Ten young patients with cervical cancer were treated by extraperitoneal ovarian transposition. All patients received pelvic irradiation post operatively. Sex hormone were analysed after treatment. 10 cervical cancer patients who were irradiated only were analysed as control. Our data suggested that extraperitoneal ovarian transposition has definite significance for reserving ovarian function in young patients with cervical cancer and improve the quality of operation. This operation is chiefly suited for young patients with early cervical cancer who were operated extra-peritoneally, also for young patients who are irradiated alone or before operation.

We have tested a new type of immunoliposomes which may effectively mediate the targeting of enzymes to be used for site-specific prodrug activation (immuno-enzymosomes). The enzyme beta-glucuronidase, capable of activating the prodrug epirubicin-glucuronide (epi-glu), was coupled to the external surface of immunoliposomes directed towards ovarian cancer cells. A significant increase in cytotoxicity of the prodrug epi-glu was shown when the in vitro cultured cancer cells were pretreated with these immuno-enzymosomes.

TITLE: Rapid development of umbilical metastases after laparoscopic cholecystectomy for unsuspected gallbladder carcinoma.

Unsuspected microscopic adenocarcinoma of the gallbladder was identified after operation in a 66-year-old woman undergoing elective laparoscopic cholecystectomy for symptomatic cholelithiasis. An abdominal wall metastasis developed at the periumbilical incision site through which the laparoscope was introduced and through which the gallbladder was removed. A review of the available information on tumor seeding of laparoscopic tracts and drain tracts is presented, as well as implications for the further management of gallbladder cancer and other intraabdominal malignancies.

The activities of the research group on cancer chemoprevention supported by the Ministry of Health and Welfare of Japan are summarized. Since the research on cancer chemoprevention is quite new in Japan, this paper describes the present status of the development of possible cancer preventive agents. Acyclic retinoid (E-5166) and sarcophytol A encourage further studies. New inhibitory compounds of experimental carcinogenesis, such as cryptoporic acid, oleanolic acid, mokko lactone and ursolic acid were isolated from plants. The importance of the keen collaboration between natural product chemists and clinicians for further meaningful progress in this research field has been emphasized.

The most important mitogen for human breast cancer is oestrogen. Since oestrogens are synthesized via a protracted series of enzymic conversions from cholesterol, there are many potential targets for inhibition which could theoretically lead to suppression of oestrogen synthesis. However, inhibition of many of these targets is complicated by a resultant interference in the synthesis of other steroids, particularly glucocorticoids. This results in inhibitors of aromatase being the most rational choice for oestrogen suppression in breast cancer patients. Several aromatase inhibitors are in clinical usage. It is important that the clinical effectiveness of these is compared with that of the antioestrogen, tamoxifen.

Although HIV-infected patients are commonly infected by organisms that require an intact T cell immune system for control or eradication, there are some exceptions. The intracellular pathogen Listeria monocytogenes is one such organism. Listeriosis occurs primarily in neonates, elderly patients, patients on immune suppressive medications, cancer patients, and during pregnancy. However, listeriosis is an uncommon opportunistic infection in HIV-infected patients. We report a case of listeriosis with cutaneous lesions in a neonate born to an HIV-infected woman.

If there are atypical cells on a cytological smear and if the squamo-columnar junction can not be visualized during a colposcopic examination, then in order to exclude an early invasive cancer in the nonvisualized area within the endocervix it is mandatory to perform a diagnostic cone biopsy. A woman with abnormal cervical cytology and an unsatisfactory colposcopic examination who refused a cone biopsy was offered Lamicel in an attempt to expose the entire transformation zone. After 4 hr the sponge was removed and colposcopy repeated. The lesion and the squamo-columnar junction was then fully visible and she was spared a cone biopsy.

Sleep disturbances are common in cancer patients, but there are few specific data on their prevalence. Among other things, sleep problems may be a symptom of the cancer itself, part of a stress reaction to having cancer, a sequela to some other cancer symptom such as pain, or a side effect of cancer treatment. Insomnia is the more common sleep problem, although hypersomnia also occurs. Most insomnias are related either to pain or to psychophysiologic factors. Treatment should start with identification of a specific cause of sleeplessness; after that, behavioral interventions, medication, or psychotherapy may be helpful. When using medications, keep in mind possible complications, such as daytime sedation, tolerance, and rebound insomnia.

Treatment of superior vena cava syndrome (SVCS) caused by advanced lung cancer is still controversial. We inserted Gianturco expandable metallic stents (GEMS) in 5 patients with SVCS due to the extension of lung cancer. GEMSs were introduced intravenously through the catheter after intraluminal balloon dilation of the stenotic sites. SVCS was successfully and easily relieved by this method without any significant complication. GEMS placement seems to be a useful alternative to bypass grafting procedure for the treatment of SVCS.

In 53 patients suffering from testicular cancer it was reviewed whether the changes of spermatogenesis due to radiation or chemotherapy are reflected by the sexual hormones (FSH, LH, T). After both therapeutic measures a reduction of the number of spermatic cells and volume of sperm was demonstrated. The reduction of spermatic cells was followed by a simultaneous increase of FSH and LH levels, whereas the testosterone level did remain normal.
