from one mother's observation regarding DES as a pregnancy supportive medication was quickly and conclusively converted into fact by an investigation with case controls. Rapid expansion of our knowledge came about through information accumulated in a Registry, and untoward effects other than cancer soon came to notice as young asymptomatic women presented themselves for examination because of known fetal exposure. The dominant event is clearly recognizable ss teratogenic, resulting in anomalous development of the vagina and cervix. Although the appearance of clear cell adenocarcinoma in a small fraction of DES-daughters is a consequence, the role of DES in its carcinogenesis is still unproved.

The ability to predict accurately the clinical course of a patient with a malignancy is critically important to the patient's subsequent management. It has been well documented that the presence of metastatic nodal disease is associated with decreased patient survival. Survival data from a group of 242 head and neck cancer patients from the University of Iowa were analyzed to determine the significance of specific characteristics of metastatic lymph nodes. Evaluation of absolute numbers and percentages of positive nodes or node size generally was not useful. However, involvement of the posterior triangle nodes, noncontiguous nodal sites, or multiple sites was associated with a worse prognosis. When that was accurate enough to be of help to the clinician in prognostication. The single most important feature seems to be documentation of the presence of metastatic nodal disease, rather than particular features (i.e., number, size) of the metastatic nodes.

Metronidazole is currently approved for use against Trichomonas infection and amebiasis. It is also effective against giardiasis, "nonspecific vaginitis," and anaerobic infections and bactericidal against almost all obligately anaerobic organisms; Actinomyces, Arachnia, and Propionibacterium are exceptions. Metronidazole diffuses well into all tissues and penetrates the central nervous system well. It normally has only a relatively minor impact on the colonic flora. The drug is well tolerated. Adverse reactions include reversible neutropenia, peripheral neuropathy, and disulfiram-like reaction when taken with alcohol. Of more concern are its mutagenicity and carcinogenicity in some, but not all, animals. These are considered low risks, and follow-up studies of patients treated to date do not reveal an increased incidence of cancer, but physicians and patients must decide whether the benefit from therapy outweighs the potential risk. Metronidazole should only be used for approved indications, except in the research setting, and should not be used prophylactically, although it is effective.

In more than 16000 gastroscopies we found 65 (0.4%) cases of early cancer (e.c.), that is an amount of 12.3% of the total diagnosed cancers of the stomach. In 8 patients we found two or three sites of involvement in the stomach (12.3%). The control reading of 53 gastrocamerafilms with e.c. shows, that 20% of suspicious areas have been missed by using the routine interpretation and gastroscopy. It is proposed, that before each stomach surgery for e.c., the upper gastric region be investigated carefully for missed second lesions. This is rather difficult with prograde optic gastroscops. The instrument of choice for that purpose is the gastrocamera.

Progress in the management of dysembryoma and choriocarcinoma of the testis has been achieved by more effective chemotherapy (particularly since CDDP was introduced) and by precise timing of surgery, occasional radiotherapy and chemotherapy. In their study, the authors have divided the 61 cancers treated into three stages, depending on whether the lesions were limited to the testis (7 patients with choriocarcinoma and 4 with dysembryoma), had invaded the lymphnodes (16 patients with choriocarcinoma and 9 with dysembryoma) or were disseminated (15 patients with choriocarcinoma and 10 with dysembryoma). The prospects of cure are about 90% for stage 1,75% for stage II and 50% for stage III.

A pharmacological analysis was made of the depolarizing acetylcholine (ACh) response found on the gastric mill I muscles of the crabs Cancer pagurus, Cancer irroratus and Cancer borealis. Acetylcholine, carbamylcholine, trimethylammonium, nicotine, and dimethyl-4-phenyl-piperazinium were effective in producing contractures and depolarizations in these muscles. No response to decamethonium, suberyldicholine, acetyl-beta-methylcholine, carbamyl-beta-methylcholine, pilocarpine and oxotremorine could be detected. High concentrations of muscarinic agonists (10(-4) to 10(-3) M) potentiated and prolonged the ACh iontophoretic response. When the acetylcholinesterase activity was inhibited with neostigmine, or when the response was elicited with carbamylcholine, muscarinic agonists partially inhibited the response. ACh responses were most effectively blocked by vertebrate nicotinic ganglionic antagonists, including dihydro-beta-erythroidine, pempidine, and mecamylamine. alpha-Bungarotoxin was without effect on the ACh response.
