I always thought it had something to do with the alleged practice of
shell-shocked WW1 soldiers of shooting themselves in the foot, as if by
accident and getting out of the trenches and into a nice safe hospital.

Hnce the meaning o something you harmful you do to yourself which
prevents you from carrying out your duties.
