TITLE: Warfarin-induced skin necrosis. A cutaneous sign of malignancy?

Patchy necrosis of the skin is a rare and unpredictable complication of oral anticoagulant therapy. Of the four patients that we have seen with this disorder, three had metastatic adenocarcinoma; in two, this was an unexpected finding. The association of a malignant neoplasm with warfarin-induced skin necrosis has not been emphasized previously. Whether such necrosis represents a clue to the presence of cancer or occurs only coincidentally in patients requiring anticoagulant therapy because of adenocarcinoma-associated thrombophlebitis must await further experience. A congenital or acquired deficiency of protein C may be the primary initiating factor.

We studied cases of stage I endometrial adenocarcinoma encountered at our institution during the period 1970-80. Five out of 164 of these tumors showed a diffusely infiltrating pattern of growth. Two of the five patients died of cancer within 5 years, with a 5-year mortality of 40%. A control group of 15 patients seen over the same period showed a 5-year mortality of 13.3%. Thus, the diffusely infiltrating pattern of growth was indicative of poor prognosis in endometrial carcinoma in stage I disease.

TITLE: Vinblastine plus cisplatin in advanced non-small cell lung cancer: lack of advantage for vinblastine infusion schedule.

We have evaluated 31 patients with advanced non-small cell lung cancer treated by short-term 5-day vinblastine infusion combined with bolus cisplatin. Nine of 31 patients (29%) had partial and complete responses. Although five of nine (55%) of the responders were alive at greater than or equal to 1 year, three of the 31 patients experienced drug-related mortality. Our experience, as well as a review of previously reported trials in the literature, suggests that the infusion schedule of vinblastine offers no advantage over the bolus schedule.

The effectiveness of bestatin combined with mitomycin C and tegafur as an adjuvant to surgery for gastric cancer was investigated in a prospective randomized controlled study. All patients underwent curative gastrectomy during the period from November 1980 to April 1984. Five-year survival rate revealed no significant difference between groups with or without bestatin. The effectiveness of bestatin was suggested in postoperative peritoneal recurrence in cases with positive histological invasion into the veins of the stomach wall.

TITLE: Familial breast cancer in males. A case report and review of the literature.

The occurrence of breast cancer in two related males, an uncle and nephew is reported. A review of previously reported cases of male breast cancer occurring in families and the association with other cancers in other family members is included. In previous reports on breast cancer in males, familial or hereditary factors have not generally been recognized as a major contributing factor. Of ten families reported (including our own) where there is sufficient information given, six families (60%) had females with breast cancer. It appears there are some families in which males as well as females have an increased risk of developing breast cancer.

TITLE: Brain metastases from small cell lung cancer treated with combination chemotherapy.

Ten patients with metastatic small cell lung cancer (SCLC) in the brain, shown by CT scan at time of diagnosis, were treated with systemic combination chemotherapy. Repeated CT scans were performed every 4 weeks. Seven patients were evaluable, since three patients died during the first 2-3 weeks. Significant responses on CT scans as well as neurologic improvement were demonstrated in all evaluable patients. Four of seven patients obtained a complete response in the brain, while three had partial remissions. The impact of this observation on the overall treatment strategies in SCLC has further to be defined.

A 55-year-old man who had liver metastasis after undergoing surgery for renal cancer was hospitalized immediately on May 4, 1987 with complaints of general malaise, epigastric pain, nausea and vomiting. Because of abnormally high levels of blood calcium 15.6 mg/dl and serum amylase 2,069 IU/l, the case was diagnosed as hypercalcemic crisis and acute pancreatitis. Following recovery from the critical stage with administration of elcatonin and FOY, therapy for cancer initiated. We report the clinical course of this patient and discuss about hypercalcemia and acute pancreatitis as cases of oncologic emergency.

No single dietary factor or ingested substance has been implicated in the aetiology of oesophageal carcinoma especially in high-risk areas of the world. Although not well defined, there is heavy consumption of tannin-rich foods, particularly sorghum, by populations at high risk for carcinoma. The carcinogenic properties of tannins and extracts of tannin-rich plants have been demonstrated experimentally. It is proposed herein that salivary proline-rich proteins (PRPs) by binding ingested tannins protect the oesophagus from the carcinogenic effects of the latter. It is also possible that genetic variants of PRPs may influence the incidence of oesophageal cancer in different populations.

