TITLE: [Staging of patients with carcinoma of the breast by bone scans? [author's transl)]

Bone scans were performed in 170 patients with widespread breast cancer and metastases which were confirmed by the clinical course. In 82% a positive x-ray and in 95% a positive bone scan could be demonstrated. In 43 patients without bone metastases the ratio of false positive scans was 5%. Seventeen out of 50 patients with early cancer had abnormal scans. In 12 cases there were metastases proven by follow-up of at least 18 months. Assuming an incidence of 25% for bone metastases in patients with early breast cancer, bone scans with a sensitivity and specificity of 95% will be valid in staging. To validate this high incidence we investigated a second group of 97 patients with early cancer in a prospective study: in only 3 cases there was an abnormal scan. Using this low incidence bone scanning is calculated to be not valid for staging of breast cancer.

TITLE: [Direct radioimmunological determination of plasma CEA in 1704 patients]

An original technique for the direct radioimmunological measurement of plasma CEA has been prepared. Compared to the other in use, this has the advantage of a very low incidence of false positivity. 1704 patients make up the series: the percentage of positivity in 247 suffering from gastroenteric adenocarcinoma was 66.8%, in 60 with lung cancer 30%, in 243 with malignant tumours in various sites 7.4%, in 199 with chronic liver diseases 29.6% while in 598 cases of other non-neoplastic diseases it was only 2.5%. 212 patients who underwent radical surgery for gastroenteric adenocarcinoma were also followed up and the test provided early diagnosis of the clinical re-onset of the neoplastic lesion. It is considered that plasma CEA measurement represents a useful aid to the clinician althouth it cannot be employed at present to screen gastroenteric adenocarcinomas.

TITLE: Combination chemotherapy in advanced ovarian cancer.

Among the 95 cases of ovarian cancer treated by us between May, 1975 and August, 1978, only 33 were suitable for complete resection. The remaining 62 cases underwent incomplete resection, followed by F.Q.C. combination chemotherapy (1-(2-tetrahydrofuryl)-5-fluorouracil, carbazilquinone, cytosine arabinoside); 54.8% showed an antitumor response. The median survival time of the responders was 13.2 months, whereas it was 7.4 months for the non-responders. The survival rate after 24 months, however, was 13% and 4%, respectively. Side effects of the drugs, including leucopenia, thrombocytopenia, nausea and vomiting, were found in roughly one half of the cases.

TITLE: Observations on the medical efficacy of computed tomography of the chest and abdomen.

In a retrospective study of 687 computed tomographic (CT) examinations of the chest and abdomen performed over a 1 year period, 16% of the examinations produced information not otherwise available which altered patient diagnosis, prognosis, or therapy. Sixteen thoracotomies and 18 laparotomies were avoided as a result of information available only from CT examinations. CT was of particular value in the evaluation of mediastinal masses, in the assessment of suspected or proven lung cancer, and in the evaluation of pleural and extrapleural masses. It was of notable clinical value in evaluating patients with intraabdominal abscesses, pancreatic disease, staging of malignancies for radiation therapy and surgery, and excluding the presence of suspected masses.

Horizontally transmitted viruses have been etiologically linked to leukemia and lymphoma in several higher mammals (gibbons, cows, and cats). In the best-studied example, the cat, feline leukemia virus is a common community-acquired virus that infrequently produces cancer. However, the infection and its complications (cancer) are not typical of infectious diseases. Epidemilogically, serologically, and virologically, infections with this type of virus can contradict classic infectious disease dogma. Therefore, previous studies that failed to link characteristics of infectious diseases (epidemiologic, serologic, or virologic) to human cancer must be cautiously interpreted. A link between retroviruses and/or DNA viruses and human lympho-reticular malignancies is hypothesized, and it is suggested that systematized nation-wide studies of selected cancer patients and their contacts be executed with the most sensitive and specific laboratory probes available.

TITLE: Skin cancer in the PUVA-48 cooperative study.

A 4-year follow-up study of 631 patients with psoriasis given 8-methoxypsoralen and UVA in the PUVA-48 cooperative study revealed only ten patients with skin cancer and eight patients with actinic keratosis. The skin cancer group averaged 14 years older than the group without lesions. The cumulative treatment ranged from 255 to 1,737 joules/cm2 at the time of development of skin cancer. The ratio of basal cell carcinoma to squamous cell carcinoma and the anatomic location of lesions generally follow the pattern of spontaneously developing lesions. Ionizing radiation was a significant risk factor, but previous arsenic exposure or skin cancer was not. Comparison with previous studies and lack of control population may indicate less risk of skin cancer following PUVA than previously suggested.

TITLE: Tumor-associated antigens in polyps and carcinoma of the human large bowel.

The presence of a number of tumor-associated antigens was studied in eight metaplastic polyps, 22 tubulovillous adenomas, and 20 carcinomas. Specific tumor antigens were identified using the immunohistochemical (P.A.P.) technique to detect carcinoembryonic antigen (CEA), human placental lactogen (HPL), alphafetoprotein (AFP), colon-specific antigen (CSA), pregnancy-specific beta lipoprotein 1 (SP1), human beta chorionic gonadotropin (beta hCG), and placental alkaline phosphatase (P Alk P), isoferritins (FE), and transferrin (TF). There is no difference in either the number of antigens present or the number of cases positive for each antigen in cancers and tubulovillous adenomas, but the majority of metaplastic polyps show only CEA and HPL positivity. The two metaplastic polyps showing a full range of positivity were atypical and over 5 mm in diameter. The findings have shown a remarkable similarity between polyps and cancer, which strengthens the concept of the relationship between adenomatous polyps and carcinoma of the colon.

