TITLE: Considerations of cost and effectiveness in the choice of mastectomy.

Operable breast cancer may be managed equally well by a number of different regimens of mastectomy and/or radiotherapy. This paper examines the financial and morbidity costs, together with the node sampling efficacy, of three commonly employed techniques: simple mastectomy; total mastectomy with node biopsy or limited node excision; and modified radical mastectomy. Adjuvant radiotherapy (and/or chemotherapy) can only be applied rationally if node sampling is effective. Simple mastectomy cannot achieve this. Total mastectomy, providing that a node sample is obtained, is effective, and is probably the cheapest option. Total axillary clearance runs the risk of increased morbidity (and financial cost), while not necessarily guaranteeing more effective node sampling. Much of the cost and morbidity of mastectomy may be reduced by a limited but rational radiation policy.

Delayed hypersensitivity skin responses to costus root oil were measured in patients with colon, stomach, breast and lung cancer and in healthy controls. Cancer patients were staged according to tumor spread. Differences were demonstrated between cancer patients and controls. Patients with localized cancer showed no impairment, but diminished responses did occur in patients with advanced or metastatic cancer. There were no significant differences among any cancer groups with lymph node involvement. In the late cancer stage, costus root oil sensitivity was lost to a greater extent in patients with gastric cancer than in those with colon cancer.

A woman's risk of cervical cancer is generally thought to be related to her sexual behaviour. The sexual background of her male partners is also important. In some societies, a woman's risk of cancer of the cervix will depend less on her own behaviour than on that of her partner. Male sexual behaviour, particularly in relation to prostitution, may account for two hitherto unexplained features of the epidemiology of this disease--the extremely high incidence in Latin America and the decline in mortality this century. If this is so and men carry the aetiological agent, it will be important to discover whether they do so for short or long periods.

The decision of the Ministry of Health to introduce National Cancer Registry and to encourage epidemiological studies, which will ultimately lead to the utilization of data and introduction of control and preventive activities for cancers are positive steps in the right direction. Meanwhile, curative and palliative treatment is available from the existing hospital facilities, and preventive activities such as actions on smoking and health will be continued until such time when a comprehensive prevention and control programme for cancers in the country is evolved.

Increased dietary fat was suggested to promote colon cancer by increasing the levels of free ionized fatty acids and bile acids in the colon contents. In the presence of calcium ions the irritating and toxic effects of the free acids on colon epithelial cells could be reduced by being converted to insoluble calcium soaps. The level of supplementary dietary calcium to supply adequate calcium and thus reduce the potential toxicity of dietary fat was considered.

The procedure of choice in the emergency surgical management of ulcerative colitis and Crohn's colitis is often subtotal colectomy and ileostomy. Further surgery of the retained rectum may be delayed, sometimes for many years. The risk of malignant change in ulcerative colitis is well documented; the cancer risk seems to be much less in Crohn's disease. Five cases are reported where carcinoma developed in the out-of-circuit rectal stump after subtotal colectomy, three cases of ulcerative colitis, and two of Crohn's disease. The importance of regular surveillance or removal of an excluded rectum is emphasized.

Twenty-seven patients with advanced ovarian cancer were treated with CAPF therapy consisting of CPA, ADM, CPDD, and 5-FU. Twenty patients were evaluable for responses. Fourteen patients (70%) had a complete (6 patients) or partial response (8 patients). The median survival time for responders was 12.0+ months and 4.0 for non-responders. Leukopenia and renal toxicity were dose limiting factors. A second-look operation was performed in 12 patients who had favorable results from CAPF. A combined modality with CAPF and second-look operation seemed to be promising.

TITLE: Fine-needle aspiration of metastatic lesions and regional lymph nodes in genitourinary cancer.

A percutaneous fine-needle aspiration biopsy of metastatic tumor and/or retroperitoneal, pelvic, or abdominal lymph nodes was accomplished in 101 patients. A diagnosis of metastatic disease was confirmed by this method in 49 patients. In 26 patients, surgical excisional biopsy or exploratory laparotomy was avoided. An unknown pelvic mass causation was diagnosed in 3 patients. In 15 of 57 patients with localized bladder, prostate, or testis cancer, involvement of regional lymph nodes was confirmed by transabdominal find-needle aspiration. All 57 transabdominal aspirations were performed without complications. The method is inexpensive, safe, and can have a high accuracy in diagnosing a local tumor spread or distant metastases.

TITLE: Should one measure serum thyroglobulin in the presence of anti-thyroglobulin antibodies?

Thyroglobulin auto-antibodies (anti-Tg) may affect measurement of serum thyroglobulin (Tg). We have assayed sera for Tg by RIA regardless of presence or absence of anti-Tg, and this study was designed to assess the influence of anti-Tg on the results. Binding of human anti-Tg by the second antibody in our RIA (sheep anti-rabbit immunoglobulin) is very low and sera containing anti-Tg of varying titres do not systematically affect assay of Tg regardless of concentration. In our follow-up study of patients with thyroid cancer, correlation between clinical state and Tg concentration is greater than 97% whether anti-Tg is present or absent. These results indicate that in our assay anti-Tg positive sera do not need to be excluded.

Bronchography and flexible fiberoptic bronchoscopy (FFB) were performed on 95 patients with suspected lung malignancy. Finally, 66 of 95 lesions turned out to be malignant and 29 benign. Bronchographic diagnosis was correct in 46 of 66 cases of cancer (70%) but gave a false negative result in 20 malignant lesions and a false positive result in four benign lesions of which three had pulmonary tuberculosis and one hamartoma of the lung. It is concluded that bronchography is rarely indicated in central malignant lesions. In peripheral lesions bronchographic findings for malignancy are unreliable or negative.

Cell-mediated immunity was assessed in 12 patients who were long-term survivors of gastric and colorectal adenocarcinomas. A slight decrease in the T-lymphocyte count was accompanied by preserved proliferative reactivity to mitogens (phytohemagglutinin) or alloantigens in 75 percent of the patients. The influence of autologous patient serum on in vitro lymphoproliferative test results was not significant. Selected sera from both study groups showed values of immune complexes that were within the normal range. The colorectal cancer group had antibody-dependent cellular cytotoxicity within the ranges already established for the normal control subjects. Cellular immune mechanisms seem to have been well preserved in long-term survivors of gastric or colorectal carcinoma.

Fourteen patients who underwent composite resections for oropharynx and tongue base cancers had lateral laryngeal suspensions in order to improve postoperative swallowing ability, minimize the surgical defect, and decrease shifting of the mandible after composite resection. Eleven of these patients had significant base of tongue resections (30-90%) and 3 had oropharyngeal resections. Thirteen (93%) were able to eat by mouth, 14 patients (100%) had a normal airway, and 14 (100%) had intelligible speech.

TITLE: Deoxycytidine and deoxythymidine kinase activities in plasma of mice and patients with neoplastic disease.

In C57BL x DBA/2 F1 (hereafter called BD2F1) mice inoculated with P815 neoplasms and in AKR mice with spontaneously developing leukemia, significant amounts of plasma deoxycytidine and thymidine kinase activities were detected in advanced disease. Undetectable or low levels of such kinase activities were observed in normal BD2F1 and in control AKR mice. Initial studies with leukemia patients revealed increased amounts of plasma deoxycytidine and thymidine kinase activities correlating favorably with the peripheral white blood cell counts. Initial studies with small numbers of patients with solid tumors revealed significant activities of both kinases in plasma of patients with four different cancers. Healthy volunteers revealed enzyme activities only insignificantly above background.
