TITLE: Non-specifically bound immunoglobulins on mouse placental cells.

There seem to be some similarities between mechanisms employed by embryos and cancer cells from the maternal or tumor bearer's immune system. In the present study, we investigated the presence of hemagglutinins on placental cells and their relationship to circulating antibodies in mice immunized against sheep erythrocytes before mating. The results obtained demonstrated that placental cells may fix large quantities of maternal immunoglobulins. The hemagglutinins are probably fixed on placental cells by the Fc, since the combining sites of the antibody were free and able to react with the antigen in the "rosette" test. It was also demonstrated that placental tissue may fix up to 1/4 of the maternal circulating antibodies, while only 1/200 of them were transmitted to the fetuses. Some theoretical implications of the presence in the placenta of antibodies not directed against feto-placental antigens are also discussed.

Incidence of colorectal cancer is increasing during the last years. 5% of the population get affected. From these a group of patients with a higher carcinoma risk must be separated: that with adenomas, familial polyposis and ulcerative colitis. Nourishment seems to be one of the main factors for the development of cancer. Although all important principles of radical surgery are realized, this 5-year survival rate for colorectal cancer is less than 40%. Prophylaxis, early diagnosis and the effect of irradiation and drug therapy must improve the relatively poor prognosis of colorectal cancer, which has reached in Germany the highest mortality and even -- with the exception of skin cancer -- the highest morbidity of all malignant neoplastic diseases.

A report is given on the results of 12 radiological explorations of the bladder in 12 patients with pseudo-neoplastic sigmoiditis. In all cases the bladder had an abnormal aspect, in two cases there was a pseudotumor, in two an inflammatory aspect, in four conical attraction of the bladder by the sigmoid. In all 12 cases morphological anomalies of the filled bladder were seen. The ureters were either normal or dilated but without true stenoses. These various aspects may contribute to the differential diagnosis between sigmoiditis and cancer. The differential diagnosis of such vesical lesions is discussed.

This study was performed in order to elucidate some of the problems of incidence, morphology and natural history concerned with Stage A prostatic cancer or prostatic microcarcinoma (PMC). The prostates of 100 patients, treated by subtotal prostatectomy for benign prostatic hyperplasia (BPH), were studied by comparing both routine and step-section techniques. The incidence of PMC was 41% by the former and 86% by the latter technique. Assessment of the size of PMC, as measured by the sum of the two main diameters, resulted in three groups: A1, A2, A3. The last of these may represent a frankly malignant condition, judged by size and the histological appearance. Radical prostatectomy is strongly suggested as appropriate therapy for this group.

TITLE: Chemotherapy for advanced cervical cancer with methotrexate, bleomycin, and cis-dichlorodiammineplatinum(II).

Nine patients with advanced cancer of the uterine cervix, having failed radiotherapy, were treated with a combination regimen of parenteral methotrexate, bleomycin, and cis-dichlorodiammineplatinum(II). Eight of the patients had objective partial remissions lasting a median of 4 months. All patients improved subjectively. Five of the patients, however, had sudden unpredictable onset of severe myelosuppression accompanied by stomatitis. Such frequent severe toxicity was not observed in patients with other malignancies treated with this combination. It is likely that impaired excretion of methotrexate resulting from ureteral dysfunction led to increased toxicity. In future protocols for advanced cervical cancer, methotrexate will be replaced by other effective agents. The very high response rate (89%) suggests that similar but less toxic combinations may contribute substantially to the future effective treatment of advanced cervical cancer.

Chlorozotocin was given to 37 patients with advanced malignant tumors in a daily X 5 schedule at 6-week intervals. Total iv doses for each course ranged from 75 to 200 mg/m2. Myelosuppression was dose-limiting, with a platelet count depression regularly observed at doses of greater or equal to 150 mg/m2; leukopenia occurred only at the highest dose level. Nausea and vomiting were mild and uncommon. No hyperglycemia or adverse drug-related effects on renal or hepatic function were observed. No major antitumor activity occurred; however, three patients with renal cell carcinoma and one patient each with lung cancer, ovarian carcinoma, and Hodgkin's disease had minor objective decreases in tumor size. A dose range of 150--200 mg/m2 iv for each 5-day course is recommended for phase II studies.

