If you simply want to be able to listen to a file from start to finish,
you can use the TextAssist software that comes with the ASP versions of
the Soundblaster 16.  This provides Dectalk-quality speech. 

If you need to be able to move around in documents, or if you need to 
speak information that can't be copied to the clipboard (such as menus, 
dialog box prompts, etc.) then you'll need to use a screen reader like 
Window Bridge from Synthavoice, Protalk from Biolink, WinVision from 
Artic or Outspoken from Berkeley Systems (of After Dark fame).
