TITLE: [A clinicopathological study of five cases of adenosquamous carcinoma of the stomach]

Five cases of adenosquamous carcinoma with resected stomach were subjected to this study, and the clinicopathological characteristics of this condition were described. The five patients represented 0.8% of 589 cases of resected stomach. There were two men and three women ranging in age from 40 to 70 (average, 62.8). Macroscopically, the adenosquamous carcinomas were all advanced gastric cancer situated in the lower body along the lesser curvature of the stomach, averaging 7.6 cm in size. The characteristic finding was the histologically differentiated type, invading to the serosa. Lymph node metastasis was most prominent.

Long-accepted prognostic factors for breast cancer patients include the number of axillary lymph nodes positive for cancer; size of the primary; histopathologic features such as nuclear grade, histologic grade, and mitotic index; and the estrogen and progesterone receptor status. Unfortunately, these factors are relatively insensitive in predicting which women are particularly at risk of relapse. Newer discoveries which are or may be better prognosticators of a breast cancer patient's future course include growth factors, oncogenes, thymidine labeling index, and measurement of DNA ploidy status by flow cytometry.

Sixty patients with esophageal cancer infiltration of mediastinal structures and metastasis to lymph nodes were prospectively evaluated by computed tomography and magnetic resonance imaging and then underwent surgical resection of the tumor (n = 57) or diagnostic thoracotomy (n = 3). Lymph nodes were excised from standardized locations and examined histologically. Sensitivity and specificity of both methods for all criteria were found so low that the value of these methods for planning surgery--or for stratification to different therapeutic arms in comparative studies--must be questioned.

During the investigation of 41 patients with 111In-bleomycin false-positive results were noted in all the cases. These results permit recommending this method for defining colon cancer spreading.

28,500 fiberoptic endoscopies were reviewed. 97 cancers of the stump were found in 1,119 patients who had had gastric resection (8.66%), in contrast to the 3.9% of cancers found in nonoperated patients. This suggests that the gastric remnant has a higher tendency to develop cancer than the nonresected stomach. A policy of periodic follow-up of these patients must be considered in order to detect the tumor at an early stage.

In a patient with long untreated slow-growing osteoblastic bone metastases from an ER/PgR negative breast carcinoma, new metastatic sites in many lymphnodes pleura and massively in liver have been observed 4 weeks after receiving tamoxifen. Cause-effect relationship between administration of the drug and this unusually rapid spreading of the disease may be considered only as a hypothesis, as it is based wholly on clinical outcome. However, reporting of any exacerbation of breast cancer apparently induced by tamoxifen, even if only anecdotal, is advisable.

The authors evaluated the diffusion of conservative treatment of breast cancer in Italian radiotherapy departments during 1984-1985. T1 and small T2 (less than 3 cm in maximal diameter) cases were particularly considered. Conservative surgery and radiotherapy of breast cancer was adopted in 88.4% of the Italian radiotherapy centers. Quadrantectomy and axillary dissection was the surgical procedure routinely adopted. Large differences emerged in radiation treatment among the different departments, particularly with regard to the irradiation of regional lymph nodes. A total of 1561 T1 and 279 T2 cases were submitted to a conservative treatment in 1984, and respectively 2185 and 334 in 1985. The 1985 versus 1984 increase in cases treated was 40% for T1 and 18% for T2.

A clinicopathological study of 9 cases presenting axillary mass alone was reported. Histological diagnosis revealed 3 occult breast cancers, 2 apocrine gland adenocarcinomas of the skin, a carcinoma and a fibroadenoma in the axillary accessory breast, a neurogenic tumor in the axilla, and a benign lymphadenopathy. There were no clinical characteristics helpful in preoperative differential diagnosis. As the axilla includes the main lymphatic routes from the breast, the axillary tail of the breast and accessory breast, breast-associated tumors should be kept in mind in the management of patients with axillary mass as a sole clinical feature.

A prospective study of transmission spectroscopy (lightscanning) has been performed on 285 women presenting with symptomatic breast disease to one outpatient clinic. Of the 41 patients with histologically confirmed breast cancer lightscanning detected 36 (87.8%). In the study group 164 patients had both lightscans and mammography performed and, of the 30 biopsy-proven carcinomas in this group, lightscanning detected 26 (87%) and mammography 25 (83%). Lightscanning was positive in five of the six non-invasive breast cancers and in 13 of the 15 invasive carcinomas 2 cm or less in diameter. No carcinoma was misdiagnosed as benign when the results of both investigations were combined.

Recent reviews of the management of hilar biliary cancer suggest that radical resection is the preferred treatment option. It seems true that most of the long term survivors are treated this way, but it is equally true that most patients receive palliative rather than curative treatment. Before we go too far in our advocacy of radical treatment, we need to develop methods for quantifying palliation because that is in truth what we usually achieve. A method of quantifying palliation is described. Surgeons are encouraged to use methods of this kind in order to allow valid comparison of the results of different methods of treatment.
