As a serious side effect of immunosuppressive therapy following organ transplantation, the incidence of malignancies shows a significant threefold increase compared with the age-matched general population. Under treatment with conventional immunosuppressive drugs (azathioprine, steroids and antilymphocyte globulin) patients are supposed to be susceptible to skin and lip cancers (40%) whereas the recently introduced new immunosuppressive drug cyclosporine A shows a comparable incidence of non-Hodgkin lymphomas (41%) and the patients seem to develop more potentially life threatening tumours of internal organs. In the present paper we report on a case of laryngeal carcinoma following immunosuppressive treatment with cyclosporine A 10 months after heart transplantation.

TITLE: Variation in recognition of recurrent colonic cancer by different CEA assays.

Forty-six patients were followed with serial CEA determinations by two CEA assays after curative resection of a primary or recurrent colonic cancer. Thirteen have had a histologically proven recurrence, of which only six (46 percent) were predicted by both assays, while seven (54 percent) were predicted by one assay only. The assays appear complementary in indicating tumor recurrence, and preliminary findings suggest that it may be beneficial to follow patients with more than one CEA assay after resection of a primary or recurrent colonic cancer.

TITLE: The sinus reaction in the axillary nodes in breast cancer related to tumour size and nodal state.

The sinuses of the axillary nodes were studied in 59 breast cancer patients. Three types of reaction were defined in them; sinus histiocytosis, a mixed sinus reaction, and sinus catarrh. Sinus histiocytosis, as defined here, was virtually confined to the mode-negative cases; sinus catarrh, common to both node-negative and positive, but increasing with tumour size in the former. It is suggested that transition may occur from sinus histiocytosis, via the mixed reaction, to sinus catarrh, and that the establishment of the latter may facilitate tumour spread.

Normal, adenoma and cancer cell nucleoli have been studied by silver impregnation in 13 cancer patients, 48 patients with adenoma (14-tubular, 35-tubulovillous and villous) and 15 controls. It has been shown that the content of silver granules per cell was significantly higher (34.0 +/- 9.7) in cancer cells than in the control (12.3 +/- 3.1). Adenoma cells were heterogeneous. Most villous adenomas, like adenocarcinomas, had high content of Ag granules per cell. The importance of these findings for theoretical and clinical oncology is discussed.

TITLE: Screening of sera and tumor extracts of cancer patients using a monoclonal antibody directed against human placental alkaline phosphatase.

A sensitive endogenous enzyme immunoassay involving an anti-human placental alkaline phosphatase (PLAP) monoclonal antibody was used in the screening of sera and tumor extracts of patients with various types of cancer. In sera of breast cancer patients an incidence of 5.2% was recorded. This value rose to 43% when tumor extracts were analyzed. For lung and bronchial cancers we found 11.2% seropositive patients. On several occasions a good correlation was observed between the PLAP determinations and the histopathological staging of tumor tissue.

A model of cell division as it occurs in the neoplastic cell is presented. The two basic features of the model are that firstly cell division is autonomous and secondly it is incomplete because not all of the processes which take place during normal division occur in the disordered division of neoplasia. As a result cellular structures may not be replicated along with the rest of the cell and they are therefore deficient. The deficiency of particular structures is then used to explain certain observations made of neoplastic cells eg. disturbances of metabolism and morphology, heterogeneity of the cell population, malignancy and immortalisation. Oncogenes are then introduced into the model in an attempt to understand their role in the development of cancer.

This report is a retrospective analysis of 62 patients treated for early glottic carcinoma during the six-year period of January 1982 through December 1987. The authors report that the cancer has been controlled in 100% of patients. For invasive stage 1 tumors the radiation cure rate is 42 of 45 patients or 93%, and voice quality after treatment was usually normal. Surgically treated tumors are usually confined to one vocal cord. Primary surgery with laser or cord stripping ultimately led to tumor control and voice preservation in all eleven patients treated although two required salvage with radiotherapy and mild but acceptable hoarseness was a usual sequel.

A cohort study of 3358 power linesmen and 6703 power station operators classified in the 1960 population census in Sweden was undertaken. The cancer incidence between 1961 and 1979 was calculated in relation to all male blue collar workers. The relative risk of cancer (all sites) was 1.1, (1.0-1.2, 90% confidence limit) in the group of power linesmen and 1.0 (0.9-1.0) for power station operators. No consistent excess risk of leukaemia or brain tumour was found in the two occupational groups. In both groups a tendency towards an excess risk of cancer of the urinary organs was found.

TITLE: Serum selenium in patients with ovarian cancer during and after therapy.

