The second part explains that you are helping us to evaluate our software by using it, and you can stop participating in this evaluation by uninstalling the software.

Research Participant Information and Electronic Consent/Authorization Form

Study Title: TopeSys Utility and Usability Evaluation

The purpose of the study is to evaluate a system that helps you find typos and reformat textual data. You will be able to use your computer to teach our system how to recognize different kinds of data, such as phone numbers, street addresses, and person names. Then, you will be able to use the system to find and correct typos in your spreadsheets, web applications, or web macros.

This system was designed to respect your security and privacy. It will not automatically modify any of your data without your direction. Although it will log occasions when you teach the system how to recognize a kind of data, it will not automatically log your data values. For example, if you teach the system how to recognize phone numbers, then the system would record this teaching event, including what a phone number looks like. However, it will not automatically log any phone numbers in your data. Therefore, all data logged automatically during this time will be completely anonymous, though you also will have the choice to explicitly share non-anonymous feedback and other data with the researchers and other users.

There will be no monetary cost to you if you participate in this study. The main risk of this study is that the software could possibly malfunction or damage some data, requiring you to uninstall it. You will not receive any monetary compensation for your participation in this study. The main benefit of this study is that it may save you time by helping you to find and correct typos. In addition, the knowledge received may be of value to humanity.

Your participation is voluntary. Refusal to participate or withdrawal of your consent or discontinued participation in the study will not result in any penalty or loss of benefits or rights to which you might otherwise be entitled. The Principal Investigator may at his/her discretion remove you from the study for any of a number of reasons. In such an event, you will not suffer any penalty or loss of benefits or rights which you might otherwise be entitled.

During data analysis and publication/presentation of results, your anonymity will be maintained by any or all of the following means: (1) You will be assigned a number, and names will not be displayed. (2) The researchers will save the data file and/or any video or audio recordings by your number, not by name. (3) Only members of the CMU Natural Programming research group and the EUSES Consortium will view collected data in detail. (4) Any recordings or files will be stored in a secured location accessed only by authorized researchers.

This study is funded by the National Science Foundation, which is supporting the costs of this research. Neither Carnegie Mellon University (CMU), nor Professor Brad Myers will receive any financial benefit based on the results of the study.