Investigators who have studied hyperthermic response of spontaneous animal tumors have reported complete remission for variable periods of 15 to 38% of tumors treated. Normal tissue complications were minimal. Long-term control appears more likely if irradiation is combined with hyperthermia, but information is lacking to date to confirm this. Dose-response assays of radiation alone have been done which would make comparisons with hyperthermia and radiation more meaningful. Probabilities for increasing tumor control without significantly increasing normal tissue response can be estimated better from such assays. Spontaneous tumors in companion animals have advantages over rodent tumor systems of relatively larger treatment fields; longer follow-up times are possible, and serial monitoring of a variety of clinical data can be done. Experience of investigators using these tumors has shown that animal owners and referring veterinarians are most cooperative in reasonable, humane approaches to experimental cancer therapy.

TITLE: Racial survival patterns for lung cancer in Hawaii.

From 1960 through 1974, the Hawaii Tumor Registry identified 1895 cases of lung cancer with either small cell or non-small cell carcinoma among the five main racial groups in the islands. There were 650 Caucasians, 549 Japanese, 362 Hawaiian, 173 Filipino and 161 Chinese patients. Analysis revealed that Caucasians with non-small cell carcinoma had a significantly poorer five-year survival rate than Chinese, Filipino, or Japanese patients after simultaneous adjustment for differences in sex, age at diagnosis, stage, and socioeconomic status. Men, older patients, patients with regional or distant disease, and patients in a middle or low socioeconomic status also did worse than the others. When the 170 patients with small cell carcinoma were compared with 1725 patients with non-small cell carcinoma, patients with small cell carcinoma had significantly poorer survival rates after five years.

To assess the role of biologically active estradiol in the development of endometrial cancer, 25 patients with endometrial tumors and a similar number of control subjects matched for age and body size were studied. No differences between the 2 groups were found for levels of total estradiol, sex hormone-binding globulin (SHBG), non-SHBG-bound estradiol, and absolute free estradiol. Body size correlated positively with levels of total, non-SHBG-bound, and absolute estradiol and negatively with SHBG levels. The obese postmenopausal women had higher total circulating levels and proportionally greater concentrations of free estradiol than nonobese subjects, suggesting a dual risk for the cellular action of circulating estradiol. These factors could contribute to the association of obesity and the occurrence of this tumor in susceptible women.

TITLE: The management of disseminated testicular cancer.

Fourteen patients with disseminated non-seminomatous germ cell tumours of the testis were treated with a 3-drug combination of vinblastine, bleomycin and cis-diamminedichloroplatinum. Of 13 patients with measurable disease, 9 (69%) had complete and 3 (23%) partial remission. One patient with a solitary metastasis failed to respond but is disease-free following surgical excision. Toxicity was significant but no drug-related deaths occurred and there were no apparent lasting side effects. No patient attaining complete remission has relapsed. Eleven (79%) of the 14 patients are without evidence of disease from 3+ to 36+ months, 6 of these for 2 years or more. Of the patients in partial remission one died at 16 months and one is undergoing further treatment for relapse with liver metastases. One patient remains in partial remission at 30 months. These results demonstrate the efficacy of this drug combination in testicular cancer.

Of 150 consecutive patients with resectable bronchogenic carcinoma, 11 were found to have multiple primary tumours of the respiratory tract. Five patients had more than one primary tumour of the lung and six had previously received treatment for carcinoma of the larynx. In the same group of 150 patients, 14 had one or more cancers involving other organs. The appearance of a new solitary lung lesion in a patient with another cancer demands special attention to determine the exact diagnosis and to decide upon the best method of treatment. Physicians should be aware that such a lung lesion may represent a new primary lung cancer rather than a metastasis. The criteria are defined for the pathological definition of two separate primary tumours in these cases.

One hundred and fifty-eight cases of paranasal sinus cancer have been reviewed retrospectively. The actuarial survival at five years for the whole group was 33%. The American Joint Committee (AJC) classification for maxillary cancer was used. T1 and T2 cases, with an actuarial survival of 44% at five years, had a significantly better prognosis than T3 cases (31%) and T4 cases (10%). Anemia of presentation and the presence of persistent disease when surgery followed pre-operative irradiation were poor prognostic indicators. The policy of combined pre-operative irradiation followed by surgery is advocated; patients with anemia should have this corrected before and during treatment.

We designed a slim gold-grain implanter with adaptable lengths to implant areas accessible only through long, narrow, examining instruments, such as a suspension laryngoscope. The implanter is loaded with the same 14-grain magazine designed for and supplied with the Royal Marsden "gun." The simplicity of the loading mechanism with a minimum of moving parts makes the instrument practically trouble free. Although it is designed to be used along narrow examining instruments, it can also be used in any situation in which a permanent implant is required, for instance, prostatic cancer and pelvic recurrences in cancer of the uterine cervix previously treated by external and intracavitary irradiation.

The tissues from 30 cases of endometrial cancer and 44 cases of cervical cancer were examined for oestrogen receptor activity. Twenty of the endometrial and 9 of the cervical tumours contained oestrogen receptor levels above 4 fmol/mg protein. The proportion of oestrogen receptor-positive tumours was significantly greater in adenocarcinomas of the cervix than in squamous carcinomas of the cervix. Tissues from 3 mixed mesodermal tumours of the uterus, 2 carcinomas of the vagina, a carcinoma in situ of the cervix and a carcinoma in situ of the endometrium were receptor-negative. One ovarian carcinoma and a single case of uterine sarcoma were receptor-positive. The implications of these findings in relation to hormonal therapies are discussed.
