TITLE: A review of the therapy of paraneoplastic neurologic syndromes.
PUBDATE: 19990101
The therapy of paraneoplastic neurologic syndromes remains unclear and warrants a systematic review of the literature. Reports in English and foreign language literature were abstracted. Data were sorted by the primary paraneoplastic neurologic syndrome, the primary malignancy, and the methods of treatment. Neurologic improvement follows surgical, chemotherapeutic, and radiation treatments. Adjuvant immunosuppressive therapy with steroids, plasmapheresis, or immunoglobulin may help stabilize the progression of neurologic symptoms. Syndromes characterized by inflammation or neurotransmitter production without neuronal loss are most responsive to therapy. While treatment of the underlying cancer with appropriate surgery, chemotherapy, and radiation is recommended, the paraneoplastic syndrome should probably be managed with immunosuppressive therapy.

Several clinical trials have now been carried out with histamine type 2 receptor antagonists in cancer patients often as an adjunct to surgical resection of the primary tumour While promising results have been obtained in some groups of patients with gastrointestinal cancer, with increased survival and evidence of increased immunological recognition of tumour, results are less encouraging for breast cancer. This may be due to differences in the levels of histamine or the role of histamine in growth of these different tumour types.

Polycystic ovary syndrome (PCOS) is the most common endocrine condition of premenopausal women and has significant metabolic abnormalities that could have an impact after the menopause. Diabetes mellitus, hyperlipidemia and hyperinsulinemia could potentially affect health in this era of life. Endometrial cancer, due to unopposed estrogen action, is more common where progestins have not been given for menstrual dysfunction. Preventive management earlier in life will avoid postmenopausal problems in PCOS.

PUBDATE: 19980601
Many methods of securing skin grafts to their recipient beds have been described. A review of the senior author's experience with facial skin cancers over a two-year period revealed that reconstruction with full-thickness skin grafts was employed in 60 of more than 500 cases. This series of 60 cases was reviewed. Uses of foam as a bolster has proven to be a simple, time-conserving and reliable technique that produces consistently excellent results with a low complication rate. The technique is reviewed, along with its advantages and disadvantages.

The introduction of colloidally stable liposomes with low drug leakage rates resulted in a renaissance in liposome applications in cancer therapy. Furthermore, a platform of sterically stabilized liposomes also allows the construction of new generations of drug delivery vehicles. These include targeted liposomes and targeted nucleic acid delivery vehicles, based either on cationic sterically stabilized liposomes or pre-condensed DNA encapsulated in neutral or negatively charged liposomes.

Recognition of congenital anomalies that predispose to childhood cancer allows for the institution of a cancer surveillance program, identification of relatives with increased cancer risk, and recurrence risk counseling. In this article, a systems approach to the diagnosis of 21 childhood cancer syndromes is set forth in the format of the pediatric physical examination. In the second part of this article, guidelines are presented for genetic testing, cancer screening, and genetic counseling for the 21 cancer syndromes.

Pain experienced during mammography can deter women from attending for breast cancer screening. Review of the current literature on pain experienced during mammography reveals three main areas of interest: reports of the frequency of pain, identification of predictors of pain and strategies for responding to pain. Implications of this literature for breast screening programmes include the need for appropriate measurements of pain during mammography that are valid for screening populations, a further understanding of organizational factors involved in screening programmes that may be predictors of pain and for the development of valid strategies for responding to pain within breast screening programmes.

PUBDATE: 20000501
The goal of radioactive seed implantation in prostate cancer is to treat the tumor to the necessary dose while minimizing the dose to adjacent normal structures. The uniform and peripheral loading schemes used in the past have serious limitations. However, with the availability of three-dimensional treatment-planning systems, it has become possible to do custom volume loading based on volumetric ultrasound imaging of the gland. As a result, the dose distributions are more uniform, and the doses to the urethra and rectum are maintained in a safe range.

TITLE: Determination of nicotine, pH, and moisture content of six U.S. commercial moist snuff products--Florida, January-February 1999.
PUBDATE: 19990501
The use of smokeless tobacco (moist snuff and chewing tobacco) can cause oral cancer and precancerous oral lesions (leukoplakia) and is a risk factor for cardiovascular diseases and nicotine addiction. Despite these adverse effects, smokeless tobacco is used commonly in the United States by young people, especially male high school students. Officials in Florida requested CDC assistance in analyzing six moist snuff products to measure three factors that affect their nicotine dose: pH, nicotine content, and moisture content. This report summarizes the results of the analysis, which indicate that the pH, amount of nicotine, and moisture vary widely among brands.

The objective of the work was to assess the impact of the shift of gastric cancer in an oral direction in the mediastinal lymph nodes. For this purpose a new miniinvasive lower mediastinoscopy was used in 30 patients. The method is safe and can be used also in other situations. Mediastinal metastases may occur in cancer in the distal and medium third of the stomach. They need not be present when cancer is in the upper third. The shift of gastric carcinoma did not influence the mediastinal nodes.

An estimated 97 million adults in the United States are overweight or obese, a condition that substantially raises their risk of morbidity from hypertension, dyslipidemia, type 2 diabetes, coronary heart disease, stroke, gallbladder disease, osteoarthritis, sleep apnea and respiratory problems, and endometrial, breast, prostate, and colon cancers. Higher body weights are also associated with increases in all-cause mortality. Obese individuals may also suffer from social stigmatization and discrimination. As a major contributor to preventive death in the United States today, overweight and obesity pose a major public health challenge.

PUBDATE: 20010401
Multicolor karyotyping technologies, such as spectral karyotyping (SKY) (Schröck et al.1996; Liyanage et al. 1996) and multiplex (M-) FISH (Speicher et al. 1996), have proved to be extremely useful in prenatal, postnatal, and cancer cytogenetics. However, these technologies have inherent limitations that, in certain situations, may result in chromosomal misclassification. In this report, we present nine cases, which fall into five categories, in which multicolor karyotyping has produced erroneous interpretations. Most errors appear to have a similar mechanistic basis.

PUBDATE: 20000401
Methadone is a synthetic opioid agonist which is mostly used for drug maintenance therapy in opioid addicts. It has also proven to be a powerful analgesic four times more powerful than oral morphine. The main indication is cancer pain as an alternative to oral morphine in case of tolerance or side effects due to accumulation of active metabolites. Methadone however shows wide inter-individual variability in half-life necessitating a carefully scheduled administration scheme. Side effects are similar for morphine and methadone. Methadone might be a safe and effective analgesic in patients with renal disease because its metabolites are almost exclusively excreted in the feces.

Tobacco and alcohol use are strong risk factors for cancer of the upper gastrointestinal tract. Saturated fat and red meat intake also appear to increase the risk of colorectal cancer. The strongest and most consistent dietary protective factors for gastrointestinal cancer are vegetables and fruits. Some micronutrients show beneficial effects, but the degree of protection is less than that observed from whole plant foods. Substantial decreases in the morbidity and mortality from gastrointestinal cancer could occur with more widespread adoption of dietary guidelines designed for cancer prevention.
