A ten year retrospective survey of 346 cases of oesophageal foreign bodies is reported. Although coins were the most frequent foreign bodies in the pediatry group, meat was the most common offender in the material taken as a whole. Rigid oesophagoscopy in general anaesthesia was performed on 312 patients. A foreign body was removed in 281 cases (90%). Only one case of oesophageal cancer was discovered. The risk of iatrogenic perforation of the oesophagus was greatest in old patients who had a lump of meat stuck in the distal third of the oesophagus.

Recent European Organization for Research and Treatment of Cancer studies show that there is less concern about the importance of platinum as an ingredient of combination regimens or as an adjunct to radiotherapy. The presence of cisplatin is related to better response and survival in patients with locoregional and distant metastatic disease.

Cancer chemotherapy is associated with a wide range of vascular toxicities, which may be related to endothelial cell damage by these agents. The authors describe a patient with Hodgkin's disease who developed an atypical aortic dissection while receiving MOPP/ABV chemotherapy (nitrogen mustard, vincristine, procarbazine, prednisone, doxorubicin, bleomycin, and vinblastine). Because small cell lung cancer (SCLC) is a systemic malignancy, chemotherapy is the mainstay treatment for patients with all stages of this tumor. Many chemotherapeutic agents have activity against SCLC, and when combined in three- or four-drug regimens, they produce a response in 75% to 90% of patients. Median survival times for patients with limited disease range from 14 to 16 months, and a small proportion may be cured. For extensive stage patients, median survival is usually less than 1 year; 2 year disease-free survival is uncommon. Current research is focusing on ways to increase the dose intensity of chemotherapy delivery.

This review summarises the current state of knowledge regarding the risk factors for pancreatic cancer. Of all the factors studied, only smoking is well established. Excessive dietary fat and protein may increase the risk, but this has not been proven conclusively. Fruits and vegetables, on the other hand, may offer a protective effect. The role of coffee remains controversial. There is no concrete evidence that alcohol, tea, occupational exposure, diabetes, gallstones, cholecystectomy or gastrectomy are significantly associated with this tumour. A possible predisposing role for chronic pancreatitis cannot be excluded.

Two cases of invasive lower ureteric cancer developed following frequent recurrence of superficial bladder tumors in the region of the ureteral orifice. The cancer focus could not be identified in the intramural ureter prior to intravenous urography which revealed hydronephrosis. When bladder tumors repeatedly develop near the ureteric orifice, careful investigations such as ureteral catheterization with a small brush for cytology or ureteroscopy are necessary for the early detection of invasive disease.

The functional results of primary reconstruction of the floor of the mouth after ablative surgery for oral cancer using a platysma myocutaneous island flap are reviewed. Seventeen patients who had simultaneous neck dissection, tumor resection, and reconstruction were evaluated. It appears from this study that primary reconstruction of the floor of the mouth with a platysma flap is not ideal. This is believed to be due partly to the high occurrence of direct, postoperative complications.

New paradigms are few and far between. The discovery that peptic ulcer disease is the result of an infection by a previously unrecognized microorganism rather than the consequence of excess secretion of gastric acid aptly qualifies as a paradigm shift. Eradication of Helicobacter pylori from the stomach by antibacterial agents heals ulcers at least as effectively as H2-blockers. Further, eradication of the organism results in long-term cures of peptic ulcers. In contrast, the relapse rate in patients successfully treated with H2-blockers exceeds 50% in the first year after healing. Also of interest is the role of H. pylori in gastric cancer.

In a 61-year-old female patient, the recurrence of peritoneal dissemination after total gastrectomy due to gastric cancer responded well to chemotherapy of sequential methotrexate and 5-FU. A total of 10 courses of this chemotherapy diminished ascites, normalized the value of CA 19-9, and re-opened the left obstructed ureter. During this therapy, the patient's condition was good, with no experience of nausea or leukopenia.

Subungual metastatic cancer from any primary site is rare. Only seven cases from primary renal cell carcinoma have previously been reported. We now describe a further example of this condition and review its clinical features, radiographic findings and pathological changes. The diagnosis of subungual metastasis should be considered in any patient with metastatic cancer who develops a new lesion of the distal phalanx.
