I assume that you reported the problem to OTI's support group in Phoenix?
I can't think of any reason off the top of my head as to why this might happen
if things are running smoothly otherwise.  Often this behaviour may result if
the lock/library access settings are incorrect but if you can normally access
the library and THEN it goes awry at a later stage then it honestly could be
anything.  The configuration you are running is definitely not uncommon and
is one of the standard configurations we test (and I run on in my day-to-day
work).  What is your server running, how many users do you have?
