There's always been an additional assumption as well.  If reply's to the
postmaster of a given organization don't result in useful corrective action, you
then move "up stream" of the posting one level and ask that administator(s) to
filter and/or cut their feed/connection.  I.e. if they won't police themselves,
prune them from the net.

This has always been the principle.  Isolate abusers into their own little
subpockets.  It doesn't results in quick action or corrections, but over the
long run specific problems get solved eventually and the effect of the principle
is felt generally.

The problem now is that points of connection are everywhere, and the ISPs don't
police them like the sysadmin's of old (who were predominantly either military
contractors or university staff).  So the abusers can simply float across
marginal ISPs that may or may not even be in business tomorrow.