Most patients with bladder cancer initially present with localized, potentially curable tumours. Endoscopic surgery offers the best opportunity to eliminate these early lesions, but the rate of tumour recurrence after adequate resection is high (around 70%). Conventional methods of treatment have a place in the management of early bladder neoplasms, but their success rate is still unsatisfactory and they frequently fail to decrease the risk of recurrence. New drugs and more effective forms of administration have enhanced the use of chemotherapeutic agents. Fundamentally different approaches, such as specific immunotherapy, the use of laser energy and photodynamic therapy, are emerging as valuable approaches in the treatment of superficial bladder cancer and the prevention of recurrence. Randomized trials to assess their value and a concerted multidisciplinary effort with combined treatment give hope for effective control of early bladder cancer.

The prognosis of cancer patients with metastases to the bone has changed little during the past 10 years despite improvement of diagnostic and therapeutic possibilities. A review of the patients treated at the Surgical Clinic of the Erlangen University between 1969 and 1975 showed that out of 78 patients with metastases to the bone only one survived 5 years. In this case a ultraradical method of treatment--hemipelvectomy--was employed. Three groups of comparison were formed; one of them being treated conservatively, the second by palliative osteosynthesis and the third by resection. In the first and second group no patient was alive after 4 1/2 years. The mean survival time was 6,7 months for patients treated conservatively and 11,2 months for those in whom osteosynthesis was used. Patients treated by resection survived 16,3 months on average.

Based on data from the cancer register of the German Democratic Republic established in 1952 and on the official mortality statistics, incidence of and mortality from malignant lymphomas (ICD 200-203) in the GDR are analysed. Age-specific incidence and mortality of Hodgkin's disease show a peak in the age group of 25-30 years and rise steadily from 45 years on up to the highest age. Lymphosarcoma and reticulosarcoma increase slowly from infancy to old age, whereas multiple myeloma is a disease of the elderly and extremely rare before the age of 40. The apparent increase of malignant lymphoma may be due to underregistration at the beginning of the cancer register. In the past years mortality from Hodgkin's disease is slowly decreasing, thus reflecting progress in methods of treatment and results.

Radiation therapy has been an integral part of curative cancer therapy for many decades. The tolerance of normal tissues traversed by radiation and resistant tumor cell populations traditionally have limited the radiocurability to certain tumors, especially with higher clinical stages. However, research in radiobiology and radiation physics is providing ways to increase cure while limiting morbidity. Computer-controlled dynamic treatment, radioprotector drugs, hyperbaric oxygen, carbogen breathing during irradiation, particle irradiation, and hypoxic cell sensitizing drugs are presently undergoing clinical evaluation with some encouraging preliminary results. We review the fundamental concepts underlying these clinical trials and analyze the results.

Experience with the EEA stapler device used in 30 esophagogastric resections for cancer with intrathoracic anastomosis, is reported. The mortality rate was 13.3%. The anastomotic failure rate was 3.3% (1/30) with only one death; three asymptomatic blind fistulas were found on a routine contrast examination of the anastomosis. It is felt that esophagogastric EEA stapled anastomosis associated with an omental graft is a very safe technique.

Cancer of the gallbladder is an uncommon malignancy which is seldom diagnosed early and often denotes a poor prognosis. Two reported cases describe the association of hyperhaptoglobinemia (greater than 700 mg./dl.) and advanced carcinoma of the gallbladder. When a patient presents with liver involvement from metastatic adenocarcinoma of suspected gastrointestinal origin and a markedly elevated serum haptoglobin (greater than 700 mg./dl.), it is suggested that a high index of suspicion be held for carcinoma of the gallbladder.

TITLE: Gastric involvement in Sjögren's syndrome simulating early gastric carcinoma.

The case of a 42-year-old woman with Sjögren's syndrome accompanied by severe gastric infiltration and interstitial nephritis is reported. Presentation was with finger stiffness, Raynaud's phenomenon and abdominal discomfort. There were endoscopic and radiological features of early cancer, type IIc (depressed type). Resected stomach showed atrophic gastritis with erosion. Histology showed focal to relatively diffuse destruction of the gastric glands with minimal intestinal metaplasia and dense collections of lymphoid cells with occasional germinal centers in the interstitium of the gastric mucosa. Renal biopsy revealed similar severe diffuse interstitial infiltration of lymphoid cells. Distal renal tubular acidosis was confirmed by sodium bicarbonate and ammonium chloride loading test. Satisfactory improvement in xerostomia, keratoconjunctivitis sicca and other laboratory data occurred on prolonged administration of adrenal steroids and cyclophosphamide.