Serum concentrations of selenium were determined by atomic absorption spectrophotometry in 40 patients with ovarian cancer in association with and after surgical and cytostatic therapy. Patients with ovarian cancer had significantly (p less than 0.001) lower serum concentrations (mean +/- SE) of selenium (0.93 +/- 0.04 mumol/l) than age-, weight- and place of residence-matched control subjects (1.22 +/- 0.03 mumol/l). In clinical stage IV disease there was a lower serum level of selenium (0.82 +/- 0.07 mumol/l) than in clinical stages I and II combined (1.00 +/- 0.04 mumol/l). Serum selenium concentrations also showed a tendency to follow the outcome of the disease; an increase in patients with remission and a decrease in patients with progressive disease, probably because of nutritional reasons.

Twenty cases of plantar melanoma treated at Ellis Fischel State Cancer Center (EFSCC) since 1940 are analyzed to determine the clinical and histologic features of prognostic significance. The EFSCC experience confirms and extends the observations made in other recent studies of plantar melanoma. This clinical pathologic study suggests that one of the histologic patterns, the lentiginous growth pattern, shares some biologic features with lentigo maligna, namely the relatively indolent growth phase. Plantar melanomas may remain latent for a number of years making these patients good candidates for therapeutic cures if detected early.

The possibility of a cancer risk associated with fluoridation of public water supplies is re-examined using mortality data for 35 US cities, 20 with fluoridated water supplies and 15 with non-fluoridated water. Crude cancer death rates, and mortality ratios standardized for age, sex, and ethnic group are examined, using four alternative sets of standard rates and three different combinations of pericensal years' deaths. Changes in cancer mortality before and after fluoridation in the fluoridated cities are compared with changes in the non-fluoridated cities over the same time period. In none of the analyses have differences in mortality trends been found that could not be due to chance alone. Thus, these results do not support the suggestion of an association between fluoridation of water supplies and cancer mortality.

A patient with a pancreatic fistula, with a defunctionalized duodenum (subsequent to a previously performed Billroth II gastrectomy for cancer and choledochojejunostomy for metastatic infiltration in the bile duct) was submitted to prolonged synthetic somatostatin infusion (48 h). There was a mean reduction of 80% in pancreatic output, with no changes in the composition of sodium, potassium, and calcium, an increase in chloride and a decrease in bicarbonate. An increase in amylase was noted. The unique clinical features of this case have provided direct confirmation of the inhibiting action of somatostatin on the exocrine pancreas.

Peripheral blood specimens were obtained from 50 patients with various stages of breast cancer (I-II = 7, III = 6, IV = 24, treated and NED = 13), and 20 biochemical tests were performed. There are significant differences of hemoglobin, LDH, SGPT, serum protein, albumin, and alpha globulin values between patients with early (I, II, NED) and late (III, IV) lesions. Among patients with stage IV diseases, those patients with bony metastases had significantly higher values of alkaline phosphatase, alpha-1 globulin, IgA, and C-reactive protein than those with nonosseous lesions. Neither CEA nor pregnancy-associated alpha-2 glycoprotein showed any correlation with different stages or sites of breast cancer in these small series of patients.

In the course of administration of an immunopotentiator, streptococcal agent OK-432, a patient with lung cancer was found to have severe anemia and a strongly positive antiglobulin test. In the absence of the drug, eluates from the patient's erythrocytes reacted strongly by the indirect antiglobulin test with a panel of washed Group O erythrocyte samples. The eluated antibody was composed of monoclonal IgG1-lambda. Upon discontinuing the drug the patient's hemoglobin level increased slowly with concomitant normalization of the reticulocyte count. Although very rare, drug-induced immune hemolytic anemia should be included in the list of adverse effects of immunopotentiators. This is the first reported case of immune hemolytic anemia associated with OK-432.

Two cases of carcinoma of the urachus are reported, 1 involving the dome of the bladder and 1 occurring beneath the peritoneum of the anterior abdominal wall below the umbilicus at the apex of an elongated bladder. Both patients died shortly after diagnosis, a fact which emphasizes the poor prognosis of this group of patients. The poor prognosis and need for more than local resection of the tumour may not be widely appreciated. Tumours arising above the bladder have a different mode of presentation from those arising within the bladder wall. Bladder wall tumours are most frequently mucus-secreting adenocarcinomas; tumours arising above the bladder may be more variable in their histological type, and include squamous and undifferentiated cancer. A simple classification into vesicle and supra-vesical tumours is proposed, to replace an early classification into 7 groups.
