For DOS/Windows based personal computers there are three commercial 
packages which allow large vocabulary dictation (speech to text).  The 
three are:

DragonDictate for Windows (and DragonDictate for DOS) from Dragon Systems 
of Newton, MA (1-800-TALK-TYP or 617-965-5200)

Kurzweil Voice for Windows from Kurzweil AI of Waltham, MA

IBM VoiceType Dictation for Windows (and IBM VoiceType Dictation for OS/2)
