TITLE: Lung cancer in radiochemical industry workers.
PUBDATE: 19940301
The frequency of lung cancer was investigated among 2346 workers in the radiochemical plant 'Mayak' who were exposed to radiation, both externally and internally from incorporation of plutonium. The results from the follow-up were analysed in terms of a linear relative risk model, accounting for the protracted exposures. On the basis of the observed data, it is deduced that the life-span incidence of radiation-induced lung cancer is several times larger than the value 0.0085 Sv-1 that is recommended in ICRP Publication 60 for the risk to this organ.

TITLE: [Medullary thyroid cancer--screening]
PUBDATE: 19940601
It is possible to screen for medullary thyroid cancer (MTC) with biochemical tests, imaging and genetic testing in families with multiple endocrine neoplasia type 2. The recommendation is based on a consensus statement made by the Members of the European Community concerted action for MTC. The prognostic characteristics are based on 117 patients with MTC diagnosed in Denmark between 1970-1991. Twenty percent of all MTC patients were members of families with two or more thyroid cancer patients. More than half of all MTC patients had neck lymph node involvement at the time of diagnosis. The frequency of neck lymph node involvement was however only 20% amongst those with familiar MTC.

PUBDATE: 19901101
Two isoforms of the regulatory subunits of cyclic AMP (cAMP)-dependent protein kinase that bind cAMP are inversely expressed during ontogeny and cell differentiation. These cAMP-binding receptor proteins in harmony may regulate the growth of normal cells and their differentiation into nondividing states. Cancer cells can also be made to differentiate and stop growing when the functional balance of these cAMP receptor proteins is restored by treatment with site-selective cAMP analogues or by the use of an antisense oligodeoxynucleotide, suggesting new approaches to cancer treatment.

PUBDATE: 19910101
The current status of cervical cancer treatment in Denmark is discussed. Diagnostic aspects and problems of classification are presented briefly supplemented with a comment on new prognostic parameters based on a semiquantitative score system and flow cytometry. Surgery is the treatment of election for the early stages whereas radiotherapy is the treatment of choice in advanced stages. Chemotherapy should only be employed in the framework of clinical trials. It is concluded that centralised treatment should be maintained.

Uveal melanoma is an uncommon cancer that is most often diagnosed in the sixth decade and is somewhat more common in males. Early detection and treatment of melanoma is important as metastasis can be fatal. A case of ciliary body melanoma that was detected in an asymptomatic patient because of extrascleral extension is presented. Signs and symptoms that may lead to the diagnosis of uveal melanoma are presented, as well as lesions that may simulate extrascleral extension of melanoma.

PUBDATE: 19920201
DNA-repair is a complex enzymatic process which enables all living cells to withstand the deleterious and mutagenic effects of most genotoxic agents. Defective DNA-repair is caused by mutations involving genes which encode the enzymes responsible for recognition and excision of DNA lesions. Some of these genes have been identified in humans. Several severe human diseases are caused by defective DNA repair affecting the entire genome (e.g. xeroderma pigmentosum and trichotiodystrophy) or only actively transcribed genes (Cockayne's syndrome). Some of these conditions are associated with extremely high rate of cancer.

PUBDATE: 19970401
Nurses play a crucial role in identifying bone metastases and managing clinical sequelae, such as pain. Understanding the metastatic process is necessary for delivering effective nursing care. Part I of this article described the pathophysiology and assessment. Part II will provide an overview of the nursing management of the sequelae of bone metastases, including pain, pathologic fractures, spinal cord compression, hypercalcemia, and anemia. Risk factor identification can lead to prevention and early detection of these clinically significant problems. Clinical management of bone metastases will contribute to the nurse's ability to improve the quality of life of patients with cancer.

Obstructive colorectal cancer is usually an indication for surgical intervention. For patients with metastatic disease, medical contraindications, or those who refuse surgery, palliation by nonoperative means is necessary. The advent of expandable metallic stents may allow for speedy palliation. We have treated one patient with unresectable colon cancer with a new selfexpanding endoprosthesis made of nitinol (Ultraflex, Microvasive, Watertown, MA, U.S.A.).

