Atypical ductal hyperplasia of the breast is a benign proliferative condition that is associated with an increased risk of development of breast cancer in either the ipsilateral or contralateral breast. We present two cases in young women, facing prolonged follow-up, one managed with observation only, and the other managed with ipsilateral mastectomy and reconstruction.

A considerable amount of data indicates that transplanted patients are at increased risk for de novo and recurrent cancer. Treatment of this population is difficult. It remains unclear if the immunosuppressive therapy should be continued, tapered or even stopped or if immunosuppressive drugs with antiproliferative properties have beneficial effects in this situation. In various models, mTOR-inhibitors were shown to have immunosuppressive and anti-tumor effects. Here, we have reviewed the current literature trying to clarify if mTOR-inhibition brings advantages for the transplanted patients suffering from tumors.

Esophageal cancer is a devastating disease. Patients are most likely to first present to a primary care physician, who should immediately undertake an initial diagnostic and staging evaluation. The approach then should become multidisciplinary, involving surgical and medical oncologists, radiation oncologists, and gastroenterologists. Optimal therapy of esophageal cancer continues to evolve. While multimodality therapy remains controversial, studies are ongoing to determine the best approaches to improve survival. Further studies to define the role of surveillance endoscopy in certain high-risk patients are warranted.

Recent advances in molecular biology have unveiled the mechanisms of carcinogenesis, especially colorectal carcinogenesis. A large body of evidence supports the idea that accumulated genetic changes due to inherited as well as environmental factors underlie colorectal cancer development. Recently, several reports have suggested that aberrant methylation of the CpG island may also play an important role in colorectal carcinogenesis. This genetic information is not yet sufficient but some useful genetic information can be applied clinically. This paper presents the results of intensive study of microsatellite instability in colorectal cancers and some clinically applicable discoveries. Future problems are also discussed.

Since the 1970s primary management for regionally advanced non-small cell lung cancer has shifted from radiotherapy alone to sequential chemoradiation to concurrent chemoradiation. The increase in survival with these approaches has been small; an approximately 3-4 month per decade increase in median survival. Future avenues to improve on these outcomes could involve: i)dose-intense radiotherapy; ii) better target delineation; and iii) combining molecularly targeted agents with optimised radiation therapy. However, to accomplish this, techniques to control tumour motion and decrease toxicity must be developed.

When a paraneoplastic Cushing syndrome occurs, it is an independent factor of poor prognosis. Management is extremely complex. Treatment must be based on high dose inhibitors prior to chemotherapy. Opportunistic infections, often fungal infections, are the main complications, even outside periods of aplasia, and cause significantly earlier mortality.

The technique, known as electrochemotherapy (ECT), uses high-voltage pulses to deliver drugs across cancerous tissues. Recent studies also indicate that the applications of ECT can be extended from the treatment of cutaneous cancers to the treatment of tumors of vital organs such as brain, liver, lungs and others. Bronchioloalveolar carcinoma occasionally presents with bronchorrhea and can appear on roentgenogram as cavitary lesions similar to a lung abscess. We present a case of multifocal, cavitary bronchioloalveolar cancer, which was originally treated unsuccessfully as lung abscesses. At surgery, two cavities were identified without evidence of tumor or chronic infection and were left open. The postoperative course was complicated by 5 weeks of massive chest tube drainage, which was finally controlled with radiation and time. This report chronicles our difficulty with diagnosis and management and suggests recommendations for future providers.

The progress of study on recombinant BCG was stated briefly. And then our studies on recombinant BCG were mentioned. Recombinant BCG secreting alpha antigen-fused merozoite surface protein 1 (MSP 1) was prepared and tested for its ability to control infections of Plasmodium yoelii. Result turned out it controlled the infection better than recombinant MSP 1 mixed with Freund incomplete adjuvant did. Recombinant BCG secreting excess amounts of antigen 85 complex A controlled infection of Mycobacterium leprae. Addition of recombinant BCG secreting alpha antigen-fused IL-2 to peritoneal exudate cells induced IFN-gamma resulting in killing bladder cancer cells more efficiently than parental BCG did.
