Sorry to give you an answer as a non-expert, but as a user of
single word recognizers like Dragon I must say that they haven't
solved this problem in their products, either, and I don't think
you (or I) can do much better.  This might be the point where it
is necessary to include higher level logic like checking for correct
grammar or switching to a continous speech recognizer.
