I recently came across a very good book on the topic, called "Stereo
Computer Graphics and Other True 3D Technologies," edited by David F.
McAllister.  It's published by Princeton University Press, copyright 1993,
Library of Congress number T385.S758.  It lists in a fairly comprehensive
manner all the main stereo-generation techniques in use today.
