I also looked at the database and could not find anything unusual.  The rotation district is correctly set to 'District', not 'In District', and the start rotation values defined in the District Editor are default, which is correct.  I reset ballot artwork, re-generated artwork as well as ballot rotations in 1-17-15 as well as 1-18-4, and got rotations 0, 2 and 1, as opposed to the original sequence.  In any case, the proper sequence should be 0, 1 and 2.

change your rotation to IN DISTRICT - reset regen and the ballots are fine
