I found another reference that I have actually looked at.

Digital Coding of Waveforms: Principles and Applications to Speech and
Video, N.S. Jayant, Peter Noll, 1984, Prentice-Hall, ISBN
0-13-211913-7.

An encyclopedic book with heavy analysis, but also a lot of good
charts and intuitive information.  Appendix E is on objective
measurement of coder performance, and Appendix F is on subjective
measures.  They mention but do not have details, however, about
objective measures that model (subjective) auditory perception.
