TITLE: Successful treatment for lung cancer associated with pulmonary sequestration.
PUBDATE: 20051201
We encountered a 69-year-old man with lung adenocarcinoma and pulmonary sequestration. The cancer lesion was located in the left upper lobe, with sequestration of the left lower lobe. Left upper lobectomy was performed after induction chemoradiotherapy, but the sequestered lung lobe was preserved because the preoperative respiratory function was poor. Preservation of the sequestered lung during surgery for lung cancer should be considered in patients who have poor respiratory function and no signs of respiratory infection.

TITLE: Genetic pathways to melanoma tumorigenesis.
PUBDATE: 20040801
The incidence of cutaneous malignant melanomas is growing faster than that of any other cancer and therefore posing a major heath threat worldwide. In melanocytic skin tumours, the feasibility of correlating a specific pathological stage with a corresponding genetic alteration provides a remarkable opportunity to study the multistep tumorigenesis model. This multistep melanoma tumorigenesis is best described as a continuum of transformation of the melanocytes, melanocytic dysplasia, and melanoma formation. These steps involve genotypic alterations including loss of tumour suppressor genes, microsatellite instability, and alterations of the mismatch repair system. This review seeks to examine melanoma tumorigenesis based on these genetic changes.

PUBDATE: 19990101
The therapy of paraneoplastic neurologic syndromes remains unclear and warrants a systematic review of the literature. Reports in English and foreign language literature were abstracted. Data were sorted by the primary paraneoplastic neurologic syndrome, the primary malignancy, and the methods of treatment. Neurologic improvement follows surgical, chemotherapeutic, and radiation treatments. Adjuvant immunosuppressive therapy with steroids, plasmapheresis, or immunoglobulin may help stabilize the progression of neurologic symptoms. Syndromes characterized by inflammation or neurotransmitter production without neuronal loss are most responsive to therapy. While treatment of the underlying cancer with appropriate surgery, chemotherapy, and radiation is recommended, the paraneoplastic syndrome should probably be managed with immunosuppressive therapy.

PUBDATE: 20050601
Prostate cancer is the most common malignancy and the second leading cancer-related cause of death in men in the USA. Despite enormous efforts in understanding the molecular basis of prostate cancer, very little progress has been made in prevention and treatment of this often lethal cancer. Recent studies have demonstrated that hedgehog signaling is frequently activated in advanced or metastatic prostate cancers. With small molecule inhibitors available to analyze the hedgehog signaling pathway, a novel rationale for prostate cancer therapy can be devised.

This study was carried out to assess the significance of apoptosis in prostatic intraepithelial neoplasia (PIN) and prostate cancer. A total of 120 prostatic specimens were studied in the department of Pathology [corrected] JNMC, Aligarh. The rate of apoptosis in PIN and prostate cancer was examined by quantifying the number of apoptotic bodies per hundred cells (apoptotic index) on haematoxylin and eosin stained histological sections [corrected] A significant correlation was noted between increasing apoptotic indices and increasing Gleason grades within a cancer.

Several clinical trials have now been carried out with histamine type 2 receptor antagonists in cancer patients often as an adjunct to surgical resection of the primary tumour While promising results have been obtained in some groups of patients with gastrointestinal cancer, with increased survival and evidence of increased immunological recognition of tumour, results are less encouraging for breast cancer. This may be due to differences in the levels of histamine or the role of histamine in growth of these different tumour types.

Venous thromboembolism (TE) is a frequent occurrence in lung cancer patients and can be precipitated by venous stasis, the activation of clotting cascades by pro-coagulant tumour factors and by vascular injury due to chemotherapy and central venous catheters. Subcutaneous low molecular weight heparin (LMWH) has replaced intravenous unfractionated heparin in the treatment of acute TE. The use of long-term LMWH is recommended to prevent the recurrence of TE. Prophylactic treatment is indicated in patients undergoing surgery or hospitalised. Heparin appears to have distinct anti-neoplastic as well as anticoagulant effects, which benefit overall survival.

Endocrine therapy in the form of ovarian ablation was developed over a century ago. It remains nonetheless one of the most effective and most clearly targeted form of systemic therapy for breast cancer. Endocrine or hormonal therapy has an effect on virtually only those women whose tumors are positive for estrogen receptors (ER) and/or progesterone receptors (PgR). The presence of these steroid hormone receptors remains the most useful predictive factor in selecting therapy for breast cancer.

The principles and methodologies used by the Cancer Services Collaborative (CSC) are particularly relevant for radiology departments. A radiology project looking at the provision of barium enema examinations is used to highlight how the principles can be applied to a radiology department. Advice on how to access available CSC literature is offered. The CSC principles and methodologies are an important part of the NHS modernization agenda, and offer an exciting vehicle to improve patient care. It is important that radiologists understand the opportunities offered and the challenges posed by the modernization agenda.

Polycystic ovary syndrome (PCOS) is the most common endocrine condition of premenopausal women and has significant metabolic abnormalities that could have an impact after the menopause. Diabetes mellitus, hyperlipidemia and hyperinsulinemia could potentially affect health in this era of life. Endometrial cancer, due to unopposed estrogen action, is more common where progestins have not been given for menstrual dysfunction. Preventive management earlier in life will avoid postmenopausal problems in PCOS.

The frequency of secondary tumours of the kidney is underestimated, especially as their diagnosis is often delayed, after nephrectomy or autopsy. The authors present the case of a 59-year-old patient presenting with renal metastases from lung cancer operated two years previously and review about forty cases published in the literature over the last twelve years. The diagnosis by percutaneous needle biopsy can avoid useless surgery and improve patient management. The specificity and sensitivity of this method need to be evaluated.

Many methods of securing skin grafts to their recipient beds have been described. A review of the senior author's experience with facial skin cancers over a two-year period revealed that reconstruction with full-thickness skin grafts was employed in 60 of more than 500 cases. This series of 60 cases was reviewed. Uses of foam as a bolster has proven to be a simple, time-conserving and reliable technique that produces consistently excellent results with a low complication rate. The technique is reviewed, along with its advantages and disadvantages.

TITLE: Sterically stabilized liposomes in cancer therapy and gene delivery.
PUBDATE: 19990401
The introduction of colloidally stable liposomes with low drug leakage rates resulted in a renaissance in liposome applications in cancer therapy. Furthermore, a platform of sterically stabilized liposomes also allows the construction of new generations of drug delivery vehicles. These include targeted liposomes and targeted nucleic acid delivery vehicles, based either on cationic sterically stabilized liposomes or pre-condensed DNA encapsulated in neutral or negatively charged liposomes.
