There are a number of was. I use a Buffalo monitor in the HC11, and just 
send the S19 file directly to the port the HC11 is connected to, after
issuing a 'load t' command.

Another way, is to use the PC-Bug program, available from Motorola's BBS
at (512) 891-3733. I'm selling HC11 kits, and I have a diskette available
that has PCBug, ICC, IC and tiny4th on it.
