MESA will sell you a 2-axis servo motor control card for your pc
for $259. qty 1. It comes with a buncha software, sourcecode, and
good documentation including schematics. 
It uses the National LM629 chip, quadrature goes in, PWM comes out.
also inputs for sense switches and what have you.
They also sell an amplifier designed to go with the board.
