TITLE: Adjuvant tamoxifen treatment in postmenopausal patients with operable breast cancer.

Between November 1, 1976 and December 31, 1981, 826 post-menopausal females with operable breast cancer were included into a trial comparing tamoxifen 40 mg daily for 2 years with no endocrine treatment. Patients without axillary lymph node metastases and tumors less than 30 mm received no other treatment whilst those with more advanced disease were in addition superrandomised to receive postoperative irradiation or 12 courses of CMF. With a mean follow-up of 44 months tamoxifen significantly reduced the incidence of recurrence. There was no significant interaction between the effect of tamoxifen and any other treatment or prognostic subgroup.

An immunoassay for prostatic acid phosphatase is described in which a high degree of specificity for the prostatic isoenzyme, obtained by the use of monoclonal antibodies, is combined with great sensitivity, made possible by enzyme-amplified measurement of the combination of the isoenzyme with its antibody. The increase in sensitivity thus achieved is of the order of 170 times that of conventional methods of measurement. The advantages of the enzyme-amplified method have been shown to be particularly useful in detecting and monitoring small abnormalities of prostatic acid phosphatase levels in patients with prostatic cancer.

TITLE: [A clinicopathological study of five cases of adenosquamous carcinoma of the stomach]

Five cases of adenosquamous carcinoma with resected stomach were subjected to this study, and the clinicopathological characteristics of this condition were described. The five patients represented 0.8% of 589 cases of resected stomach. There were two men and three women ranging in age from 40 to 70 (average, 62.8). Macroscopically, the adenosquamous carcinomas were all advanced gastric cancer situated in the lower body along the lesser curvature of the stomach, averaging 7.6 cm in size. The characteristic finding was the histologically differentiated type, invading to the serosa. Lymph node metastasis was most prominent.

Long-accepted prognostic factors for breast cancer patients include the number of axillary lymph nodes positive for cancer; size of the primary; histopathologic features such as nuclear grade, histologic grade, and mitotic index; and the estrogen and progesterone receptor status. Unfortunately, these factors are relatively insensitive in predicting which women are particularly at risk of relapse. Newer discoveries which are or may be better prognosticators of a breast cancer patient's future course include growth factors, oncogenes, thymidine labeling index, and measurement of DNA ploidy status by flow cytometry.

Sixty patients with esophageal cancer infiltration of mediastinal structures and metastasis to lymph nodes were prospectively evaluated by computed tomography and magnetic resonance imaging and then underwent surgical resection of the tumor (n = 57) or diagnostic thoracotomy (n = 3). Lymph nodes were excised from standardized locations and examined histologically. Sensitivity and specificity of both methods for all criteria were found so low that the value of these methods for planning surgery--or for stratification to different therapeutic arms in comparative studies--must be questioned.

The co-culture of mouse peritoneal macrophages and Lewis lung carcinoma cells induces the release of a metal-dependent type IV collagen-degrading proteinase which is not produced in detectable amounts by either cell type cultivated alone. Conditioned media of the co-cultures degrade both pepsin-extracted type IV collagen from human placenta and mouse type IV procollagen. Thus macrophages can interact with tumor cells to degrade basement membrane type IV collagen: this might be of importance to allow cancer invasion and metastasis.

A strategy for treatment of disseminated cancer is proposed which unites short-course chemotherapy, local hyperthermia, and allogeneic lymphocyte immunotherapy, designed and timed so as to optimize their synergistic interactions. Short-course chemotherapy potentiates allogeneic lymphocyte immunotherapy, and in turn is potentiated by subsequent courses of local hyperthermia. Response to hyperthermia can be enhanced by measures which selectively inhibit tumor blood flow, impair tumor energy metabolism, and produce tumor acidification; these include induction of prolonged hyperglycemia, and administration of selectively activated "hypoenergic" agents which inhibit or uncouple tumor respiration. It is anticipated that these methods will enable a significant advance in the control of metastasized solid tumors.

During the investigation of 41 patients with 111In-bleomycin false-positive results were noted in all the cases. These results permit recommending this method for defining colon cancer spreading.

28,500 fiberoptic endoscopies were reviewed. 97 cancers of the stump were found in 1,119 patients who had had gastric resection (8.66%), in contrast to the 3.9% of cancers found in nonoperated patients. This suggests that the gastric remnant has a higher tendency to develop cancer than the nonresected stomach. A policy of periodic follow-up of these patients must be considered in order to detect the tumor at an early stage.

Carcinosarcoma is a tumor-containing malignant epithelial and connective tissue elements. The 18th case of carcinosarcoma of the gallbladder is reported in the context of 17 prior cases from the world literature. The 18-year experience of St. Luke's Hospital with extrahepatic and primary gallbladder cancer is presented to provide perspective on this unusual tumor. The diagnosis of malignant disease of the gallbladder is reviewed with special emphasis on the roles of sonography, computerized tomography, and arteriography. The importance of staging as a guideline to prognosis and therapy of gallbladder cancer is discussed. Carcinosarcoma of the gallbladder behaves clinically like carcinoma and guidelines for surgical and palliative therapy of the more common malignancies of the gallbladder should be followed for carcinosarcoma as well.

In a patient with long untreated slow-growing osteoblastic bone metastases from an ER/PgR negative breast carcinoma, new metastatic sites in many lymphnodes pleura and massively in liver have been observed 4 weeks after receiving tamoxifen. Cause-effect relationship between administration of the drug and this unusually rapid spreading of the disease may be considered only as a hypothesis, as it is based wholly on clinical outcome. However, reporting of any exacerbation of breast cancer apparently induced by tamoxifen, even if only anecdotal, is advisable.

The ceruloplasmin concentration was determined in 145 cancer patients prior to and after treatment with different radiotherapeutic and chemotherapeutic regimes. The ceruloplasmin concentration was observed to be higher in patients with malignancies than in healthy controls. There was a positive correlation of the values with the clinical condition of the patients. The ceruloplasmin concentration was noted to stop increasing and subsequently fall in patients who responded to therapy, and, in contrast, to remain high or become higher in those who did not respond to therapy. The diagnostic and prognostic value of ceruloplasmin determination is discussed.
