TITLE: The sinus reaction in the axillary nodes in breast cancer related to tumour size and nodal state.
PUBDATE: 19821101
The sinuses of the axillary nodes were studied in 59 breast cancer patients. Three types of reaction were defined in them; sinus histiocytosis, a mixed sinus reaction, and sinus catarrh. Sinus histiocytosis, as defined here, was virtually confined to the mode-negative cases; sinus catarrh, common to both node-negative and positive, but increasing with tumour size in the former. It is suggested that transition may occur from sinus histiocytosis, via the mixed reaction, to sinus catarrh, and that the establishment of the latter may facilitate tumour spread.

PUBDATE: 19850101
A sensitive endogenous enzyme immunoassay involving an anti-human placental alkaline phosphatase (PLAP) monoclonal antibody was used in the screening of sera and tumor extracts of patients with various types of cancer. In sera of breast cancer patients an incidence of 5.2% was recorded. This value rose to 43% when tumor extracts were analyzed. For lung and bronchial cancers we found 11.2% seropositive patients. On several occasions a good correlation was observed between the PLAP determinations and the histopathological staging of tumor tissue.

TITLE: Serum selenium in patients with ovarian cancer during and after therapy.
PUBDATE: 19840601
Serum concentrations of selenium were determined by atomic absorption spectrophotometry in 40 patients with ovarian cancer in association with and after surgical and cytostatic therapy. Patients with ovarian cancer had significantly (p less than 0.001) lower serum concentrations (mean +/- SE) of selenium (0.93 +/- 0.04 mumol/l) than age-, weight- and place of residence-matched control subjects (1.22 +/- 0.03 mumol/l). In clinical stage IV disease there was a lower serum level of selenium (0.82 +/- 0.07 mumol/l) than in clinical stages I and II combined (1.00 +/- 0.04 mumol/l). Serum selenium concentrations also showed a tendency to follow the outcome of the disease; an increase in patients with remission and a decrease in patients with progressive disease, probably because of nutritional reasons.

PUBDATE: 19831201
The possibility of a cancer risk associated with fluoridation of public water supplies is re-examined using mortality data for 35 US cities, 20 with fluoridated water supplies and 15 with non-fluoridated water. Crude cancer death rates, and mortality ratios standardized for age, sex, and ethnic group are examined, using four alternative sets of standard rates and three different combinations of pericensal years' deaths. Changes in cancer mortality before and after fluoridation in the fluoridated cities are compared with changes in the non-fluoridated cities over the same time period. In none of the analyses have differences in mortality trends been found that could not be due to chance alone. Thus, these results do not support the suggestion of an association between fluoridation of water supplies and cancer mortality.

TITLE: Effect of somatostatin on exocrine pancreas evaluated on a total external pancreatic fistula of neoplastic origin.
PUBDATE: 19830701
A patient with a pancreatic fistula, with a defunctionalized duodenum (subsequent to a previously performed Billroth II gastrectomy for cancer and choledochojejunostomy for metastatic infiltration in the bile duct) was submitted to prolonged synthetic somatostatin infusion (48 h). There was a mean reduction of 80% in pancreatic output, with no changes in the composition of sodium, potassium, and calcium, an increase in chloride and a decrease in bicarbonate. An increase in amylase was noted. The unique clinical features of this case have provided direct confirmation of the inhibiting action of somatostatin on the exocrine pancreas.

Peripheral blood specimens were obtained from 50 patients with various stages of breast cancer (I-II = 7, III = 6, IV = 24, treated and NED = 13), and 20 biochemical tests were performed. There are significant differences of hemoglobin, LDH, SGPT, serum protein, albumin, and alpha globulin values between patients with early (I, II, NED) and late (III, IV) lesions. Among patients with stage IV diseases, those patients with bony metastases had significantly higher values of alkaline phosphatase, alpha-1 globulin, IgA, and C-reactive protein than those with nonosseous lesions. Neither CEA nor pregnancy-associated alpha-2 glycoprotein showed any correlation with different stages or sites of breast cancer in these small series of patients.

PUBDATE: 19840401
Two cases of carcinoma of the urachus are reported, 1 involving the dome of the bladder and 1 occurring beneath the peritoneum of the anterior abdominal wall below the umbilicus at the apex of an elongated bladder. Both patients died shortly after diagnosis, a fact which emphasizes the poor prognosis of this group of patients. The poor prognosis and need for more than local resection of the tumour may not be widely appreciated. Tumours arising above the bladder have a different mode of presentation from those arising within the bladder wall. Bladder wall tumours are most frequently mucus-secreting adenocarcinomas; tumours arising above the bladder may be more variable in their histological type, and include squamous and undifferentiated cancer. A simple classification into vesicle and supra-vesical tumours is proposed, to replace an early classification into 7 groups.