The activity of a novel macrolide, RU 28965, was compared to that of erythromycin against Gram-positive organisms isolated mainly from cancer patients. RU 28965 was active against streptococci including Streptococcus pneumoniae and streptococcal groups A, B and G. It was also active against methicillin-susceptible staphylococci, Bacillus cereus and Listeria monocytogenes. Its activity against these organisms was generally equal to that of erythromycin. RU 28965 was moderately active against enterococci and inactive against JK diphtheroids and three different species of coagulase-negative staphylococci. Organisms resistant to erythromycin were also resistant to RU 28965.

Twenty-nine patients with inoperable non-small cell lung cancer were treated as out-patients with vindesine and mitomycin C. Eight patients had a complete response, and nine a partial response. Response was generally associated with either stable or improving ECOG and symptom scores. The incidence of serious side-effects was low--only two patients' white cell count fell below 3000 cells/mm2, only one patient's platelet count fell below 100,000 cells/mm2, and two patients developed vomiting associated with treatment. The combination of vindesine and mitomycin C appears to be effective in the treatment of inoperable non-small cell lung cancer. Side effects were generally well tolerated and allowed out-patient treatment.

Scattered reports have suggested the association of cancer with bullous disease. We report herein, two surgical cases of lung cancer arising from bullae. Both the patients were males who smoked heavily and the generation of cancer was found simultaneously with the discovery of the bulla. At thoracotomy, it was found that the cancer had arisen from the firm, scarred and contracted area adjacent to the bulla wall. On the basis of this experience and review of the literature, it is suggested that physicians should always pay careful attention to the generation and complication of cancer while treating bullous disease in heavy smoking individuals.

A prospective 3-year survey of nonmelanoma skin cancer in Japanese-American residents of the Hawaiian island of Kauai was conducted beginning in January 1983. The crude rate for basal cell and squamous cell carcinoma was 123/100,000, 88 times higher than the corresponding figure from Japan. This discrepancy may be attributed to an increase in ultraviolet exposure due to lifestyle and latitude, but arsenic exposure may also have played a role. Nonmelanoma skin cancer in Japanese in Hawaii appears to be a significant problem that has not been appreciated because these malignancies are not reportable to tumor registries.

In cultured fibroblasts initiated from fresh tumor biopsies of 11 patients with thyroid tumors, 5 medullary carcinoma of the thyroid (MCT) and 6 papillary follicular, the chromosomes are relatively unstable when compared with those from control fibroblast cultures. Tumor-derived fibroblast samples showed a mean number of cells in metaphase with chromosome abnormalities at a frequency of 9.1% and a range of 6-13%, whereas 5 controls exhibited a mean frequency of 4% and a range of 3-5%. Our results indicate that genetic instability in thyroid cancer patients is present not only in peripheral blood cultures, as reported earlier, but in their fibroblast cultures as well.

Cortisol concentration rose steadily after interferon administration and was significantly different from that on control day at 8 h following intramuscular injection of interferon-alpha 2. Cortisol elevation was increased as weekly doses of interferon were increased (0-120 X 10(6) units). Recent clinical trials of interferons for treatment of neurological and malignant diseases provide a compelling need to understand the actions and side effects of exogenously administered interferons.

Antiretina antibodies have been found in the serum samples of four patients with cancer experiencing concomitant loss of vision (cancer-associated retinopathy [CAR] syndrome). These immunoglobulins bound an antigen from normal pooled retina having a molecular weight of 23,000 daltons, which we describe as the CAR antigen. No corresponding antibodies could be found in serum samples obtained from patients with cancer not experiencing vision loss, from patients having retinitis pigmentosa, or from normally sighted individuals who did not have cancer. The early detection of rising antibody titers against the CAR antigen could prove important in the identification of patients likely to suffer from CARs.
