TITLE: Successful treatment for lung cancer associated with pulmonary sequestration.
PUBDATE: 20051201
We encountered a 69-year-old man with lung adenocarcinoma and pulmonary sequestration. The cancer lesion was located in the left upper lobe, with sequestration of the left lower lobe. Left upper lobectomy was performed after induction chemoradiotherapy, but the sequestered lung lobe was preserved because the preoperative respiratory function was poor. Preservation of the sequestered lung during surgery for lung cancer should be considered in patients who have poor respiratory function and no signs of respiratory infection.

TITLE: Genetic pathways to melanoma tumorigenesis.
PUBDATE: 20040801
The incidence of cutaneous malignant melanomas is growing faster than that of any other cancer and therefore posing a major heath threat worldwide. In melanocytic skin tumours, the feasibility of correlating a specific pathological stage with a corresponding genetic alteration provides a remarkable opportunity to study the multistep tumorigenesis model. This multistep melanoma tumorigenesis is best described as a continuum of transformation of the melanocytes, melanocytic dysplasia, and melanoma formation. These steps involve genotypic alterations including loss of tumour suppressor genes, microsatellite instability, and alterations of the mismatch repair system. This review seeks to examine melanoma tumorigenesis based on these genetic changes.

PUBDATE: 20050601
Prostate cancer is the most common malignancy and the second leading cancer-related cause of death in men in the USA. Despite enormous efforts in understanding the molecular basis of prostate cancer, very little progress has been made in prevention and treatment of this often lethal cancer. Recent studies have demonstrated that hedgehog signaling is frequently activated in advanced or metastatic prostate cancers. With small molecule inhibitors available to analyze the hedgehog signaling pathway, a novel rationale for prostate cancer therapy can be devised.

This study was carried out to assess the significance of apoptosis in prostatic intraepithelial neoplasia (PIN) and prostate cancer. A total of 120 prostatic specimens were studied in the department of Pathology [corrected] JNMC, Aligarh. The rate of apoptosis in PIN and prostate cancer was examined by quantifying the number of apoptotic bodies per hundred cells (apoptotic index) on haematoxylin and eosin stained histological sections [corrected] A significant correlation was noted between increasing apoptotic indices and increasing Gleason grades within a cancer.

Venous thromboembolism (TE) is a frequent occurrence in lung cancer patients and can be precipitated by venous stasis, the activation of clotting cascades by pro-coagulant tumour factors and by vascular injury due to chemotherapy and central venous catheters. Subcutaneous low molecular weight heparin (LMWH) has replaced intravenous unfractionated heparin in the treatment of acute TE. The use of long-term LMWH is recommended to prevent the recurrence of TE. Prophylactic treatment is indicated in patients undergoing surgery or hospitalised. Heparin appears to have distinct anti-neoplastic as well as anticoagulant effects, which benefit overall survival.

Endocrine therapy in the form of ovarian ablation was developed over a century ago. It remains nonetheless one of the most effective and most clearly targeted form of systemic therapy for breast cancer. Endocrine or hormonal therapy has an effect on virtually only those women whose tumors are positive for estrogen receptors (ER) and/or progesterone receptors (PgR). The presence of these steroid hormone receptors remains the most useful predictive factor in selecting therapy for breast cancer.

The principles and methodologies used by the Cancer Services Collaborative (CSC) are particularly relevant for radiology departments. A radiology project looking at the provision of barium enema examinations is used to highlight how the principles can be applied to a radiology department. Advice on how to access available CSC literature is offered. The CSC principles and methodologies are an important part of the NHS modernization agenda, and offer an exciting vehicle to improve patient care. It is important that radiologists understand the opportunities offered and the challenges posed by the modernization agenda.

By April 2000, CpG-7909 had entered phase I/II trials for cancer and in March 2002, Coley initiated a phase I trial in non-Hodgkin's lymphoma in combination with rituximab (Rituxan). By October 2002, CpG-7909 was in phase II trials as a vaccine adjuvant. Cpg-7909 is currently also undergoing phase II trials for melanoma.

PUBDATE: 20020401
The frequency of secondary tumours of the kidney is underestimated, especially as their diagnosis is often delayed, after nephrectomy or autopsy. The authors present the case of a 59-year-old patient presenting with renal metastases from lung cancer operated two years previously and review about forty cases published in the literature over the last twelve years. The diagnosis by percutaneous needle biopsy can avoid useless surgery and improve patient management. The specificity and sensitivity of this method need to be evaluated.

PUBDATE: 20040901
Pancreatic cancer is one of the commonest causes of death from cancer. Despite therapy with surgery, conventional chemotherapy, and radiation, 5-year survival for patients with this diagnosis remains poor. However, advances in the molecular understanding of this malignant disease over the past 5 years might lead to new treatment strategies. Strategies of gene therapy, antiangiogenic treatments, immunotherapy, and signal-transduction inhibition are in preclinical development. This review presents an overview of molecular therapy in pancreatic cancer.

PUBDATE: 20051001
Neoadjuvant chemotherapy in transitional-cell carcinoma of the bladder (TCC) improves survival. This is one of the most important developments in the management of muscle-invasive bladder cancer in recent times. There is an improved absolute 5-year survival of at least 5% for T2-T4 disease. To achieve this benefit, a cisplatin-containing combination is required. There is no difference in survival whether radical radiotherapy or radical cystectomy is given as subsequent definitive treatment.

PUBDATE: 20040601
Mutations in the breast cancer susceptibility protein BRCA2 cause inherited susceptibility to breast, ovarian and other cancers. There is now compelling experimental evidence that a major biological function of BRCA2 is the maintenance of chromosome structure stability in dividing cells by regulation of steps in recombination between homologous DNA strands. Recent experimental findings shape the current models for BRCA2 function, and structural and biochemical advances shed new light on the interactions between BRCA2, the RAD51 recombinase and single-stranded DNA during DNA recombination.

TITLE: Oral mucositis.
PUBDATE: 20050101
Oral mucositis refers to erythematous, erosive, and ulcerative lesions of the oral mucosa seen in two patient populations: (1) head and neck cancer patients undergoing radiation therapy to fields involving the oral cavity, and (2) patients receiving high-dose chemotherapy for cancer. Oral mucositis is a significant and dose-limiting toxicity of cancer therapy, with important clinical and economic implications. This article reviews the current knowledge on the pathogenesis, clinical presentation, diagnosis, and management of oral mucositis.