Psychiatrists diagnose and treat the mental disorders of cancer patients, but they are also concerned with pain management, the patient's adaptation to the disease, and the attitudes of relatives and health care professionals toward the cancer patient. The author reviews recent research on these areas of concern and discusses the psychiatric complications of cancer such as organic brain syndromes, depression, and anxiety; the pharmacotherapy of such complications; and the role of psychological factors in the etiology of cancer. Approaches to informing patients of their cancer diagnosis and other implications for clinical practice are also covered.

TITLE: Carotid sinus hypersensitivity: a cause of syncope in patients with tumors of the head and neck.
PUBDATE: 19831001
Carotid sinus hypersensitivity secondary to the presence of an underlying head and neck cancer has only recently been discussed in the literature. We present seven cases of carotid sinus hypersensitivity, each with paroxysmal bradycardia and hypotension associated both with and without palpation of the carotid bifucation. A discussion of the anatomy and the etiology of the hypersensitivity is given. Currently available therapeutic modalities are discussed in regard to their efficacy in this study. Carotid sinus hypersensitivity secondary to tumor in the neck is most often vasodilatory in nature and hence does not respond well to transvenous pacemaker. The best treatment for severe hypersensitivity appears to be periarteriectomy or intracranial nerve root section. Radiation therapy is occasionally beneficial.

PUBDATE: 19850501
Neoplasms of the canine and feline alimentary tract represent diagnostic and therapeutic challenges for the veterinarian in practice. Careful attention to historical details and thorough and repeated examination are of paramount importance in evaluating animals with chronic digestive disturbances. By judicious use of ancillary studies such as plain and contrast radiography, endoscopy, and exploratory surgery, the clinician may arrive at a diagnosis earlier in the course of a neoplasm. This in turn may facilitate more efficacious treatment. A particularly interesting aspect of tumors of the gastrointestinal tract is their frequent association with environmental carcinogens. By being aware of these relationships, veterinarians stand a better chance of reaching the ultimate goal of cancer therapists--the prevention of malignant disease.

In cancer therapy, the authors have attempted to decrease the side effects. Since 1975 they have used immuno-thermo-chemotherapy(so-called ITC therapy). At this time the ITC therapy is used in combination with induced-hypertension-chemotherapy (IHC therapy). This new approach has been applied to patients with terminal or advanced malignancy, which have been unsuccessfully treated by conventional ITC therapy. This report compares the clinical effects of conventional ITC therapy those of new modality in the same patient. The clinical results are 6 cases of I-A and one case of I-B according to Karnofsky's criteria. This New modality is more effective than the conventional ITC therapy in all 7 patients.

The autopsy findings of 100 patients who had a resection for large-bowel cancer and who were classified by a clinicopathologic staging system were analyzed. Seventy patients had carcinoma at autopsy. The value of this method of staging was assessed by relating the stage at operation to these findings. Forty-five per cent of early stage tumors (A or B) were found to have metastases. If the detection rate of occult metastases could be improved for stage A and B tumors, this might increase the prognostic accuracy of this staging system.

A total of 1986 cases of primary lung cancer, observed in the Pathology Department of the University of Florence during 1971-1981, were analysed by age, sex, year of diagnosis, source of specimen, and cell type. Adenocarcinomas occurred more frequently in females and at younger ages and were diagnosed mainly from surgically obtained specimens. In contrast, squamous cell carcinomas developed more often in males and at older ages, and were chiefly diagnosed by bronchial biopsy. Over the 11-year period of study, a significant increase in the percentage of squamous cell carcinomas and a consequent decrease in the percentage of adenocarcinomas occurred both in females and in males, independent of changes in the specimen sources.

The false negative rate of the stabilized guaiac faecal occult blood tests, Fecatest and Haemoccult, was determined in 61 patients with known colorectal cancer. When subjects tested their stools over three days, 9.7% of the Fecatest slides and 27.8% of the Haemoccult slides were negative (p less than 0.02). Extending the number of days of testing from three to six reduced the false negative rate of Fecatest to 4.8% and Haemoccult to 9.7% (p less than 0.05). There was no significant difference between the false negative rate of Fecatest and Haemoccult test when subjects performed six-day testing. Haemoccult false negative results occurred mainly in patients with well differentiated left sided carcinomas.
