To the best of my knowledge, you can not currently get a comercial system 
which meets your requirements.

Three companies make dictation systems which allow you to dictate text on 
IBM PC systems.  DragonDictate for Windows, which I am most familiar 
with, allows you to completely control your computer by voice including 
generating any keystrokes and also moving the mouse by voice.  This is 
on top of a dictation vocabulary of up to 60,000 words at a time.  

The three PC dictation programs are listed below.  All of these systems 
are discrete speech (you must pause briefly between words and commands:

DragonDictate from Dragon Systems (1-800-TALK-TYP or 617-965-5200)
Kurzweil VOICE from Kurzweil AI
IBM VoiceType Dictation from IBM

There are also a handful of PC based programs which will allow you to 
control your computer using continuous speech.  These programs are 
usually based on a limited context-free grammar.  None of the continuous 
speech command and control programs that I have seen will allow you to 
either dictate text or completely control your computer by voice.