PUBDATE: 19951001
The oral signs of syphilis have been comprehensively described and can be identified at each stage of the infection. Despite the historical recognition that inadequately treated or untreated syphilitic leukoplakia may undergo malignant transformation, many surgical units no longer routinely screen patients presenting with malignant or premalignant lesions. In this study, five of the 63 patients (8%) who presented with neoplasms of the tongue reacted to syphilis antibodies. These results indicate that syphilis is still prevalent in this group of patients and suggest that serological investigations may be warranted in the initial assessment of all patients with oral cancer.

PUBDATE: 19951101
Paraneoplastic peripheral neuropathies are not common. They are out-numbered by the far more common peripheral neuropathies that occur either as a direct result of the cancer (metastases) or its treatment (usually chemotherapy). Nevertheless, paraneoplastic peripheral neuropathies are important because they may be the first sign of an otherwise occult cancer and/or because they may substantially disable the patient even when the cancer itself is asymptomatic. Paraneoplastic disorders are sometimes marked by the presence of autoantibodies that react with proteins both in the underlying cancer and in the nervous system. Their discovery may lead to an early diagnosis and potential cure of the underlying cancer.

TITLE: Routine follow-up of patients after primary therapy for early breast cancer: changing concepts and challenges for the future.
PUBDATE: 19930301
In an era of increasing financial constraints, physicians are being forced to critically re-evaluate various clinical practices that have not been shown to be cost-effective or associated with definitive patient benefit. Routine follow-up programs following primary therapy for early-stage breast cancer vary widely from centre to centre, and although the subject of numerous retrospective analyses, they have not been prospectively evaluated to date. This review paper presents arguments for changing the emphasis of follow-up visits and stresses the need for prospective clinical and cost-benefit evaluations of current follow-up practices.

PUBDATE: 19940201
The role of radioimmunodetection in the detection of cancer has been established through the development of high affinity immunoconjugates and recent improvements in radiochemistry. Advances in imaging techniques, particularly single-photon emission computed tomography and image registration, also have dramatically improved the sensitivity and accuracy of tumor detection. Molecular engineering of immune constructs provides promise of enhanced tumor localization properties and reduced immunogenicity, allowing repeated studies to be performed. The role of radioimmunodetection in the evaluation of cancer patients will increase in importance in the future.

PUBDATE: 19951101
Radiation therapy is used increasingly as a definitive or adjunctive treatment for cancer. Definitive irradiation allows functional preservation of structures critical to function. Adjunctive irradiation following surgical resection of the primary lesion treats regional lymph nodes and/or sites of primary extension at high risk for microscopic involvement. Specialized techniques can be applied to deliver higher total doses of irradiation and minimize late normal tissue effects to adjacent normal tissues. Local-regional tumor control combined with functional preservation represents the specific goals of radiation therapy applied alone or in conjunction with surgical resection.

PUBDATE: 19950701
Papillary carcinoma, the commonest thyroid malignancy, has a good prognosis and low incidence of distant metastases. Brain metastasis is extremely rare with a frequency of 0.1-5% in reported series. The clinical details of two cases of histologically proven brain metastases from papillary thyroid cancer treated in our centre are presented. In one patient, the lesion was in the cerebellar hemisphere, and in the other, the frontal lobe. Also presented is a review of the literature of this unusual clinical presentation.

PUBDATE: 19960201
The mere mention of prostate cancer strikes fear in the hearts of men. Fortunately, prostate-specific antigen (PSA) is now a recognized tumor marker that can indicate but not exclude the presence of this potentially fatal disease. In this article, Drs Randrup and Baum explain the nature of PSA and the practical application of serum PSA determination. Also discussed are three PSA-based strategies for early diagnosis and guidelines for interpreting PSA values in the primary care setting.
