A vast experience in the introduction of modern mathematical methods into research activities of the Central Research Roentgenoradiology Institute was accumulated in the Department of Biomathematics. The Information-retrieval System for Storage and Analysis of Data on the Activities of X-ray Service in the USSR, a package of applied programs for statistical analysis of radiation therapy effectiveness, a controlled data base on 6 cancer sites, 2 systems for simulation of radiation effects on heterogeneous cell systems were worked out. Some priority results in the fields of mathematical biology and statistical analysis of incomplete selective data were obtained. The Department is heading the work on the problem "Mathematics and computer technology in radiobiology and radiation biophysics".

TITLE: Breast cancer in the young.

Breast cancer in the young is rare. Some cases can be treated with modest surgical intervention and prognosis is good. We present a report of a 17-year-old black girl with adenocarcinoma of the breast. In our case, the disease pursued a devastatingly rapid course in spite of treatment and the patient died within 6 months of diagnosis. The literature relating to clinical experience with various types of breast carcinoma seen in pre- and postpubertal patients is also reviewed. We conclude that this case, and certain other cases cited, demonstrates the need for a high index of suspicion and earlier and more aggressive intervention in young patients with axillary breast masses.

TITLE: Isolated tumoral pyruvate dehydrogenase can synthesize acetoin which inhibits pyruvate oxidation as well as other aldehydes.

Oxidation of 1 mM pyruvate by Ehrlich and AS30-D tumor mitochondria is inhibited by acetoin, an unusual and important metabolite of pyruvate utilization by cancer cells, by acetaldehyde, methylglyoxal and excess pyruvate. The respiratory inhibition is reversed by other substrates added to pyruvate and also by 0.5 mM ATP. Kinetic properties of pyruvate dehydrogenase complex isolated from these tumor mitochondria have been studied. This complex appears to be able to synthesize acetoin from acetaldehyde plus pyruvate and is competitively inhibited by acetoin. The role of a new regulatory pattern for tumoral pyruvate dehydrogenase is presented.

Recently, preservation of physiological functions have become to be considered in the operative treatment for rectal cancer. Dysfunction caused following radical operation for rectal cancer is divided into two parts as follows, (1) disturbance or loss of anorectal function due to sacrifice of the anus in abdomino-perineal resection and decrease function as fecal reservoir following anal sphincter preserving resection of the rectum. (2) anorectal, urinary and sexual dysfunction following surgical injury to the autonomic nerves by pelvic lymph nodes dissection. Our conceptions of the indication for sphincter preserving operation and for wide dissection of pelvic lymph nodes which cause severe dysfunction of urinary and sexual functions postoperatively were discussed.

TITLE: Lymphocytic infiltration in juvenile thyroid carcinoma.

In this study, the significance of lymphocytic infiltration in juvenile thyroid carcinoma is clarified. We examined nine patients younger than 20 years of age. Histopathologically, there was good correlation between lymphocytic infiltration and the development and spread of carcinoma. It is believed that lymphocytic infiltration around the tumor is an immunologic reaction induced by antigens from the carcinoma itself, and also that the reaction may progress according to tumor development. Immunocytochemically, we determined what type of inflammatory cells infiltrated the thyroid and demonstrated HLA-DR expression in the cancer cells. These findings are similar to autoimmune thyroiditis in which antibody-dependent cellular cytotoxicity (ADCC) works as a main immunomechanism.

TITLE: [A case of metastatic brain tumor from prostatic cancer showing remarkable shrinkage following CDDP administration]

A patient with brain metastases from prostatic cancer received chemotherapy using CDDP alone. A dose of 80 mg of CDDP was administered twice intravenously, followed by intra-arterial injections of 70 mg and 50 mg for the primary lesion over the course of three months. The main brain tumor showed a 70% decrease in size when estimated by CT, along with improvement of clinical symptoms related to the brain metastases. Intravenous injection of CDDP may thus have potential therapeutic value against metastatic brain tumors.

An approach to the management of patients with large (greater than 4 cm) operable breast cancers is described. The conventional sequence of mastectomy followed by systemic therapy is reversed, allowing accurate measurements of response to individual forms of endocrine therapy or chemotherapy. Such a method not only permits individual selection of appropriate systemic therapy, but also allows clinical response to be related to histological and biochemical tumour parameters. A response was observed in eleven of twenty-three patients to endocrine treatment and in twelve of thirteen to combination chemotherapy. In five of the latter the response was histologically complete.

The radiograms of the skeleton of 41 patients with advanced breast cancer treated with bilateral ovariectomy were reviewed. The modifications in the secondary localizations and/or their appearance after castration were compared with the findings of the clinical examination. Appearance and progression of osteolytic lesions, corresponded to a clinical progression of the disease. Osteoblastic evolution of osteolytic lesions and the appearance of osteoblastic lesions in bones undamaged before ovariectomy were signs of a favorable response to therapy.

TITLE: Declining breast cancer mortality among young American women.

Changes in age-specific breast cancer mortality rates among white females in the United States during 1950-80 were shown to be correlated with changes in patterns of childbearing in early adulthood. However, for the most recent 5-year period among women below age 40 years, small declines in breast cancer mortality were observed in the late 1970's, despite a predicted increase following delays in childbearing that began in the 1960's and despite evidence of a rising incidence of the cancer. Although correlation analyses have inherent limitations, the findings raise the possibility that recent changes in the detection and management of breast cancer have contributed to a lowered mortality from this cancer among young American women.

We studied 89 cases of thyroid cancer registered at the Nagasaki Tumor Registry during the period from 1973--1977. The incidence of thyroid cancer tended to be higher in persons exposed to the atomic bomb, especially women, than in the nonexposed population. Compared with nonexposed individuals, the relative risk for thyroid cancer was increased in distally as well as proximally exposed persons, but not in early entrants to the bombed city. In 83 histologically examined patients, papillary carcinomas were highly predominant, they were more prevalent in the exposed than in the nonexposed population.

The relationship between histologic staging of cervical adenocarcinoma and pelvic lymph node metastasis was analyzed in 138 patients who had undergone hysterectomy. In AIS, the cervical glands were replaced by cancer. When glandular or solid cancer budding into the stroma was identified in AIS, it was designated Stage Ia adenocarcinoma. Pelvic lymph node metastases were found in cases with Stage Ib-III adenocarcinoma, but not in Stage Ia adenocarcinoma. This suggests that Stage Ia adenocarcinoma might be treated the same as AIS.

Twelve women with previously irradiated advanced pelvic cancer were treated with doxorubicin and cisplatin to determine a maximum tolerable dose. Response was not an objective of this phase I study, but one partial response was seen in four cases of cervix cancer with measurable lesions. Hematologic toxicity was frequent and severe at the starting doses (doxorubicin 30 mg/M2 and cisplatin 100 mg/M2 every 3 weeks), but was acceptable when cisplatin was used at 75 mg/M2. Further evaluation of this combination in squamous carcinoma of the cervix is not recommended.

In 88 operative specimens topography, size and histopathological findings of lung metastases were analyzed. The stages of formation of metastases as the phase of invasion, embolization and implantation were differentiated by postmortem angiographies investigating the neo-vascularization of the metastases by 20 to 60 micron vessels. Analyses of metastases by morphological quantification of tumor regression after cytostatic therapy and immune histochemical examinations of lung metastases in unknown primary cancer have gained clinical importance.
