TITLE: Considerations of cost and effectiveness in the choice of mastectomy.

Operable breast cancer may be managed equally well by a number of different regimens of mastectomy and/or radiotherapy. This paper examines the financial and morbidity costs, together with the node sampling efficacy, of three commonly employed techniques: simple mastectomy; total mastectomy with node biopsy or limited node excision; and modified radical mastectomy. Adjuvant radiotherapy (and/or chemotherapy) can only be applied rationally if node sampling is effective. Simple mastectomy cannot achieve this. Total mastectomy, providing that a node sample is obtained, is effective, and is probably the cheapest option. Total axillary clearance runs the risk of increased morbidity (and financial cost), while not necessarily guaranteeing more effective node sampling. Much of the cost and morbidity of mastectomy may be reduced by a limited but rational radiation policy.

Delayed hypersensitivity skin responses to costus root oil were measured in patients with colon, stomach, breast and lung cancer and in healthy controls. Cancer patients were staged according to tumor spread. Differences were demonstrated between cancer patients and controls. Patients with localized cancer showed no impairment, but diminished responses did occur in patients with advanced or metastatic cancer. There were no significant differences among any cancer groups with lymph node involvement. In the late cancer stage, costus root oil sensitivity was lost to a greater extent in patients with gastric cancer than in those with colon cancer.

TITLE: A preliminary study on the link between PUVA and skin cancer.

One hundred four psoriatic patients who had 8-methoxypsoralen-ultraviolet light A (PUVA) treatment between 1975-1987 were investigated to determine if skin cancer had ever occurred during or after the therapeutic period. Incidence data of skin cancer was applied to this group of patients to measure the expected number of patients with skin cancer. The significance of observed number versus expected of PUVA-treated patients with skin cancer was analyzed. The authors did not observe a significant increase of patients with skin cancer.

A 68-year-old Japanese woman presented with complaints of appetite and weight loss. Early pancreatic duct adenocarcinoma was diagnosed by endoscopic retrograde cholangiopancreatography (ERCP), which revealed partial obstruction of the main pancreatic duct. Pancreatoduodenectomy with lymph node dissection was performed. Macroscopically, no pancreatic tumor was detected. Histologically, the pancreatic lesion showed continuous changes consisting of duct epithelial cell hyperplasia, carcinoma in situ, and invasive carcinoma. Immunohistochemical stains for carcinoembryonic antigen and CA19-9 were positive in cancer cells.

Eight patients were subjected to two types of preoperative investigation in the attempt to stage cancers localised in the final 13 cm of the rectum: CT scan and transrectal echography. CT scan was used to assess infiltration of perirectal tissues and any lymph node metastases. Echography was used to examine the degree of infiltration of the rectal wall. Patients were then submitted to surgery. The predictivity of these studies proved sufficiently high. 7 Miles amputations and 1 anterior resection were carried out. One case only in which the scan did not show signs of parietal lesions presented two perirectal lymph node metastases. If refined further, these evaluations will therefore influence surgical tactics.

A woman's risk of cervical cancer is generally thought to be related to her sexual behaviour. The sexual background of her male partners is also important. In some societies, a woman's risk of cancer of the cervix will depend less on her own behaviour than on that of her partner. Male sexual behaviour, particularly in relation to prostitution, may account for two hitherto unexplained features of the epidemiology of this disease--the extremely high incidence in Latin America and the decline in mortality this century. If this is so and men carry the aetiological agent, it will be important to discover whether they do so for short or long periods.

Experiences of cancer patients' self-help groups can provide impulses for the programming of rehabilitation measures. Exemplary is the information work (information procurement and distribution) of these groups. Problems emanating from disturbed interpersonal relationships and the stigma of the disease are dealt with implicitly rather than explicitly, with only certain patients, however, benefitting selectively from this kind of coping, depending on the stage of the disease and on their social context. The groups succeed in substituting missing or disturbed interpersonal relationships, they sometimes however find it difficult to ward off those aspects of isolation that have arisen with the onset of the disease. These difficulties weigh on other parts of the informal network, especially on the families.

Some information on cancer in Malaysia are available, and its epidemiology is described. There is a need for systematic and coordinated collection of cancer statistics which are essential to patient management, cancer control programme formulation, implementation and evaluation. The decision of the Ministry of Health to introduce National Cancer Registry and to encourage epidemiological studies, which will ultimately lead to the utilization of data and introduction of control and preventive activities for cancers are positive steps in the right direction. Meanwhile, curative and palliative treatment is available from the existing hospital facilities, and preventive activities such as actions on smoking and health will be continued until such time when a comprehensive prevention and control programme for cancers in the country is evolved.

Direct chromosome preparations were made on a brain metastasis of a large cell lung cancer from a 46-year-old female patient. In the 169 cells examined, the modal chromosomes number was 73, and the majority (53.2%) of mitoses were in the near-triploid region. The tumor cells showed considerable heterogeneity in chromosome number and content. Chromosomes 3, 4, and 5 were the most frequently involved in aberrations. The mean number of unidentified marker chromosomes was five per mitosis.

To achieve better understanding of the factors influencing local control of ovarian cancer, an analysis of failure in early stages of ovarian carcinoma has been carried out. The material consisted of 185 patients diagnosed in early stages; 40 of them failed in their disease, 17 with local recurrence, 23 with distant metastases. The material was analyzed with respect to age, histology, grade, size and surface of the tumor, the surgical procedure and treatment modalities, and the time between treatment and recurrence, and recurrence and death. In the analysis, both the behavior of the ovarian cancer tumor and the pattern of failure was shown to be different for each histological group.

The clinical applications of GnRH and its analogues are discussed. GnRH can be used diagnostically to assess the releasable pool of LH and the status of gonadotrophin secretion. Clinically it can be used to induce follicle growth and ovulation in patients with hypogonadotrophic amenorrhoea, especially when given in pulsatile form, and there are reports that it might also stimulate spermatogenesis in men with hypogonadotrophic hypogonadism. Analogues are proving valuable in the treatment of precocious puberty, but are not yet practical for contraception. They are more valuable in the treatment of endometriosis, hormone-dependent cancers, the polycystic ovary and for the induction of ovulation for in-vitro fertilization.

Increased dietary fat was suggested to promote colon cancer by increasing the levels of free ionized fatty acids and bile acids in the colon contents. The procedure of choice in the emergency surgical management of ulcerative colitis and Crohn's colitis is often subtotal colectomy and ileostomy. Further surgery of the retained rectum may be delayed, sometimes for many years. The risk of malignant change in ulcerative colitis is well documented; the cancer risk seems to be much less in Crohn's disease. Five cases are reported where carcinoma developed in the out-of-circuit rectal stump after subtotal colectomy, three cases of ulcerative colitis, and two of Crohn's disease. The importance of regular surveillance or removal of an excluded rectum is emphasized.
