A review on the chemotherapy in advanced pancreatic cancer is presented. Pancreatic cancer is an extremely chemotherapy resistant tumor and results of cytostatic drug treatment so far are unsatisfactory. For patients with locally advanced disease the combination of chemo- and radiotherapy may palliate some patients. To date no standard treatment is available and patients should be entered in clinical protocols assessing new treatment options.

TITLE: Palliation of biliary obstruction in patients with advanced breast cancer using endoscopic stents.

Endoscopic insertion of a biliary endoprosthesis was successful in eight patients with extrahepatic biliary obstruction caused by breast cancer metastases. The serum bilirubin level was significantly reduced in seven patients and in four this was accompanied by marked symptomatic improvement. Endoprostheses required replacement after a median of 8 (range 3-127) weeks. Two patients responding to systemic anticancer therapy survived 27 and 43 months. Endoscopically placed stents offer effective palliation of extrahepatic biliary obstruction caused by metastatic breast cancer and long-term survival may be possible.

A case is described of a 71 year old woman with scirrhous breast cancer and multiple hypoechogenic lesions in the spleen. The patient was treated successfully by mastectomy and splenectomy. Morphologic examination of the spleen showed a primary, centroblastic-centrocytic, low grade non-Hodgkin's lymphoma. Eight months after splenectomy the patient still remains in complete remission.

Monoclonal antibodies E-11 and G-4 were purified by Sepharose 4B column chromatography and labelled with europium as tracer. The solid-phase antibodies were prepared by attaching pure E-11 and G-4 to polysterene strips. The supernatant of 8611 cell culture or perchloric acid extracts from ovarian cancer tissues provided standard antigens. The solid-phase sandwich-type DELFIAs for monoclonal antibodies E-11 and G-4 were thus established, and the resulting standard curves were extremely steep. The ratio of CPS in high-concentration tubes to CPS in the zero tube exceeded 300:1, indicating the very high sensitivity and accuracy of this method.

Tyrosine kinases are associated with the cytoplasmic domains of growth factor receptors as well as oncoproteins and many have the potential to cause transformation if mutated or hyperexpressed. Tyrosine kinases therefore represent an excellent target for the development of cancer drugs. A large number of inhibitors have now been identified and many show promising cytostatic activity, particularly using in vitro models. Some in vivo activity has been reported. Progress with various structural classes is reviewed. It is not clear whether specific or broad spectrum tyrosine kinase inhibitors should be developed as potential anticancer drugs. It does seem likely, however, that tyrosine inhibitors will enter clinical trial in cancer patients.

The analysis of clinical and immunologic findings indicate that retinol and decaris influence functional activity of immune cells at stage II of lung cancer irrespective of histological structure. In stage III, retinol induced changes in proportion of lymphocyte subpopulations, more evident in glandular cancer. Both immunomodulators lowered the number of purulent complications after surgical interventions. It is only retinol that influenced metastatic dissemination in lung cancer stage II-III.

The method of the operation and its main poits were described in detail; the indications of the operation were discussed. The anastomosis was influenced by the length, blood supply and cleaning of the proximal colon. Anastomotic leakage and pelvic infection were the main surgical complications.

At autopsy, a spinal cord metastasis was found. The poor results of the National Breast Screening Study of Canada regarding the screening of women aged 40-49 have been interpreted to suggest that mammography might be detrimental. Our comparison of women who had mammography and had their cancers detected by mammography with women who did not have mammography and had palpable cancers suggest that this is not the case. There is no detrimental effect; rather, the use of mammography results in an improvement in survival. The poor results of the National Breast Screening Study likely are due to the unbalanced allocation of women with advanced cancers to the screened group, the poor quality of the mammography in the trial, and an insufficient sample size.

Hypercalcemia is a common metabolic problem in patients with metastatic breast cancer. Osteolytic bone destruction with release of calcium into circulation and humoral factors can lead to hypercalcemia. Clinical manifestations may be so mild as to be overlooked or include severe nervous system, gastrointestinal and renovascular complications. Treatment with saline infusions to replenish intravascular volume and restore renal function provides the basis for other therapeutic interventions. A variety of pharmacologic approaches include intravenous fluids, diuretics, steroids, calcitonin, bisphosphonates, and plicamycin. Investigation of new agents, particularly bisphosphonates and gallium, continues. The optimal treatment of hypercalcemia in breast cancer patients has not been defined, though control of metastatic disease obviously is of utmost importance.

Mortality statistics serve as the most common tool for health policy planning. However, this method suffers from several shortcomings. The mortality patterns in Israel in 1986 are presented in this article using the measure of years of potential life lost (YPLL), which better expresses premature mortality and the burden of diseases on society. Ischemic heart disease, cancer, and accidents of all types are responsible equally for most premature deaths in males in Israel, whereas cancer is the leading cause of premature mortality in women in Israel. Planning future health policy should rely more heavily on measures such as YPLL.
