We got 3 of them direct from VLSI Vision.  Excellent product.
It came in a small case about 1 inch cube with either wide angle
or narrower angle lens fitted (about another 3/4 inch length),
needed a 7 Volt power supply and gave out composite monochrome
CCIR video.

You can't get them from VLSI Vision any more, I believe, and they
cost a fair bit more from the distributor than they did from the
manufacturer (as might be expected).
