Build my TC211 camera, the design has been finished just about a
week ago,
it's been designed for astronomical purposes but can easily be
adapted to your needs.
Plans including PCB layout are available via FTP on ftp.wwa.com in
pub/ccd/CCD211***.ps.gz.
Uncompress them with Gnu Z and kick them into a postscript printer. 
Some details: 192x165 pixels, 12 bits resolution digital output, 
clocking done by the computer through the interface, low power
(5V/200 mA), VERY small,
can be cooled if desired. For your purposes I suggest you change the
mechanical layout,
leave the cooling and heatsink away and make it even much smaller.
It's VERY sensitive,
so if you want to avoid image smearing due to readout, increase the
exposure time and 
put a gray filter in front of the chip. Advantage for your
application: increase the 
exposure time even more (uncooled up to about 20 seconds) and make
it seeing in almost
black darkness!
