Benign prostatic hyperplasia (BPH) and prostate cancer are multifactorial disease processes, involving a growing number of biochemical, genetic and epigenetic factors. Their pathogenesis, however, remains poorly understood. The present review examines current morphogenetic concepts of normal and abnormal growth in the human prostate. This includes the role of basal cells in organogenesis and cancerogenesis, the impact of cell-matrix interactions, and the importance of cellular heterogeneity in tumour progression and hormone-insensitive growth. Knowledge of morphogenesis and morphology is required in any scientific approach to BPH and prostate cancer.

The frequency of lung cancer was investigated among 2346 workers in the radiochemical plant 'Mayak' who were exposed to radiation, both externally and internally from incorporation of plutonium. The results from the follow-up were analysed in terms of a linear relative risk model, accounting for the protracted exposures. On the basis of the observed data, it is deduced that the life-span incidence of radiation-induced lung cancer is several times larger than the value 0.0085 Sv-1 that is recommended in ICRP Publication 60 for the risk to this organ.

It is possible to screen for medullary thyroid cancer (MTC) with biochemical tests, imaging and genetic testing in families with multiple endocrine neoplasia type 2. The recommendation is based on a consensus statement made by the Members of the European Community concerted action for MTC. The prognostic characteristics are based on 117 patients with MTC diagnosed in Denmark between 1970-1991. Twenty percent of all MTC patients were members of families with two or more thyroid cancer patients. More than half of all MTC patients had neck lymph node involvement at the time of diagnosis. The frequency of neck lymph node involvement was however only 20% amongst those with familiar MTC.

The methylation of lysine residues on histone tails is catalyzed by proteins containing a conserved SET domain. A recent flurry of structures of SET domain proteins has revealed a new protein fold and a scaffold for understanding catalysis and substrate binding by these enzymes. The prospect that histone methylation might form an epigenetic code and the implicated involvement of SET domain proteins in cancer assures that structure-function studies of these enzymes will continue until their detailed mechanism of action is determined.

Two isoforms of the regulatory subunits of cyclic AMP (cAMP)-dependent protein kinase that bind cAMP are inversely expressed during ontogeny and cell differentiation. These cAMP-binding receptor proteins in harmony may regulate the growth of normal cells and their differentiation into nondividing states. Cancer cells can also be made to differentiate and stop growing when the functional balance of these cAMP receptor proteins is restored by treatment with site-selective cAMP analogues or by the use of an antisense oligodeoxynucleotide, suggesting new approaches to cancer treatment.

The automatic diagnosis of breast cancer is an important, real-world medical problem. In this paper we focus on the Wisconsin breast cancer diagnosis (WBCD) problem, combining two methodologies-fuzzy systems and evolutionary algorithms-so as to automatically produce diagnostic systems. We find that our fuzzy-genetic approach produces systems exhibiting two prime characteristics: first, they attain high classification performance (the best shown to date), with the possibility of attributing a confidence measure to the output diagnosis; second, the resulting systems involve a few simple rules, and are therefore (human-) interpretable.

The current status of cervical cancer treatment in Denmark is discussed. Diagnostic aspects and problems of classification are presented briefly supplemented with a comment on new prognostic parameters based on a semiquantitative score system and flow cytometry. Surgery is the treatment of election for the early stages whereas radiotherapy is the treatment of choice in advanced stages. Chemotherapy should only be employed in the framework of clinical trials. It is concluded that centralised treatment should be maintained.

DNA-repair is a complex enzymatic process which enables all living cells to withstand the deleterious and mutagenic effects of most genotoxic agents. Defective DNA-repair is caused by mutations involving genes which encode the enzymes responsible for recognition and excision of DNA lesions. Some of these genes have been identified in humans. Several severe human diseases are caused by defective DNA repair affecting the entire genome (e.g. xeroderma pigmentosum and trichotiodystrophy) or only actively transcribed genes (Cockayne's syndrome). Some of these conditions are associated with extremely high rate of cancer.

Can sociological variables significantly predict researchers' positions on a scientific issue? Carcinogenesis offers a good test case because it is not definitively established how important man-made substances are as contributors to human cancer. American cancer researchers divide on this issue, some broadly emphasizing the importance of man-made carcinogens, others consistently minimizing their importance relative to natural carcinogens. We find that cancer researchers who are male, are affiliated with a medical school, and have more publications, are more likely than their colleagues to minimize the importance of man-made carcinogens.

The oral signs of syphilis have been comprehensively described and can be identified at each stage of the infection. Despite the historical recognition that inadequately treated or untreated syphilitic leukoplakia may undergo malignant transformation, many surgical units no longer routinely screen patients presenting with malignant or premalignant lesions. In this study, five of the 63 patients (8%) who presented with neoplasms of the tongue reacted to syphilis antibodies. These results indicate that syphilis is still prevalent in this group of patients and suggest that serological investigations may be warranted in the initial assessment of all patients with oral cancer.

Paraneoplastic peripheral neuropathies are not common. They are out-numbered by the far more common peripheral neuropathies that occur either as a direct result of the cancer (metastases) or its treatment (usually chemotherapy). Nevertheless, paraneoplastic peripheral neuropathies are important because they may be the first sign of an otherwise occult cancer and/or because they may substantially disable the patient even when the cancer itself is asymptomatic. Paraneoplastic disorders are sometimes marked by the presence of autoantibodies that react with proteins both in the underlying cancer and in the nervous system. Their discovery may lead to an early diagnosis and potential cure of the underlying cancer.
