TITLE: Colectomy and cancer in Melbourne children with ulcerative colitis.

A search of hospital records dating from 1945 to 1977 inclusive identified 67 children with a definite diagnosis of ulcerative colitis, and 26 in whom the diagnosis was suspected, but not proven. Actuarial analysis of follow-up data available in December, 1978, indicated 75% patient survival after 18 years of disease, and 50% colon survival after 12 years of disease. Deaths were due to cancer (2 patients), complications of surgery (four patients) and incidental respiratory tract infection (one patient). Cancer risk was found to be lower than expected, which does not support the argument for prophylactic colectomy in childhood-onset disease. Restricting colectomy to medical indications during childhood will minimize the complications of colectomy without exposing the patient to risks greater than those of surgery itself.

TITLE: Radiotherapy for carcinoma of the nasal cavity.

Twenty-five patients with primary epithelial carcinoma of the nasal cavity without nodal or distant metastases were treated by irradiation between 1967 and 1978. Small field, beam-directed techniques delivered 6,000 to 7,000 rads with conventional fractionation. Control of the primary tumor was achieved in 21 (84%) patients after irradiation. All five treatment failures (one infield only, three infield recurrence with lymph node metastases, and one regional cervical node metastasis) were evident within six months; all five patients died of cancer. The adjusted actuarial survival rate at three years was 76%. Failure in the untreated neck was only 5% when the primary carcinoma was controlled and 16% overall. The literature has been reviewed with attention to tumor control rates and survival.

TITLE: Plasma copper and zinc levels in patients with malignant tumors of digestive organs: clinical evaluation of the C1/Zn ratio.

The plasma copper level (PCL) and plasma zinc level (PZL) were determined in 126 patients: 54 had carcinomas of which 40 were carcinomas of the digestive organs, 37 had benign lesions, and 35 were without disease. The PCL increased and the PZL decreased significantly in the group with carcinoma of the digestive organs. The CuHZn ratio was not only significantly higher than in the normal group, but also was high in cases with advanced carcinoma especially when liver metastasis was present. Therefore, the Cu/Zn ratio may be valuable in estimating the extent of the carcinoma as well as in determining the prognosis of the cancer patients.

The results of simple closure were compared with those of partial resection in the treatment of acute gastric perforation in 13 patients. Perforation was due to ulcer in 10 cases and to cancer in 3 cases. Differential diagnosis between cancer and ulcer during emergency surgery is difficult to establish and could be made in 2 cases only postoperatively. In the 8 patients undergoing gastric resection there was one fatal outcome by pulmonary embolism. In 5 patients where the perforation was only sutured, one patient aged 97 died of pneumonia. 2 patients developed severe hemorrhage and needed an emergency gastric resection, that was survived by only one patient. - Our results and those of the literature suggest that in acute gastric perforation due to ulcer or carcinoma, emergency gastric resection gives far better results than simple closure by suture.

Better diagnosis of prostatic cancer by RIA of serum prostatic acid phosphatase? The results of serum determination of RIA prostatic acid phosphatase (PAP) and enzymatic serum phosphatase were compared in 267 patients with the clinical diagnosis to reveal the diagnostic value of the RIA-PAP in prostatic carcinomata. In 33 of the investigated patients (12.4%) we found elevated values of the PAP over the normal range of 0--5 ng/ml. In these 33 patients with pathologic PAP-values those with adenomata and in adenomata intracapsulated prostatic cancers had normal serum values measured enzymatically. There was no correlation between PAP-RIA results and the stage of the prostatic carcinoma. In nearly 20% we found better information by RIA-PAP than enzymatic laboratory tests, but combined with a digital investigation.

Many human cancer cells appear to differ from their normal counterparts in ways that are recognisable by the immune system of the host or patient. The specificities of these differences, however, and the nature, extent and significance of the most responses to them are often not very clear. Disturbances of the immune system generally contribute little to the occurrence, progression and clinical features of common malignant diseases. The present value of immunological monitoring of cancer patients is rather limited. Immunotherapy remains an experimental mode of treatment, the results of which are often disappointing. Current studies of the host-tumour relationship in experimental animals and man suggest, however, that a wholly pessimistic view is not justified.

During an investigation of the serotypes of Serratia marcescens present in a cancer centre, it was found that 8.7% of 241 strains had more than one H antigen. There were 19 diphasic and two triphasic strains. Antigen H1 was a component of the phases in the approximately half of the strains with multiple phases. This is the first report of such phases in the genus Serratia.

Levels of Clq binding activity have been measured in the sera of patients with benign and malignant breast disease. Cancer patients showed significantly higher binding activity than patients with gross fibrocystic disease. Clq binding levels were not related to levels of carcinoembryonic antigen, or rheumatoid factor. The Clq reactive material was identified as being precipitable with Protein A, and predominantly eluted in gel filtration fractions with a molecular weight of approximately 1 x 10(6). These studies indicate that Clq binding level may be of use in the differential diagnosis of benign and malignant breast disease.

The characteristics of bone pain in metastatic cancer of the prostate were studied in 23 patients. The pain may be continuous or intermittent, show diurnal variations, and be migratory. The effects of activity, rest, and alcohol vary in different individuals. Relief of bone pain by analgesic medications is unsatisfactory. Aspirin-containing compounds are often more effective than narcotics for pain relief. The effects of external beam irradiation for palliation of pain are unpredictable.

In a longitudinal study performed before and after surgical resection of a chemically-induced, transplanted rat tumour we found that a monocytosis- and tumour-induced anti-inflammatory effect directed against macrophages was promptly corrected by tumour excision but recurred with metastasis. These observations indicate that the monocyte abnormalities are acquired and related to cancer progression. The anti-inflammatory effect required relatively large primary or metastatic tumour burdens. Our findings support the clinical impression that monocyte abnormalities are associated with a poor prognosis.

The effect of single and five-fold administration of 12 chemotherapeutic drugs on human strains of larynx, lung and kidney cancers and Wilms' tumor transplanted in nude mice was studied. Wilms' tumor was found to be more responsive to dactinomycin and cyclophosphamide. Larynx, lung and kidney cancers were resistant to all the chemotherapeutic drugs studied. The effect of irradiation of larynx cancer which resulted in short-term tumor growth retardation was also studied.

Although initially used exclusively as a single agent, bleomycin has been considered recently for use in combination chemotherapy. Cancer mortality in Hamburg, calculated per 10,000 living inhabitants of each age group of men and women for the years 1873 to 1978 proved to be essentially constant after eliminating sources of statistical error. Calculation or organ involvement by the same method for 1933 to 1978 showed that the increase in mortality for one type of location is compensated by a decrease elsewhere, so that altogether the constancy remains. This behavior can be explained if it be assumed. This behavior can be explained if it be assumed that a certain constant percentage of the population is prone to cancer and - possibly due to the action of exogenous noxae - becomes ill when the necessary age is reached. The nature of a determining factor which is most probably present is not known. Comparison with international figure shows that the results of the Hamburg Study can be considered a model case.

The effect of a single systemic or intratracheal administration of viable Mycobacterium bovis strain Bacillus Calmette-Guérin on the development of primary lung cancer in Syrian golden hamsters was assessed. M. bovis strain Bacillus Calmette-Guérin administered i.v. prior to, during, or after carcinogen treatment did not alter the incidence of lung tumors. Animals receiving intratracheal M. bovis strain Bacillus Calmette-Guérin had an increased incidence of lung tumors. Both i.v. and intratracheal routes accelerated the appearance of tumors.
